What the care home does well

Residents are offered choices in all aspects of daily living to promote independence and dignity. Care plans are clear and provide staff with the information, which they need to meet the needs of the residents. Residents are supported and feel that they are well looked after. Residents spoken with feel that staff work hard to provide a homely atmosphere for them to live in.

What has improved since the last inspection?

Some radiators have been fitted with covers to protect service users from scalding if they fall against them. The management and staff have made much improvement to the cleanliness and hygiene in the home.

What the care home could do better:

To ensure that the home is safe for people living there, risk assessments should be carried out for the remaining radiators not covered and the two windows on the upper floor, which do not have restricted openings. Residents should be reminded that they should not be held open unless they have self-closing devices that would be activated in the event of a fire.

Date of last inspection 9/12/04 Brief Description of the Service: Greenways is a care home registered to provide personal care and accommodation for up to 15 older people. The property is detached and is situated in a residential area close to the centre of Southwick. Shops and other local community facilities are nearby.The majority of private accommodation is arranged on two floors. The upper floor can be accessed by a passenger lift, however four bedrooms on the middle mezzanine floor can only be accessed by steps.The majority of rooms are for single occupancy and have en suite facilities.Communal space consists of a lounge/dining room and a conservatory Greenways Residential Home H60-H11 S14543 Greenways V221891 310505 Stage 4.doc Version 1.30 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This unannounced inspection took place over 4 hours on the 2nd June 2005. The inspector arrived at 1pm and was greeted by the two staff on duty and residents who had just finished their mid-day meal. A full tour of the home took place and all residents were spoken with individually or in a group. The care records of 3 residents were inspected during the visit along with other records, which showed how care needs are to be met. The manager was present for part of the inspection and was available to discuss the running of the home. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3,4,5 Prospective residents and their families are provided with the information they need to make an informed choice about the home. Residents are assessed prior to moving into the home to make sure that the home can provide a care plan which residents or their families know will meet their needs. Prospective residents, their relatives and friends have an opportunity to visit the home to look at the facilities available and assess its suitability before moving in for a trial period. EVIDENCE: The manager showed the inspector the Service User Guide, which is currently being updated to show changes in staffing and information about what is covered by the fee charged. During the tour of the home a Service User Guide was seen to be present in most bedrooms. Greenways Residential Home H60-H11 S14543 Greenways V221891 310505 Stage 4.doc Version 1.30 Page 9 One resident told the inspector that they had recently moved to the home and that a relative had visited the home on their behalf to have a look around and meet the staff. Another resident who has lived at the home for a number of years said that they had come to look at the home themselves with a friend. They had seen the room available and met the staff and other residents before deciding to move into the home. Care records viewed showed that assessments are carried out to make sure that residents needs can be met. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10, The care planning system gives clear information to assist with all aspects of health, personal and social care needs. Resident’s healthcare needs are being addressed by healthcare professionals to make sure that these identified needs are being met. The medication at Greenways is well managed, promoting good health. EVIDENCE: Care plans examined were seen to be regularly updated to show the changing needs of residents. They were clear and gave staff the information, which they need to meet the needs of the residents in all aspects of heath, personal and social care. Those records viewed showed that residents are involved in the drawing up and review of the care plan. A member of staff explained that the role of the keyworker included the regular review of care plans with residents. Records showed that the physical heath of residents is regularly monitored. All visits by Doctors and other heath care professionals are recorded and outcomes to visits detailed. Greenways Residential Home H60-H11 S14543 Greenways V221891 310505 Stage 4.doc Version 1.30 Page 11 A resident commented that they had just returned from a short stay in hospital that day and had been pleased that a meal was ready for them when they arrived home. Another resident also spoke of a short stay in hospital organised after the Doctor had visited. The resident also talked about regular visits from the chiropodist. Medication records, viewed by the inspector, were clear and fully completed. The medication was being stored in a safe lockable trolley and locked cupboards. Accident records examined during the visit were seen to be clear well documented. During the visit the inspector observed that staff spoke to residents courteously and promoted choice, dignity and privacy. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,and15 The home provides a service, which enables residents to continue with a lifestyle, which suits their requirements, enabling residents to take part in social activities if they wish. Residents are encouraged to maintain contact with their family and friends so that they can satisfy their social and emotional needs. Meals are well managed and offer home cooked food, which residents like. EVIDENCE: The inspector arrived at 1pm and joined eight residents who had remained in the dining room after dinner. The atmosphere was relaxed and the group of residents used this time to tell the inspector about the meal which they had just eaten. The meal had been sausage and gravy, cabbage, carrots and mash potatoes followed by rice pudding. Everyone had enjoyed the meal. The residents talked about the fact that home cooked food is offered and that if they don’t like what is on the menu they can ask for something different. Staff come round after the main meal at dinnertime and ask each resident what they want for tea. The inspector heard these choices being made when arriving at the home. Greenways Residential Home H60-H11 S14543 Greenways V221891 310505 Stage 4.doc Version 1.30 Page 13 Later a resident who had recently moved into the home said that they had “enjoyed the meals and it was nice to have someone else cook them”. For breakfast this resident “just likes toast.” Residents spoken with felt that their relatives and friends are made welcome and said that they could see their visitors in the privacy of their own rooms. Some residents are able to go out and continue their social life outside the home whilst others confine their social life to the home. One resident commented that they enjoyed the musical entertainers who occasionally visit Greenways. During the visit a resident was encouraged by staff to show other residents his talent of playing the piano. Residents have regular meeting which enable them to discuss matters concerning the running of the home affecting them. They use this as a means of exercising choice and control over their lives. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. JUDGEMENT – we looked at outcomes for standard(s) The complaints procedure is clear and enables those using the service to have the confidence that their complaint will be responded to within a maximum of 28 days. EVIDENCE: Greenways have a complaints procedure included in the Service User Guide. It is also on display in the conservatory and the entrance hall by the lift. There have been no complaints made to the home or to the CSCI since the last inspection. Residents told the inspector that if they had any complaints they could discuss them with the owner and they can also discuss issues at residents meetings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,23,24,25,and 26 Residents have a safe comfortable, homely environment to live in. Residents are encouraged to contribute to making their bedrooms their own by having their own personal possessions around them. The management and staff have made much improvement to the cleanliness and hygiene in the home. Residents now have a clean home to live in. EVIDENCE: The inspectors looked around the home visiting all the accommodation. Residents spoken with in their bedrooms appreciated the opportunity to bring personal items of furniture to the home. Most residents had on display their own photographs, paintings and ornaments that they have collected over a number of years. Residents confirmed that their rooms are cleaned regularly and it was observed that there had been a vast improvement to the cleanliness of the home since the last inspection. Some radiators remain uncovered and these are radiators, which the manger stated are not considered a risk to residents because of where they are situated or due to the fact that they are never turned on because of alternative heating. The Environmental Heath Officer visited on the 24th May 2005 and did not make any requirements at Greenways. The officer did discuss the matter of safety to the upper floor windows, giving advice that physical conditions such as height of the window, size of the window and accessibility should be taken into account when considering the risk of falling. Two windows on the upper floor were observed to be unrestricted during this visit by the inspector. The manger has agreed to carry out a risk assessment and put restrictors in place if there is a risk of residents falling from the window. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 The staffing levels and skills mix are sufficient to meet the assessed needs of residents. EVIDENCE: The rotas seen at this inspection showed that there are three care assistants on duty between 8am and 2 pm. Between 2 pm and 8 pm there are two care assistants. At night, from 8pm to 8 am the next day there is one care assistant awake and a person sleeping in. Management hours are additional hours on the rota. Residents spoken with had positive comments to make about the present staff team. One resident said that “they are very nice” and another commented that the carers were “very good at the moment”. Care assistants spoken with were able to demonstrate that they understood the needs of residents and that they were familiar with individual residents care plans. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37and 38 Records required are constructed and maintained so that up to date information is available about residents to safeguard their best interests. Greenways provide a greatly improved environment, which promotes the health, safety and welfare of residents and staff. EVIDENCE: During the visit the environment was seen to be well maintained and reasonably safe for residents. A programme of covering radiators has taken place and all except two windows on the upper floor have restricted openings. The manger has agreed to document risk assessments for those radiators not covered and the windows, which are not restricted.
