This inspection was carried out on 2nd December 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Hilton Park provides high quality accommodation in two wings, each containing two units, within attractive and well-maintained gardens. It has an excellent activities program with activities provided for service users of all ages and capabilities. The unit for younger adults strives to make life full and interesting and the manager and staff are open to any ideas put forward by those involved in and using the service. Staffing levels are good and accommodation is available for staff. Training is available for all grades of staff and exceeds the recommended level of staff trained to NVQ level 2 in care.

What has improved since the last inspection?

A separate service user guide has been written for the Churchill Unit for young physically disabled people. The refurbishment of the building has been completed and new adjustable height baths fitted in all bathrooms. All service users are assessed as to their ability to maintain control of their own medication and a policy is now in place with several service users now managing their own medication.

What the care home could do better:

Hilton Park provides excellent facilities in all aspects of the care they offer.

Clients 18 years and over with a physical disability Date of last inspection 25th May 2005 Brief Description of the Service: Hilton Park Care Centre is owned by Barchester Healthcare Homes Limited and is registered as a care home with nursing. The home is situated in the village of Bottisham and is 7 miles from the centre of Cambridge and 6 miles from Newmarket. The home is divided into two units the ‘Cambridge Suite’ and ‘Newmarket Suite’. Both Suites consist of two floors and there are shaft lifts to the upper floors. There are 91 single bedrooms and one double bedroom, all of which have en-suite facilities. Within the Newmarket suite is a unit for 12 young physically disabled adults. The home is registered to provide care to 23 Younger Adults with physical disabilities. The unannounced inspection commenced at 11.30 with the Deputy Manager who has recently been promoted from the post of unit manager. The manager was out doing assessment visits. The inspection process consisted of spending time with the Deputy Manager reviewing the requirements from the last inspection and assessing the minimum standards that were not inspected at the last inspection. This involved viewing documents and records. A tour of the home was conducted, staff and service users met and spoken to. The inspector found the home to be well staffed, clean and fresh and was able to observe activities taking place and observe good interaction between service users and staff. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, & 4 All prospective service users have sufficient information to make an informed choice about Hilton Park and the terms and conditions of their residence ant that their needs can be met. EVIDENCE: There is a full statement of purpose and service user guide for each of the categories of registration. All service users have a statement of terms and conditions of residence issued by Barchester Healthcare. All prospective service users have a full assessment of their needs before being offered a place at Hilton Park and are formally notified of the homes ability to meet their needs when being accepted for a place. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9, 10 & 11 Service users are able to manage their own medication following a risk assessment. The wishes of all service users with regard to final illness and dying are known. EVIDENCE: There is a policy in place to ensure service users are able to continue to manage their own medication if they are capable and wish to do so. This is clearly recorded on the care plan and appropriate records are kept of all medication. There is a care plan in place that records the individuals wishes for care in their final illness and after death. Respect and dignity are paramount to the care of the service users and forms a great part of new staff induction training. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 The service users have a choice of a well balanced diet at all meal times. EVIDENCE: The menu offers a choice at each meal time including a cooked breakfast. There are several dining areas that are all pleasant with nicely laid tables and the presence of adequate staff to offer any assistance that is needed. All diets can be provided including those needed on religious grounds. If a service user does not want the choices offered for that day then alternatives will be provided. JUDGEMENT – we looked at outcomes for the following standard(s): 21, 23 & 24 There are sufficient washing, bathing and toilet facilities to meet the needs of all the service users. The standard of accommodation is excellent. EVIDENCE: All the bedrooms have en-suite toilet and washing facilities. There are additional toilets around the home so service users are never far from one. All bathrooms have variable height baths with hoist facilities. Bedrooms are light and airy and personalised to the individual with their own possessions around them. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 & 29 The home is well staffed by people recruited after a thorough checking procedure. EVIDENCE: The home is staffed by Registered nurses, senior care staff, care assistants, domestic, catering, maintenance and administrative staff. The management team is in close contact with the staff on a daily basis. Care staff are encouraged to undertake NVQ level 2 and above and the home exceeds the 50 of staff recommended to have this qualification. Registered Nurses are supported to keep up to date with training and study time. All staff recruited are thoroughly vetted to ensure they are fit to care for vulnerable people and the homes recruitment procedure ensure all appropriate checks are made.. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34, 35 & 37 The management of the home ensures that the well being and safety of the service users and staff are protected at all times. EVIDENCE: The manager is approachable and available to both staff service users and their families. The management team is known to all staff and service users. Policies and procedures are in place to safeguard service users finances and records were seen as part of the inspection. All records are maintained as required.
