Conditions of registration: Date of last inspection 20th December 2004 Brief Description of the Service: Hill View Care Home is a small family run home. The home has been developed as an adult placement to accommodate one service user, within the category of learning disability LD. The home is a considerable distance from the local shops. However, the home has its own transport. The premises are homely and nicely furnished. There is one single bedroom on the first floor, and facilities in the home include a lounge, kitchen with dining table on the ground floor. A bathroom/toilet with shower, an office also used as a sleep in room on the first floor. The garden is small but nicely laid out on levels rising upwards, the patio area on the lower level has a BBQ. There is ample parking in the main car park for the entire house; the home is accessible by a ramp and steps to the main door. There is a small but nicely laid front garden. This is an overview of what the inspector found during the inspection. The aim of the announced inspection was to provide an opportunity to meet and speak with the resident and staff and to follow up the previous recommendations made as a result of the last inspection. Mrs V Bulbeck lead inspector undertook the announced inspection over a twohour period. It will be necessary to review both inspection reports for 2005-06, undertaken by the Commission for Social Care Inspection, to obtain a full understanding of the extent to which the home meets the 30 standards of The National Minimum Standards for Adult Placement. Three members of staff were on duty and the atmosphere in the home was relaxed and comfortable. Mr & Mrs Khulpateea, the proprietor/manager and wife cover the management of the home. The inspector received a letter from the resident informing the inspector she was very happy living in the home and would not want to return to where she lived previously. The resident also informed the inspector verbally that she is very happy and has a good rapport with the staff team. The Service user in this home is called a resident and therefore the word resident will be used in the remainder of this report. The resident was seen to be well supported with her personal care needs and to be relaxed and confident with staff on duty. However it was noted that the resident is kept informed but the records are not indicating the involvement of the resident. The intended outcome for Standards 1 is: 1. Each person is able to live an ordinary life in their community and to share family life. JUDGEMENT – we looked at outcomes for the following standard(s): 1. The resident has a written contract and was admitted to the home following a needs assessment to ensure that the home can meet the residents identified needs. EVIDENCE: The homes statement of purpose has been updated and includes relevant information as detailed in The Care Homes Regulations 2001, Schedule 1. Prospective residents would be able to understand the service users guide and if difficult to understand staff would support the resident. The document is in a format for the resident to read. One care plan was sampled and it was noted that the care plan has been updated and contains detailed information. The manager informed the inspector that the care plan is updated on a regular basis. For example the resident has improved in a number of areas in the last year, these changes have been clearly documented on the residents care plan. However, the resident should be more involved, and should sign to indicate they understand if this is not possible the care manager, doctor or relative should sign the care plan on her behalf. The intended outcomes for Standards 2 – 3 are: 2. 3. Each person’s individual aspirations and needs are assessed. Each person knows that the placement they choose will meet their needs and aspirations. JUDGEMENT – we looked at outcomes for the following standard(s): 2 and 3. There is a proactive approach to promoting the independence and life skills of the resident through careful care planning and risk assessment. EVIDENCE: The care plan sampled during the inspection was comprehensive and easy to follow it acknowledged the goals achieved by the resident. Reasons for not achieving aims are also recorded in the care plan. For example the resident likes to go on the bus shopping this is used as a learning curve being able to identify her independence. Risk assessments are in place and have been reviewed in 2005, the manager needs to sign risk assessments. The management of the home has undertaken a regular report on file. There is a recent photograph and description of the resident, which would be used in the event of reporting a missing person. The inspector was informed that the resident assists in the kitchen with meal preparation under close supervision of a staff member. The intended outcomes for Standards 4 - 7 are: 4. 5. 6. 7. Each person is supported to live the kind of life they wish to live. Each person has an individual written Placement Agreement and Service User’s Plan. Each person can be sure that their current and changing needs are met. Each person knows their AP Carer is well supported and their work reviewed by the Scheme. JUDGEMENT – we looked at outcomes for the following standard(s): 4 and 5. Residents have the majority of information they need to make an informed choice about where to live. Resident’s needs are fully assessed prior to admission and all residents have a written contract. EVIDENCE: A written placement agreement had been undertaken. Details of the staff would be included as well as the bedroom allocated. Details of the registered manager’s qualifications and skills need to be included. Perspective residents are invited to visit the home prior to admission, to enable the resident to familiarise themselves with the staff and premises. This can take several weeks prior to admission, and would involve having a meal, perhaps stay overnight and another option is to stay for a couple of nights. The home operates a trial period of three months for perspective residents. The intended outcome for Standard 8 is: 8. Each person knows they will be supported by a trained, responsible and competent AP Carer. JUDGEMENT – we looked at outcomes for the following standard(s): 8. The resident has the opportunities for personal development, to take part in appropriate activities within the home and in the local community. The resident is supported to maintain and develop appropriate personal and family relationships. Meals are well balanced and varied. Systems are in place to ensure that resident’s rights are respected. EVIDENCE: The resident has a varied lifestyle; she is able to travel on the bus on her own to Guildford every Friday for shopping. There is also a varied programme of activities, which the home is involved in. The home has its own transport and is able to take the resident to the library every week, as well as shopping, which the resident enjoys. She also visits her mother on a regular basis. This year the resident went on holiday to the Isle of Wight with two members of staff. The inspector was able to see the photographs of the holiday and it would appear from the photos that the resident enjoyed her holiday. The resident also informed the inspector she enjoyed her holiday. However, the management of the home needs to speak with the care manager to enable the resident to go on holidays in the future as her funds are low, this is partly because she has to pay for a member of staff as well, there will not be sufficient money for a holiday next year. A member of staff who was cooking the evening meal informed the inspector the resident had informed her that she would like to lose some weight. A healthy eating programme has been introduced, which is well balanced and nourishing. The resident informed the inspector her favourite meal is shepherds pie, she also likes to help with the preparation of meals and laying the table. The intended outcomes for Standards 9 – 10 are: 9. 10. The person benefits from a properly conducted AP Scheme. Each person is safe in the placement. JUDGEMENT – we looked at outcomes for the following standard(s): 10. All required policies, procedures and practices are in place to ensure that residents are safeguarded, as far as reasonably possible, from harm or abuse. EVIDENCE: A number of records were observed; these include the accident book, fire records, training as well as health and safety records and were found to be well documented. There is a need to ensure all fire records are maintained in one folder the fire alarm system was recorded on a regular weekly basis and fire drills are undertaken three monthly. There is a need to produce a fire risk assessment on the whole building room by room, and management needs to produce an emergency plan for use in the event of any emergency and details must be kept in the fire record folder.
