Conditions of registration: Date of last inspection 15 July 2005 Brief Description of the Service: 30 High Barn Road is a care home for two older people with learning disabilities. The home is located in School Aycliffe, a residential area on the outskirts of Newton Aycliffe, County Durham. Although the two service users accommodated have their own, semiindependent, living accommodation, the premises are also the home of the registered provider - Mrs. Frances Bainbridge - and her family. Prior to the Care Standards Act 2000 the home was Registered as a Small Home where a lighter touch to regulation, and the standards required of such a home, was applicable. Further, although the home was registered it was not inspected. As such, over a period of twelve years, the service has developed its own, common sense, family-lifestyle approach to care. This is an overview of what the inspector found during the inspection. By law we have to inspect all care homes at least twice a year. This unannounced inspection was carried out in accordance with this obligation, over 1½ hours, on the morning of Friday 20 January 2006. In line with current CSCI policy on ‘Proportionality’, the inspection focused upon a number of key standard outcomes for service users. The key standard outcomes not inspected on this occasion were assessed during the last inspection of the home. The inspector looked around the service users’ living accommodation. The proprietor was spoken to as were - within the limits of their communication and understanding - the two service users accommodated. To ensure that the proprietor can continue to meet the service users’ needs, as highlighted in previous inspection reports, some formal training is recommended – see pages 18 & 19 of this report. Also, the home’s adult protection policies should be reviewed and amended to reflect: recent developments; the local adult protection policy - including the

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 & 10. Health and personal care needs are appropriately met. EVIDENCE: 30 High Barn Road is registered to accommodate two older people with learning disabilities. The two service users accommodated (now aged 75 & 80) came to live at the home in 1990. The provider has stated that it is her intention not to admit any more people into her home. Although the inspector was unable to communicate accommodated to any significant degree, the inspector company. The people accommodated were seen to be comfortable in their home. Care reviews are regularly held, the most recent of which were held in November 2005. These continue to confirm the suitability of the placements and other support arrangements in place to meet any assessed needs. As stated in previous inspection reports, it is repeated here that the manner in which the care and support is provided does not readily translate into full compliance with any Regulations and National Minimum Standards under the Care Standards Act 2000. Nevertheless, it is clear that the home has been instrumental in bringing about significant improvements in the quality of care and quality of life experienced by the service users accommodated. The stigma of long-term institutional settings and the negative behaviours associated with those settings have been replaced by a far more prestigious and enjoyable lifestyle within a homely, family setting. The home’s systems relating to medication were checked. The home uses a monitored dosage system. There are appropriate policies, procedures and systems in place relating to the receipt, recording, storage, handling, administration and disposal of medicines. However, as highlighted in previous inspection reports, it is recommended that the proprietor should undergo formal training in the safe handling of medicines. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. EVIDENCE: Observations made, together with discussions with the home’s proprietor indicate that - within the limits of their communication and understanding service users’ views and concerns are listened to. The home has an appropriate complaints policy/procedure. Information about complaints, how and who to make them to, is also provided in the home’s ‘Service Users Guide’. The home has its own policy and procedure document covering abuse. However, this should be reviewed and amended to reflect: recent developments; the local adult protection policy - including the local contacts; and local protocols about the initial action to be taken (things to do and things not to do) if an allegation of abuse arises. Further, as highlighted in previous inspection reports, in the absence of family involvement and alternative safeguards, service users financial matters should be considered in any care management reviews. Although no staff are currently employed, the provider is meeting the needs of the people accommodated. EVIDENCE: Although no staff are currently employed, the provider is meeting the needs of the people accommodated with assistance from other members of her family if necessary. This staffing arrangement has been in place for some time and is considered adequate to meet the needs of the 2 service users currently accommodated. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 & 38. The home is well run. EVIDENCE: 30 High Barn Road is owned and run by the proprietor - Mrs. Bainbridge - who is fully involved in the day-to-day running of the home. Other than a part-time domestic assistant no staff are employed. Mrs. Bainbridge is meeting the needs of the persons accommodated, with assistance from other members of her family if necessary. However, whilst it is recognised that Mrs. Bainbridge possesses a wealth of common sense experience in relation to the care of the two people accommodated, it is incumbent upon her to ensure that she should undertake, from time to time, such training as is appropriate to ensure that she has the experience and skills necessary for carrying on the care home (Reg. 10(2)). As highlighted in previous inspection reports, the proprietor should ensure that from time to time she undertakes such training as is appropriate to ensure that she has the experience and skills necessary for carrying on the care home. Because the proprietor is actively involved in the running of the home she ensures that 30 High Barn Road is run within the ethos of the above standards in as much as they relate to the scale and domestic nature of the home. Documents examined and discussion with the home’s proprietor during the course of the inspection confirmed that appropriate systems are in place to ensure service users’ heath, safety and welfare.
