The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Food provided to residents was good. Residents had a choice of meal and received large or small portions dependent on needs. The relationship between residents and staff working at the home were seen to be good. Staff had a good understanding of resident`s individual needs and personal preferences. Although the building works are taking a long time to finish once completed the home environment will be much improved for residents and will provide a homely environment. The home maintains appropriate levels of staff for the numbers of residents living at the home.

What has improved since the last inspection?

Since the last inspection the deputy manager and one senior member of staff have spent a lot of time improving the home`s care records and assessment procedures. Much effort has been undertaken by the registered manager/provider to ensure that recruitment records as required are up to date and available. The system for record keeping has been improved and is much better organised. NVQ training for staff is encouraged and staff are keen to undertake/complete. The cook was observed to wear appropriate clothing in the kitchen.

What the care home could do better:

The home must ensure that all residents have things available to keep them occupied, both within the care home and within the local community. Both basic and specialist training must be provided to all staff working at the care home. All staff working at the care home should receive regular supervision. The registered manager/provider must ensure that he has scheduled time off duty each week. In addition the registered manager/provider must reduce the high number of hours worked by him to an appropriate level. The registered manager/provider must ensure that residents monetary records are accurate and kept up to date.

It currently offers 17 places on two floors with a passenger lift to the first floor. The home offers single and double bedrooms, a limited number of which have en-suite facilities. There are on-going building works to improve the premises and increase the numbers to 25 places, and works remain in progress. The home currently offers residents one large lounge/dining area and one smaller lounge area. The home has a rear garden which is well maintained and accessible to residents. Limited parking is available to the front of the property. The unannounced inspection was carried out by two inspectors, Michelle Love and Bernadette Little. The inspection took place over seven hours. At this visit a tour of the premises took place and some care records and staff employment files were inspected. At the time of the visit the registered manager/provider, the deputy manager, four members of staff and four residents were spoken with. No visitors were observed to visit the home at the time of the inspection. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2, 3, 4 and 5 Arrangements have been made to ensure that residents are assessed prior to admission and information from placing authorities sought and included. It is unclear as to whether prospective residents are given information and have the opportunity to visit the home prior to admission so as to make an informed choice as to whether or not Hollywood Rest Home is a care home they wish to live in. EVIDENCE: Pre Admission Assessments were completed for the newest residents to be admitted. Assessments and information from residents placing authorities were completed in advance of the residents being admitted to the care home. Two residents were unclear as to whether or not they had received a copy of the homes Service Users Guide prior to admission. Residents were noted to have a signed and dated Statement of Terms and Conditions. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10 and 11 The care planning process has improved since the last inspection. Care plans have been reviewed and provide a good basis for care to be delivered to residents. Medication at the home is well managed and storage and records were seen to be appropriate. The systems for resident/representative consultation remain limited with little evidence to indicate that resident’s views are sought. EVIDENCE: Since the last inspection individual care plans and risk assessments sampled contained more detailed evidence of resident’s health, personal, emotional and social care needs. Information related to healthcare issues, visits and professionals involved were clearly documented. Information pertaining to funeral arrangements for residents were clearly detailed within the care plan. Staff spoken with demonstrated a good awareness of resident’s needs and abilities. Little evidence was available to indicate that residents had been involved within the care planning process. Since the last inspection a homely remedy policy/procedures have been devised. Additional information is required specifying those homely remedies used at the care home. Medication training updates are required for some staff. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15 Activities within the home and local community are fairly limited for residents. The meals are good offering both choice and variety. EVIDENCE: Records indicate that since the last inspection there has been little opportunity for residents to receive meaningful stimulation and occupation within the home. In addition, little evidence suggested that residents have the opportunity for activities within the local community. No members of staff have received training in relation to `activities`. Since 26.1.2005, records indicate that on eight occasions activities were provided to some residents e.g. reading newspapers and magazines, listening to music, external entertainers visiting the home, colouring and walking in the garden. At the time of the inspection the local library were seen to visit and replenish the homes stock of books for residents. Residents were offered the choice of two meals at lunchtime. Food provided to residents looked appealing and plentiful and residents comments were complimentary. Care staff provided appropriate care, support and sensitivity to residents during the lunchtime meal. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 The homes Complaint and Adult Protection Policies and Procedures were appropriate so as to ensure that residents are protected from harm or abuse. Some complaint records/processes were incomplete. Staff have knowledge and understanding of Adult Protection issues which protects residents from abuse. EVIDENCE: The home’s Complaints Procedure was displayed within the main hallway. Since the last inspection the home has received one complaint. Records detailing the investigation undertaken, action taken and outcomes were not available. The majority of staff had not received training pertaining to Protection of Vulnerable Adults. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 Building works continue to be in progress at Hollywood Rest Home. Several areas within the home will be renovated and decorated as part of the home’s continuing refurbishment. EVIDENCE: Five additional bedrooms for residents are not ready for occupation. The registered providers stated that works should be completed within two months. Since the last inspection lockable cabinets for residents monies and valuables have been purchased. Furniture within one resident’s bedroom was of poor quality. No door handles were available on one resident’s sink cabinet. Two bedrooms did not have a plug for their sink. One hot water tap was observed to emit cold water instead of hot water. Residents have access to a range of specialist equipment such as walking frames, wheelchairs, bedrails and protective covers, hoist and slings. One bedrail was seen to have a substantial gap between the bedrail and mattress, which could pose a health and safety risk to the resident. Privacy screens within shared bedrooms offered little privacy for residents. Room 6 emitted a very strong and unpleasant odour. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29 and 30 Staffing levels at the home remain appropriate for the needs and numbers of residents. Since the last inspection the standard of recruitment practices within the home has improved so as to offer protection to those people living at Hollywood Rest Home. Staff training for staff remains poor. The home’s induction process is considered inadequate. The registered managers hours remain excessive. EVIDENCE: Staff rotas showed that agreed staffing levels are being maintained. The rotas provide evidence that from 16.5.2005 to 9.6.2005 inclusive the registered manager did not have a day off and week commencing 16.5.2005 worked a total of 64 hours, 23.5.2005 worked a total of 71 hours and 30.5.2005 worked a total of 66 hours. Recruitment records were organised and much improved. Since the last inspection two new members of staff have been recruited to the care home. No Criminal Record Bureau check was available for the visiting hairdresser or chiropodist. Staff training records evidenced that the majority of staff require mandatory training/updates and some specialist training which meet the specific needs of residents. The deputy manager and one senior member of staff are scheduled to receive four days of training relating to continence, diabetes, pressure sore care etc. The home’s induction format is not detailed and needs to be Hollywood Rest Home I06 I56 S18090 Hollywood Rest Home V230248 090605 Stage 4.doc Version 1.30 Page 16 progressed for the future. NVQ training for care staff, senior staff and the deputy manager is progressing well. The registered manager stated that NVQ Level 4 will not be achieved by June 2005, but hopefully by the end of December 2005. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35, 36, 37 and 38 Much effort has been undertaken by both the registered manager/provider and deputy manager to ensure that residents’ needs are met wherever possible. Staff have not received a regular programme of supervision. The home’s monetary arrangements for residents is not appropriate. EVIDENCE: Since the last inspection no management changes have been made at the care home. Much improvement has been undertaken since the last inspection to address previous identified shortfalls. Since May 2004/August 2004, staff have not received regular supervision by the registered manager/deputy manager. A random sample of records as required by regulation were inspected. Some gaps were noted pertaining to recording of fire drills for staff, the home’s hoist Hollywood Rest Home I06 I56 S18090 Hollywood Rest Home V230248 090605 Stage 4.doc Version 1.30 Page 18 had not been serviced since July 2004 and the passenger lift safety inspection had not been undertaken since July 2004. Records for water temperatures were unavailable on the day. Information on this can be obtained in the booklets “ Essential Information for providers of residential accommodation” and “A guide for employers” on 01787 881165 or at www.hsebooks.co.uk Resident’s monies are held by their placing authority for safekeeping. Records evidenced that the registered manager/provider is owed lots of money for individual residents by the different local authorities. One resident’s monetary record did not tally with monies held within the home. No individual receipts were available for chiropody and hairdressing.
