Conditions of registration: Date of last inspection Brief Description of the Service: Hillside Farm Residential Home offers accommodation for up to 7 people who are over 65 years of age. The building is located just off the main road through to Nottingham from Loughborough. There is ample parking space to the front of the premises. The owner/manager resides in a house connected to the residential home. The building is surrounded by countryside. All areas of the premises are accessible for people with limited mobility. This is an overview of what the inspector found during the inspection. This inspection was conducted unannounced to review the key standards. A range of information was used to inform the report; The service history since the last inspection, the pre-inspection questionnaire, the views of the residents and one relative, observation of the environment and the information provided by the care assistant in charge at the time of the inspection. The main method of inspection was to case track three residents, this involves looking at the care plans of the resident and tracking the care they receive. A brief tour of the premises was also undertaken which included viewing the rooms of those residents case tracked. Residents were very happy with the care provided. Each inspection has found the outcomes of the care received by the residents to be good. Changes to the needs of the residents are identified and met, however the records of new identified risks should be updated each month or as problems arise to ensure that all staff have access to information about the resident that is up to date.

Whilst it is recognised that staff are working towards NVQ qualifications it is necessary to record the mandatory training that staff undertake including the certificated evidence of accredited training as required. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1-2-3-4-5-6 Quality in this outcome area is excellent. This Judgement is made using available evidence including a visit to this service. Prospective residents and their representatives have the information they need to choose a home which will meet their needs. They have their needs assessed and a contract which clearly tells them about the service they will receive. Intermediate care is not provided. EVIDENCE: The home has a sustained track record of meeting these standards. On this visit the records of two residents recently admitted to the home were examined and demonstrated that the manager had fully assessed the persons needs prior to agreeing admission. Where required, a copy of the social work assessment of the persons care needs had also been obtained. The relative of a recently admitted resident confirmed that they had visited the home prior to the arranged admission, had been supplied with written information about the service and a contract of the terms and conditions of the home.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7-8-9-10 Quality in this outcome area is good. This Judgement is made using available evidence including a visit to this service. The health and personal care which a resident receives is based on their individual needs. The principles of respect, dignity and privacy are put into practice. Medicines are safely managed however, accreddited staff training in medicine management could not be verified. EVIDENCE: The home has consistantly met these standards. The records of two residents show that each had a plan of care that is reviewed monthly and in consultation with the resident and their representatives. The plans are in large print and easy to understand. There were risk assessments for the residents’ risk of falling. Hillside Farm Care Home DS0000008694.V298826.R01.S.doc Version 5.2 Page 10 Where residents had fallen a record of the accident had been written appropriately, however there was no clear evidence of what action the service had planned to control the risk within the care plan document. Daily records were updated to reflect changes in the persons condition. There was no clear instruction on informing relatives in this event and this had not been undertaken on this occasion Staff responsible for administering medicines had been trained to do so by the home manager. There were no records available of this training, and therefore could not be judged as accredited as required. Residents spoken with confirmed good access to health care support through the district nurse and General Practitioner. All residents spoken with felt staff were respectful and showed particular attention to their privacy and dignity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12-13-14-15 Quality in this outcome area is excellent. This Judgement is made using available evidence including a visit to this service. Residents are able to choose their lifestyle, social activity and keep in contact with their relatives and friends. Residents receive a healthy and varied diet. EVIDENCE: The service has a sustained track record in meeting these standards. Residents have a variety of options to choose from to enjoy activities within the home. A full list of the available activities is posted on the lounge notice board. Residents are actively encouraged to keep contact with their families and friends. Residents said they appreciated the good quality food provided and are able to have drinks or snacks as they wish. JUDGEMENT – we looked at outcomes for the following standard(s): 16-18 Quality in this outcome area is good. This Judgement is made using available evidence including a visit to this service. Residents have access to a robust, effective complaints procedure. Staff are aware of the issues around adult protection but best practice would be to provide suitable training for staff in these areas. EVIDENCE: The service has a clear complaints procedure which is visible within the home. Complaint records were not viewed at this time. No complaints have been received by the commission regarding this service. The staff file viewed did not contain evidence that training in adult protection had been undertaken. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19-26 Quality in this outcome area is excellent. This Judgement is made using available evidence including a visit to this service. The physical design and layout of the home enables residents to live in a safe, well maintained environment, which encouarges independence. EVIDENCE: This small home provides a safe, comfortable and attractive environment in which residents are encouraged to see as their own home. The home is on one level and has a range of adaptations and equipment to meet the needs of the residents. Individual rooms are fitted with good quality fixtures and fittings. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27-28-29-30 Quality in this outcome area is good. This Judgement is made using available evidence including a visit to this service. Staff in the home are trained in the care of older people through National Vocational Standards. There are sufficient staff to meet the needs of the residents. EVIDENCE: The home has a sustained track record in meeting these standards. One staff file examined demonstrated that appropriate recruitment continues with safeguards in place to protect vulnerable adults. The staff are accessing NVQ training as required.. Staff confirmed that they have regular meetings with the manager regarding supervision of their practice. Records of staff training and certificates of acheivement did not provide sufficient evidence of required training such as moving and handling. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31-33-35-37-38 Quality in this outcome area is excellent. This Judgement is made using available evidence including a visit to this service. This is a well managed home with substantial strengths and a sustained track record of delivering good performance and manageing improvement. EVIDENCE: There is a sustained track record of meeting these standards. The owner/manager was on holiday at the time of this inspection. The manager lives adjacent to the home and is available on site 24hrs a day except when on holiday. A senior care staff member was in charge during the managers absence and undertsood her role. Each resident’s file contains evidence of appropriate referral to legal or financial representation. The home does not manage the personal finances of the residents. The registered person is committed to ensuring the health, welfare and safety of the residents. The systems for fire prevention were examined and found to contain appropriate risk assessment, training, equipment maintenance and tests to comply with the requirements. Staff files did not contain evidence of training provision in moving and handling, medicine administration, food hygiene and adult protection.
