The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home has very thorough arrangements in place to ensure that the needs of potential residents can be met at the home. This includes good assessments of all aspects of care that may be needed. The staff are well-trained. The speak to residents in a friendly and respectful way. The home produces an excellent monthly magazine that keeps residents informed about activities and events. The home takes health and safety very seriously and provides comprehensive information that is accessible and available to residents, staff and visitors.

What has improved since the last inspection?

The home has improved the way in which it stores and transports medicines. These systems are now robust.

What the care home could do better:

The home needs to ensure that 2 written references are obtained for all staff appointments. The home needs to ensure that formal staff supervision takes place more frequently so that staff feel well supported.

Room numbered 101, which is on the first floor, can only be occupied by a person who is independently mobile. 8th November 2005 Date of last inspection Brief Description of the Service: Holmwood House is a care home with nursing that provides care and accommodation for up to 44 residents. The home is owned by Integrated Nursing Homes Ltd The home is located within easy walking distance of the town centre of Swaffham and is close to all local amenities, shops and pubs. Holmwood House is a large Georgian house, with recent extensions. It stands in its own grounds and has a car park at the front of the home. Mrs Chilvers confirmed that the current fee range is £315:00 to £650:00, dependent upon the care received. The fees are made available to potential residents and/or their representatives before moving into the home and are included in the Service User Guide. Residents are advised of any increases to fees in writing. This inspection was unannounced and took place during the day on 13th November 2006. The manager, Mrs Chilvers, was on duty on the day of inspection. Information was obtained from a variety of sources. Prior to the inspection, Mrs Chilvers had completed an annual pre-inspection questionnaire. She also provided a copy of the home’s recent satisfaction survey. A total of 18 questionnaires were returned from residents although no relatives responded on this occasion. On the day of inspection, information was obtained by looking at records, speaking with residents, visitors and staff. A tour of the building also took place and lunch was eaten with residents in the dining room. Overall, Holmwood House provides good care in a pleasing environment. One requirement has been made as a result of this inspection and 4 recommendations have also been made that are based on best practice. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3 & 6. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents receive the information they need to make an informed decision to live at the home. Any admission is made after thorough needs assessments are completed to ensure resident’s individual needs can be met. EVIDENCE: Resident’s needs are assessed before they are admitted to the home to ensure the home can meet their needs. All residents receive a contract that sets out the terms and conditions of their stay. The home does not provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 & 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has developed good care plans that are informed by thorough needs assessments. Residents and their relatives where appropriate, are involved in the care planning and risk assessment process and this is recorded. One care plan did not provide evidence that 2 specific care plans were being complied with consistently. The home ensures residents receive timely GP visits when necessary. The home operates a safe and well-organised medication system. Residents are treated with dignity and respect. EVIDENCE: Care plans demonstrated that individual needs are assessed and met. One care plan did not provide evidence that staff were complying as required and this was brought to the attention of Mrs Chilvers. Residents are involved in the care planning process where possible. Residents said they are treated well and were happy to be at the home. Staff were observed treating residents with respect and dignity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 & 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are able to make choices about their daily living that is respected by staff. Visitors are welcomed to the home at any time and residents are supported to go out of the home. The home provides a choice menu and residents enjoyed their meal on the day of inspection. Special diets are catered for. The home produces a magazine each month that contains information about activities and events. EVIDENCE: The home has an activities programme that is enjoyed by most residents. Residents said they felt they could decide for themselves if they wished to join in activities. Residents said they can make choices about their daily living. Visitors are welcomed to the home at any time and residents are supported to go out of the home. Most residents enjoy their food and some residents spoke about the choices available from the menu. The home produces an excellent magazine each month that contains information about activities and events as well as colourful photographs, quizzes and humour. The home is commended. This judgement has been made using available evidence including a visit to this service. The home is well maintained and in a good state of decoration. All areas are accessible to residents. Resident’s bedrooms are personalised, clean and tidy. All parts of the home were clean and no unpleasant odours were detected. EVIDENCE: All areas of the home were seen during this inspection. The home was in a good state of decoration and repair. The home suffers from a lack of storage space for items such as wheelchairs and trolleys. Some parts of the home are noisy, especially the dining room. This may be due in part to the wooden floors in this room. Resident’s bedrooms were personalised, clean and fresh. This judgement has been made using available evidence including a visit to this service. The home operates good recruitment procedures but must ensure that a minimum of 2 written references is obtained in all cases. Staff are welltrained. Staff are employed in sufficient numbers to meet the needs of residents. Qualified nurses are on duty throughout the day and night. EVIDENCE: The home employs staff in sufficient numbers to meet the needs of residents in a timely way. A qualified nurse is on duty throughout the day and night. Ancillary staff are employed in sufficient numbers to ensure that care staff do not need to undertake domestic and catering tasks. The home operates a good recruitment process although all appointments must be subject to a minimum of 2 written references. Staff are well-trained and receive training in subjects relevant to the care they provide. The home has a quality assurance process that seeks the views of all stakeholders. Staff supervision needs to be carried out more consistently. The home has excellent health and safety arrangements in place. EVIDENCE: The manager is well qualified, competent and experienced to manage the home. STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards.
