What the care home does well

Greenways provides a home from home environment for residents. People living at the home regard the other residents, staff and management of the home as their friends. Residents, who are able, are encouraged to retain an independent lifestyle, accessing local communal facilities and maintaining relationships. Care plans are clear and provide staff with the information, which they need to meet the needs of the residents. The staff team treat residents as individuals and are able to assist them to maintain their dignity. Residents are supported and feel that they are well looked after and feel that staff work hard to provide them with a homely atmosphere to live in.

Date of last inspection 2nd June 2005 Brief Description of the Service: Greenways is a care home registered to provide personal care and accommodation for up to 15 older people.The property is detached and is situated in a residential area close to the centre of Southwick. Shops and other local community facilities are nearby.The majority of private accommodation is arranged on two floors. The upper floor can be accessed by a passenger lift, however four bedrooms on the middle mezzanine floor can only be accessed by steps.The majority of rooms are for single occupancy and have en suite facilities.Communal space consists of a lounge/dining room and a conservatory. This announced inspection took place over 5.40 hours on the 18th October 2005. The inspector arrived at 9am and was greeted by the manager, staff and a few residents who were in the lounge after breakfast. A tour of the home took place; four residents were spoken with at length to find out what it is like to live at Greenways and others residents were spoken with in the privacy of their own rooms. During the visit the inspector spoke with one visitor, who confirmed that they were regular a regular visitor to Greenways and was satisfied with the level of care provided for their friend/relative living at the home. The care records of three residents were inspected during the visit along with other records, which showed how care needs are to be met. The records of three staff were also inspected and three staff were spoken with informally to discuss staffing levels and access to training. The manager was present for the inspection and was available to discuss the running of the home. Prior to the inspection 9 Service User comment cards and 5 Relative/Visitors comments cards were returned to the inspector a further comment card was handed to the inspector during the visit. All feedback from these comment cards was positive and made particular reference to the homely environment and caring staff team. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4 and 6 A Service User Guide provides information about the home and the service, which it offers to enable prospective residents to make an informed choice about moving into the home Residents are assessed prior to moving into the home to make sure that the home can provide a care plan which residents or their families know will meet their needs. EVIDENCE: The home has a new Service User Guide, which provides information about the care home. Prospective residents can use this information to ensure that Greenways can offer the service, which they require. Each resident has a contract/residents agreement, which sets out the terms and conditions of occupancy at the home. The manager was able to give an example of visiting a prospective resident in hospital on three occasions, to carry out an assessment of need. Care records viewed at this visit showed that an initial assessment is carried out prior to moving into the home. Greenways does not provide intermediate care but does offer periods of respite care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10,11 The care planning system gives clear information to assist with all aspects of health, personal and social care needs. Resident’s healthcare needs are being addressed by healthcare professionals to make sure that these identified needs are being met. Residents are treated with respect and their rights to privacy are recognised. EVIDENCE: The three care plans examined were clear and gave staff the information, which they need to meet the needs of the residents in all aspects of heath, personal and social care. They had been reviewed regularly to show the changing needs of the residents. Daily progress notes are in use to monitor the heath and well being of residents. Visits to residents by heath care professionals are recorded and outcomes detailed. Residents spoken with talked about visits to hospital appointments and some residents at the home are visited by district nurses. During the visit staff were observed to treat residents in a respectful and courteous manner. All nine residents returning comment cards to the inspector prior to the visit reported that their privacy is respected and relatives /visitors returning comment cards reported that they could visit their relative or friend in private if they wished. Quotes from relatives sending letters and cards of thanks for the care provided to their relative when they were dying include “ she was able to die with dignity in her own bed in the place she called home” and “she was shown the greatest respect, love and care”. Two staff have undertaken a training course to assist in coping with bereavement. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 The home provides a service, which enables residents to continue with a lifestyle, which suits their requirements, enabling residents to take part in social activities if they wish. Residents are encouraged to maintain contact with their family and friends so that they can satisfy their social and emotional needs. EVIDENCE: Residents spoken with felt that their relatives and friends are made welcome and said that they could see their visitors in the privacy of their own rooms. Comments cards returned from relatives and friends all indicated that the staff make them feel welcome when they visit and that they feel that they can visit at any time. Some residents are able to go out and continue their social life outside the home whilst others confine their social life to the home. Discussion with residents confirmed that the majority of residents felt that their social needs were being met but one resident said in their returned comment card that they would like “more Bingo”. Photographs of events and activities within the home are displayed on the notice board in the office. Residents are encouraged to manage their own money for as long as they wish and residents spoke about how their relatives “sort out their finances” One resident confirmed that they are able to continue looking after their own affairs by the assistance of an advocate who they have known for many years. One resident said that by having the chance to move to a larger room they had been able to bring items of furniture to the home, which has made the bedroom homely and comfortable. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 17and 18 The complaints procedure is clear and enables those using the service to have the confidence that their complaint will be responded to within a maximum of 28 days. Residents are protected by the homes adult protection procedure and training has been planned for staff to attend so that they are more aware of adult protection procedures. EVIDENCE: The complaints procedure is available in the Service User Guide and on display in the hallway, the conservatory and the office. It is clear and specifies how complaints should be made, with an assurance that any complaint will be responded to within 28 days. There have been no complaints made to the home or to the CSCI since the last visit. Residents spoken with were of the opinion that they are able to take part in the political voting process if they wanted by either using the postal voting system or attending the poling station in person with the assistance of a relative or advocate. Information about advocacy services is available in the Service User Guide. There have been no adult protection issues brought to the attention of the CSCI in the last year. The home has its own policies and procedures to safeguard residents from abuse in addition to the West Sussex Multi-Agency Policy for protecting vulnerable adults from abuse. Staff have been booked on formal adult protection training during the next few months. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25 and 26 Greenways is well maintained and provides a safe environment for residents to move around in. It is clean and residents have a comfortable, homely environment to live in. Comfortable bedrooms are provided which residents have personalised to make homely. EVIDENCE: The has been much improvement to the environment since the last visit. There is an ongoing schedule of improvements to décor. The entrance hall is much brighter after being redecorated and residents spoken with commented upon the redecoration of the lounge/dining room. Six bedrooms have been redecorated as part of the ongoing programme of redecorating bedrooms. The assisted bathroom downstairs has been redecorated along with the toilet near to the lounge. One residents who had changed rooms since the last visit commented upon how they liked their new room and being able to look out onto the road. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29,and 30 The staffing numbers are set at level, which allows residents assessed needs to be met. Recruitment procedures safeguard and protect residents at the home. Staff receive training to support them in carrying out their jobs and meeting the needs of residents. EVIDENCE: Staffing rotas observed prior to the visit show that sufficient staff are employed with the appropriate skill mix to meet the needs of residents over the 24-hour period. Staff confirmed that they felt that they are able to meet the needs of the current residents with the current staffing levels. Residents felt there are usually enough staff on duty. Relatives and visitors returning comment cards prior to the visit reported that they felt that there is sufficient staff on duty. Staff records show that 50 percent of care staff now have an NVQ level 2 or above which meets the 2005 target set. The staff files of three members of staff were fully inspected during the visit and other records were viewed to make sure that CRB and POVA clearance is sought for all staff. Records viewed at this visit were in good order and had evidence of identification. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,35,36,38 The home is well managed providing leadership and guidance for staff. Records required are constructed and maintained so that up to date information is available about residents to safeguard their best interests. Greenways provides an environment, which promotes the health, safety and welfare of residents and staff. EVIDENCE: Residents feel that the home is well managed. The registered manager has more than two years experience as a manager and has worked at the home for many years. She has completed an NVQ Level 4 in Care Management and continues to update her knowledge and skills by attending further training. Staff spoken with felt that the manager is approachable and gives them opportunities to make suggestions. Individual staff commented about how supportive the manager is in times of difficulty. A suggestions and comments book is available in the entrance hall for anyone to make entries. Six questionnaires were sampled at this visit all showing that residents were in the majority satisfied and that where they felt there was room for improvement they felt that they could make comment. Records show that residents meetings and staff meetings take place. All records viewed at this visit were in good order and up to date. A recommendation has been made that the external ramp from the lounge to the rear patio is either repaired or replaced.
