Conditions of registration: Date of last inspection 11/1/05 Brief Description of the Service: Hill House provides personal care, including nursing care, to older people but does not provide care in other categories such as dementia care or people with mental illness. Its services include “Intermediate Care” and care of the “Terminally ill”. Both of these specialist services are supported directly by the relevant Health Authority agencies. The hotel-style home comprises 35 single rooms and 15 shared rooms. There are 4 dayrooms plus bathrooms, showerrooms and toilets throughout the home. There is a library, a hairdressing salon plus the usual facilities, including a laundry, offices and staff rooms. There is a garden to the rear plus an enclosed courtyard and to the front off-street parking is available. This is an overview of what the inspector found during the inspection. This care home continues to achieve high standards throughout. The Directors of this care home can be satisfied they are maintaining a very good service. Only a small number of requirements arose and the CSCI is confident any outstanding matters will be addressed. The purpose of the visit was to undertake an unannounced inspection of key standards. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 6 Service users’ needs are being assessed prior to admission to ensure their needs can be appropriately met. When service users are admitted without such an assessment the nursing staff in the home assess service users’ needs immediately after admission to ensure they have been appropriately referred. Service users that are admitted for intermediate care are offered appropriate support to enable them to return to their homes. EVIDENCE: The arrangements for assessing service users prior to admission were discussed in some detail and the manager confirmed that the home will in future assert its right to admit only service users within its registration category and has the right of veto to not admit service users out of category. This was a previous requirement and is now met. Service users’ case files were examined and found to contain the necessary assessments and service users advised the inspector that their needs were made known to the home before they were admitted. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10. Care Plans are in place for each service user to ensure their health and personal care needs are being fully met. However the frequency of which care plans are review needs to be reassessed for those service users whose needs are complex and may well have changed since admission. Medication procedures and policies ensure that service users are protected. Service users are treated with respect and dignity at all times by the staff. EVIDENCE: Individual plans of care, ‘care plans’, are in place for all service users. A standardised format is used for all service user documentation, this makes the information about service users’ needs readily available for all staff. These personal case files for each service user include the initial assessments from which arise the goals designed to meet the specific needs of service users. Medication is administered in most instances by the nursing staff. The service users admitted for intermediate care hold and administer medication themselves with the support of nursing staff. Service users confirm that they are treated with respect and dignity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 to 15 The lifestyle of service users is much as they expected when they entered the home and this home supports and encourages them to exercise they of choice whenever possible. The meals are varied, nutritious and service in pleasant and enjoyed by the service users. EVIDENCE: In discussion with staff and service users it is clear that the home attends to the social, cultural and emotional needs of service users. They are supported in maintaining contact with family and friends and in some cases will be returning to their own homes and are therefore encouraged to maintain community links. Various social, recreational and therapeutic activities are offered in this home and on the day of inspection an art class was underway; quizzes are also very popular. Choice in nursing homes depends upon the capacity of the service user to make choices but every opportunity to make choices is offered, for example with respect to their admission, with a range of meals, how they will spend their day and so forth. The service users commended the chef for the very nice meals provided. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 18. Complaints are dealt with in a sensitive and professional manner this ensures service users and their representatives feel free to make their concerns known and they will be acted upon. Policies, Procedures and training are in place for the safety and wellbeing of service users. EVIDENCE: The record of complaints was examined and it shows that the home deals with complaints in a timely and helpful manner. No complaints arose during the course of the inspection but one service user did make observations about a range of issues concerning her care and in order to ensure her needs can still be met in this particular care home a recommendation is made under the provisions of standard eight (about assessing and meeting service users changing needs). Staff are aware of their responsibility to protect service from abuse and to report untoward incidents to the correct authorities. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 26. Service users live in a safe, clean, well-maintained and comfortable environment. This large domestic style property extended to meet the needs of service users and is subject to ongoing refurbishment. EVIDENCE: Communal areas are pleasantly decorated and individual rooms are spacious and each bedroom has a shower or wash hand basin and a range of suitable bedroom furniture and fittings. Minor damage noted in some areas such as a missing bathroom door lock. In most areas the use of tasteful colour schemes make this a large but homely environment. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 Staffing levels are adequate for the home’s stated aims and objective enabling staff to meet the wide range of service users’ needs. EVIDENCE: Staffing levels are based upon the home’s existing Staffing Notice issued under previous guidance. The staff rosters indicated that staffing levels are frequently above these minimum levels. In addition to the Nursing and Care staff, the home employs a large ancillary staff team including a professional chef and assistant in the kitchen plus catering assistants; housekeeper and domestics and gardener/maintenance persons. The CSCI is therefore satisfied that staffing levels in this home can meet the current needs of service users. The inspector was reminded that the staff team is large enough to allow shift leaders on each of the three floors to vary the deployment of staff, between floors, depending upon variations in service users’ dependency. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33, 38 This is clearly a well managed care home, that is run in such a manner as to ensure the health and well being of the service users. A number health and safety hazards were identified and need to be addressed to ensure that safety of service users. EVIDENCE: The registered manager has been assessed by the CSCI as competent and fit to manage this home and has been doing so for several years an a series of excellent inspection reports testify to the competency of the management team including the directors and the on-site manager. Once again only a small number of matters require attention so service users can be assured their interests, their safety and their comfort is paramount in this home. A number of matters do however require attention including materials to be cleared from fire exit routes and unlocked chemical cupboard. Whilst the home has embarked on an assessment of hot surfaces that may pose a risk it has yet to deal with heated towel rails in service users’ ensuite facilities.

STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard 38 Regulation 13(4)(b) Requirement Management: The home must risk assess hot surfaces, including radiators and towel rails and must take steps to reduce those risks by reducing the temperature to 430C or fitting protective covers. This remains an outstanding requirement from 30/3/05 Fire safety: It is required that all fire exits routes be kept clear at all times; materials such as carperts rolls and wheelchairs must be removed from exit routes. Chemical Hazards: it is required that hazaradous chemical be stored in a locked cupboard. Aids and adaptations: Wheelchairs must have their footrests in place when in use. Electircal hazards: Multiplug adaptors should not be used; either increase the number of wall-mounted sockets or use fused extension units.
