Conditions of registration: Two (2) Day Care Places Date of last inspection 2nd March 2005 Brief Description of the Service: Green Gables is purpose built and suitably adapated and equipped and provides personal care and support for up to 28 service users with severe physical disabilities who are wheelchair users. It is located on the boundary of the village of Oakerthorpe within fairly short vehicular access to Alfreton town centre. Accommodation comprises of three units all named by service users who live there - Treetops and Fairways are single storey level access accommodation and The Snug, being the original building, is split level and provides both passenger lift and wheelchair access. Each unit has its own dedicated facilities, although there is also a central kitchen/catering facility. All service users are provided with single room accommodation with accessible en suites. The homes stated aims include that of enabling service users to take an active part in the management of the home and promoting their independence and right of choice. Services are designed to meet the physical, social and emotional needs of service users. The Registered Manager is supported by a team of care and hotel services staff and also via external management arrangements. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3, 4 & 5 Service users and their representatives (as appropriate) are provided with the necessary information about the home and its services, including the terms and conditions of their accommodation there. Service users who were able were involved in the assessment of their needs and in the formulation of their care plans. EVIDENCE: A number of service users were case tracked and discussion took place with them and/or their representatives about the arrangements for their admission to the home and their involvement in this process. The recorded care needs assessments of each of those service users were examined, together with associated care records, the format of which was under review. Discussions took place with staff regarding the meeting of service users needs and with service users and their representatives about the care they received. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 8, 9 & 10. Service users were properly consulted about the arrangements for their individual care and support and were able to participate in all aspects of the home and make meaningful decisions about their lives within a framework of risk management. EVIDENCE: The recorded care plans of service users case tracked were examined and their care discussed with them. Care was formulated in accordance with their recorded needs assessments and within a framework of risk management and care plans evidenced regular reviews. The Inspector discussed with staff the arrangements for and approaches to the care of service users. Their responses included the promotion of encouragement and support for service users whilst ensuring their safety and without unnecessarily compromising their independence and choice. Discussions with service users and their representatives about the approaches of staff and their relationships with them, together with the care and support they received were reflective of this. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 15, 16 & 17 There were satisfactory arrangements in place for the routine access of service users to a wide variety of social, educational and recreational activities of their choice, both within and outside the home. The Manager was taking active measures to try to increase spontaneous access to the local community for service users in accordance with their wishes. EVIDENCE: Discussions were held with service users and staff about the arrangements for activities/education, recreation and leisure, meals/mealtimes and also daily living routines. Records were also examined in respect of these. Feedback was generally very positive, particularly in relation to planned and organised educational social and recreational activities. Staff and service users felt that improvements could be made in terms of enabling service users to have greater access to the local community on a more spontaneous basis. The Manager was taking steps to review the arrangements for this and gave details of proposals in hand. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19 & 20. There were satisfactory arrangements in place to ensure that the health care needs of service users were met in consultation with them and outside health care professionals and in accordance with their risk assessed needs. EVIDENCE: The personal and support needs of service users case tracked were clearly identified within individual care plans, which were up to date and in accordance with their recorded needs assessments and personal preferences. There was evidence of the ongoing involvement of service users in the planning and review of their care and discussions with service users and their representatives (including the district nurse) indicated that their healthcare needs were being properly met. Records of the inputs of outside healthcare professionals were kept for individual service users, including both specialist inputs and also for the purposes of routine health care screening. The arrangements for the administration and management of the medicines of those service users case tracked were examined. The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 & 23 There were satisfactory arrangements in place to enable service users and their representatives to make a complaint or air their views and to protect service users from abuse. EVIDENCE: Suitable information was provided for service users and their representatives as to how to complain. Service users and their representatives spoken with knew how to complain. Feedback from them strongly indicated that any concerns and issues they had could be easily raised and were dealt with promptly and positively without the need to make a formal complaint. There was a suitable system in place for the reporting and recording of complaints/concerns raised. Staff spoken with was familiar with the home’s policies and procedures in relation to complaints and also adult protection and had received training in respect of these. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 25, 26, 27, 28, 29 & 30. Service users live in a homely, comfortable and attractive environment, which is suitably organised, equipped and adapted to meet their needs EVIDENCE: A full inspection of the premises was undertaken. The home was generally well maintained and was furnished, decorated and equipped to a good standard. Although there were some areas of repair/renewal plans for these were in hand in accordance with the arrangements for the routine maintenance and renewal in the home. Standards of cleanliness in the home were good. Service users spoken with were satisfied with their private and communal accommodation, which was adapted and equipped to suit their disability needs. The Manager had developed an action plan following a visit from the environmental health officer on 19/05/05, a large part of which had been achieved.
