What the care home does well

The home itself is clean, odour free and practical for the residents of Hollymede.The manager and team are warm friendly, experienced and overall seem a contented and happy team. The cook provides quality home cooked meals that reflect the residents taste. Residents spoken with demonstrated that the cook goes out of her way to cater for individual tastes and to provide alternative meals. Observation and conversation evidenced that plenty of food and refreshments are available. Relatives and friends are actively encouraged to visit the home and there are no time restrictions. Many compliments and cards were in place from relatives. Throughout the home photographs of staff are in place to allow residents to see who is on duty, menus for the meals offered that day are placed on each table and a calendar featuring the residents is in use. Staff spoken with are happy in their jobs, enjoy working with the residents and feel supported by the manager. All residents and relatives spoken with had high praise for the manager and staff. The following statements are just a few of the many positive comments received via the surveys sent out to relatives, residents and staff; ` I feel the care and support my mother receives is over and above what I expected`. `I am very pleased that the staff are continually gaining qualifications`. `Everything is done to a high standard and this is the third home my mother has lived in and this is the first home that I have had complete trust in`. `It is always nice and clean. The food is very nice and dining tables are laid out nice. Mum is very happy there`. ` They encourage my mother to join in activities and arrange outings for her`. The family is always made welcome and mums personal needs are always met. This is a home from home`. `The staff care for my every need. I couldn`t ask for any better care`. `The staff are very kind and always there for me`. `I am very well looked after and everyone is so friendly. The premise is situated in a residential area and successfully presents a homely and comfortable environment. There is a dining room and two additional communal seating areas one of which is a conservatory style lounge overlooking the garden. Residents’ bedrooms are sited on the ground and first floors and all are single occupancy. The shaft lift provides access to all areas used by residents. The bathrooms are available on both floors and the downstairs bathroom is equipped with a Parker Bath. A range of individual and group social activities is provided by/or supported by staff. Outside entertainers visit and residents attend community events. The management at Hollymede cottage offer general hairdressing services free of charge and more specialist services are readily accessible. This was an unannounced site Inspection that looked at all the key standards. Throughout the inspection the manager and fit person were present. Three staff, seven residents and three relatives were spoken with as part of the inspection process. Previous to the inspection CSCI (Commission for Social Care Inspection) sent out surveys to all interested parties. Information collated from surveys and discussion during the site inspection will be reflected within this report. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Prospective residents and their relatives are given a service user guide that informs them of the services provided. They are also encouraged to visit the home prior to any admission. This allows prospective residents and their families to make an informed choice as to whether or not the home will meet their needs. EVIDENCE: Prior to moving in Residents and relatives are given a statement of purpose and service user guide. Residents spoken with informed me that prior to moving in they had information given to them regarding the home and services offered, that they were offered visits and if this was not possible for them to carry this out then their relatives had visited. During the site inspection I spoke to one resident who was staying for a sixweek trial period and was informed she had had sufficient visits and information before anyone expected her to make a decision about moving into the home permanently. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The homes policies, procedures and discussion with residents, relatives and staff evidenced good practice is in place. EVIDENCE: Individuals or their representative have signed that consultation has gone ahead with care plans. All parties are fully encouraged to participate in this process. Included within the Care plan are individual’s preferences likes and dislikes. Plans include a good standard of risk assessment, and do ensure that all assessed needs are catered for. The home has good links with GP and local PCT services. Medication recording, administration, handling and training of staff are to a good standard. Boots provide training and carry out yearly audits. During the inspection a relative was spoken with who’s relative is unable to leave her bed. This relative felt that the service provided had enabled her relative to improve rather than diminish in health. Documentation looked at Hollymede Cottage DS0000069791.V348075.R01.S.doc Version 5.2 Page 11 around this individual and information shared with me during inspection evidenced that the staff and manager of the home provide a well monitored, sensitive individualised care package which allowed for good a quality of care to be given. The home is preventative in its practice and the manager and staff are experienced and knowledgble in its approaches having had no ‘pressure sore’ issue amongst residents who are confined to bed for long periods of time. Observation of staff evidenced that respect; courtesy and dignity are in place whilst working with individuals. Residents spoken with informed me that they get their own post and can make or receive telephones calls in private. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and 15 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Relatives and friends are encouraged to have regular contact with the home. A variety of regular nutritious meals were being provided. EVIDENCE: Residents spoken with felt that the home provides enough different sorts of activities for their needs. Alongside this relatives are actively involved in taking out their relatives and visiting when they or their relative wished them to do so. It was observed during inspection that individuals were supplied with newspapers and other literature they wished to have and that the television was not on at all times at their request. Other residents spoken with felt that it was just as important that staff allowed them time alone to have space from being expected to participate in activities, this they informed me did go ahead. Residents are offered the chance to attend a religious venue of their choice if they wished to do so. Hollymede Cottage DS0000069791.V348075.R01.S.doc Version 5.2 Page 13 Residents and staff spoken with evidenced that regular residents meetings go ahead to involve them in the daily running of Hollymede, to seek views and update them on any necessary or interesting information. During the inspection lunchtime was observed. The individual tables were decorated nicely and ‘picture’ menu cards were provided on each setting. The dining room provided a nice and relaxed environment and staff did not rush residents to eat their lunch or rush those that needed support. The food was to a good standard and looked appealing. There was a choice of meals available to individuals. Food provided is fresh and of good quality. Most residents and the cook were spoken with regarding meals provided and was informed that if a resident had changed their minds about the meal provided then an alternative would be provided. This was often a daily requirement of one the individual residents and conversation and documentation evidenced that everything was in place to cater for this individuals needs. The cook who provided meals evidenced through discussion that she was highly aware of individuals needs, had good ideas around implementing special diets within the already established menus, had good experience around nutrition and did not liquidise foods but made them soft enough so that taste, texture and quality was still enjoyed by the individual. Service users and relative have a service users guide that highlights the complaints procedure. The homes policy and procedures and training of staff protect residents from abuse. EVIDENCE: The home has good and clear systems in place for the complaints procedure and is provided within the statement of purpose and service users guide. The home has not received any complaints. Relatives and residents spoken with said they knew how to make a complaint and who to. The home has received many compliments from relatives and professionals involved within individuals care. The homes systems around ‘safe guarding’ adults and the training/ induction of staff promotes the health, safety and welfare of the residents of Hollymede cottage. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. On the whole the outside environment is pleasant, attractive and provides appropriate and practical usage for the residents. The home environment provides a clean, comfortable and safe environment in which to live in. EVIDENCE: The home itself provides a warm, cosy and relaxed environment. Resident’s rooms were inspected and these were found to be clean, odour free and personalised. All necessary equipment for individuals is in place. Residents spoken with were very happy with the facilities provided and felt their rooms were comfortable and homely. Communal rooms were clean and practical for the use of the residents of Hollymede. Attached to the back part of the lounge is a conservatory that looks out onto the garden. The garden is maintained to a high standard and provides residents to be able to look at the beautiful flowerbeds and shrubbery provided. At the end of the garden is a pond with fish. One resident had planted a rose within the garden in memory of her close friend and was Hollymede Cottage DS0000069791.V348075.R01.S.doc Version 5.2 Page 16 attending to this with support from staff during the inspection. Residents spoken with informed me that they quite often use the garden and sometimes have meals outside weather permitting. Overall the environment is maintained to a good standard both inside and outside of the building. Recently a new nursing call system has been provided for all parts of the home. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 and 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The number of staff on duty, their experience and skill was able to meet the needs of residents. Recruitment is to a good standard. More than 50 staff are NVQ trained. EVIDENCE: Staff spoken with during inspection evidenced that they are experienced well trained and have good knowledge of individuals needs. Observation evidenced that staff were fully engaged with their duties and attentive towards residents needs. Overall the staff team on duty during inspection evidenced that they have good relationships between the residents and their relatives. Residents and relatives had also commented that they thought highly of the manager, staff and service provided. All staff spoken with evidenced that they enjoyed their jobs and enjoyed working for Hollymede. The induction package carried out for new staff covers all aspects of the expected standard. A high percentage of the staff has had NVQ training. Training in general is on going and to a good standard. Staff meetings, supervisions are regular and yearly appraisals are in place. Staff recruitment is to a good standard. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35 and 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Looking at paperwork and through discussion with individual residents, relatives, staff and the manager evidenced good teamwork and clear leadership skills. EVIDENCE: The registered manager is qualified, highly competent and shows good leaderships skills. The staff know what is expected of them and have expressed that they feel fully supported by the manager. Documentation was to a good standard and well thought out. General organisation of the home is good and little things are put into place to include and stimulate the residents. This is apparent in the development of photographs used so that residents can see who is on duty, menus of what Hollymede Cottage DS0000069791.V348075.R01.S.doc Version 5.2 Page 19 foods are provided and the use of the calendar having residents pictures on it for each month of the year. Residents also get to choose papers of their choice, reading books and authors they prefer and lots of thoughtful and general things that give residents a good quality of life which is important to them. Additionally the manager has put into place a Quality assurance Survey’ monitoring system and is waiting for the return of these to collate and action plan them.
