Mrs Roy the manager was not available at the start of the inspection but came in when she was informed of our arrival. The owner Mrs Hepworth-Lloydd, who was working at the sister home across the road, also came to meet with us. We checked that the home had complied with 4 of the requirements we made at our key inspection on 10/08/09. We consider that the four requirements had been met. The manager, providers and staff team have worked hard to rectify the issues we found. What the care home does well:
We looked at the way medication is handled. Staff had completed the Medication Administration Record (MAR) charts carefully using the correct ommission codes when necessary. We also noted that medications were correctly recorded when they were received into the home. The MAR charts were stored in a medication file. The manager has developed a detailed profile for each of the people who lives here which includes a photograph of the person, details of their medications, their GP and any special instructions about their medication. We looked at one profile, for a person who takes medication to control diabetes, which detailed the need for staff to watch for signs of hypo or hyperglycaemia attacks and to make sure the persons blood sugar level is taken regularly. The same instruction was written in the persons care plan. We asked the manager how staff would know what to look for and she told us that, although they were working as carers, most of the staff had a nursing or medical qualification from their country of origin, and would know. After disussion she accepted that other staff might not have this background knowledge, which could lead to a change in condition not being noticed quickly enough. She agreed to put in further detail and guidelines. Controlled drugs are now correctly stored in the CD cupboard. The manager showed us that wound care is now detailed in the care plans when needed. Good infection control guidelines for MRSA were in one persons file. The manager showed us that all staff had received Safeguarding of Vulnerable Adults (SOVA) training or an update by 27/08/09. This training was provided to some of the team by Bedford Borough Council and to others by a training provider to ensure it was completed in the timescale given following the last inspection. The manager had also made sure an up to date SOVA policy was available. We spoke to the manager and the senior carer about how they would deal with a potential safeguarding issue. They were able to answer correctly and were clear about who should be informed. We looked at two staff files. One for the part-time activity co-ordinator who, at our last inspection, had not had any pre-employment checks, and one for a staff member on duty. Both had all had the required checks on the files. Following the recent inspection, the provider purchased and installed a small safe. The
Care Homes for Older People Page 4 of 9 manager showed us that any money and valuables belonging to people who live here is now kept in the safe. We also noted that a supply of dinner plates had been purchased, and visitors were no longer able to enter the home through the conservatory.
