The age/age range of the persons to be accommodated will be: 18-50 YEARS Only one service user between the age of 18 and 20 years may be admitted, within the total of six service users. 22nd September 2004 Date of last inspection Brief Description of the Service: Grovelands Farmhouse is a care home providing personal care and accommodation for six adults with learning disabilities between the ages of 18 and 65 years. The home provides for service users with complex needs, particularly those of a psychological nature. The home is owned by THF Care Estates Limited. The company operates other care homes, in Sussex and Surrey. The company also owns a working farm next door to the home. A range of daytime services for people with learning disabilities operate from the farm and service users living at the farmhouse attend for individually planned sessions.The farmhouse is in a remote country location, accessible via a single-track road. There is no public transport route nearby so visitors to the home use their own private car or taxi service. The home is situated close to the junction of the M23 and M25 motorways. The towns of Oxted and Godstone are easily accessible by car, and the home has its own minibus. Grovelands Farmhouse DS0000013661.V261462.R01.S.doc Version 5.0 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This is the second inspection that has taken place this year 2005/2006. The inspection was announced and therefore the residents and staff were aware that the inspector was visiting. The manager was present throughout the inspection and the inspector was given plenty of opportunity to speak to residents and staff who were on duty. Records were also seen. The atmosphere in the home was relaxed. Residents were freely accessing many different areas of the home. The home was clean and tidy and there were adequate staff on duty. The inspector wishes to thank all those involved in providing the information in this report and in particular the residents, the relatives, the manager, the provider and the staff present. The home has consistently offered a good standard of care and support to the service users. However it is essential that the home is allowed to retain all original documentation concerning the residents or the service, the head office should no longer keep them. The CSCI must be given access to ensure that the home complies with standard 42 of the National Minimum Standards for Younger Adults. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7&9 The service users’ individual plans are clear and comprehensive including details of needs and goals. They also incorporate known or indicated preferences and choices EVIDENCE: Care plans inspected were comprehensive and set out actions which need to be taken by care staff to ensure that the needs of the service users are met. They also set out the protocols for staff to follow to ensure that the service users are encouraged and assisted to make decisions about their own lives. The care plans fully evidence that staff enable service users to take responsible risks. These risks are assessed and included in each care plan with appropriate actions/training identified to reduce the level of risk. One service user was very happy to share is passion for Chelsea Football Club with the inspector the staff fully support him to be a proactive member of the football teams supporters clubs. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17 The meals in this home are good offering both choice and variety and catering for special dietary needs. EVIDENCE: Care workers encourage service users to make their own choices and this is reflected in the variety of foods the service users receive. The home provides three main meals per day with snacks of fruit, soft drinks, tea coffee and biscuits throughout the day. On the day of inspection there was a good supply of food, in the home. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Personal care and healthcare support and assistance is planned and was seen to be provided, where needed, in a respectful and sensitive manner. EVIDENCE: At the time of this inspection only one service user required the assistance of staff for their personal care needs, this was clearly set out in the service users individual plan. The manager explained that the remaining service users are independent with their personal care and are only offered the minimum of assisted to maintain and develop that independence. The care plans sampled also provided evidence that service users’ healthcare needs are being met. Service users are all registered with a local GP and referrals to other health care professionals are obtained, as necessary, from the GP surgery. JUDGEMENT – we looked at outcomes for the following standard(s): 35 Staff appeared to be enthusiastic and committed to supporting residents. Generally the training and development of staff within this service is given a high priority. EVIDENCE: The relationship between the service users and staff was observed to be relaxed and friendly, creating a warm and homely feel. The service users are encouraged and supported to be as independent as they were able. Training and development of staff has been given a high priority. The service has embarked upon the Investors in People award and has successfully completed the initial process for the award. Training continues to improve within the organisation and there is a detailed training programme in place. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 39, 42 & 43 There is good leadership and consistent direction to staff in this home to ensure that Service Users receive consistent quality care. The manager is fully aware of the needs of the Service Users in the home and as such is able to communicate this to staff through regular staff meetings and individual supervision sessions. The inspector was not able to review standard 43 again as the information was not available. EVIDENCE: Grovelands Farmhouse DS0000013661.V261462.R01.S.doc Version 5.0 Page 16 The evidence seen of the frequency of staff meetings and informal supervision is indicative of the open and supportive atmosphere of this home. Regulation 26 (Monthly visits by the proprietor) are undertaken and copies are sent to the CSCI inspector. All relevant policies and procedures were in place. They included policies and procedures for the health, safety and welfare of service users and staff. Systems existed to demonstrate these had been communicated to staff. Detailed policies and procedures were in place in relation to safe working practices. There is a system where the organisation conducts a full quality assurance audit of the relatives of the service users and any other relevant professional who have regular contact with the service users. However the manager is not party to this process and is only given basic feed back from her head office. It is essential that the manager has full access to the returned forms and is allowed to retain the originals on site for future reviews. The inspector was also unable to review the financial status of the home (standard 42), as the relevant items for audit were not available. The manager had made considerable efforts to ensure that the required items were on site and called the financial manager during the inspection to ascertain if the papers would be made available unfortunately these items failed to arrive before the inspection was concluded.
