Conditions of registration: Date of last inspection 22nd February 2005 Brief Description of the Service: James Hirons is a care home providing personal care and accommodation for 24 older people aged 65 and over. It does not offer any specialist services. The home is located in Leamington Spa and is close to shops, pubs, the post office and other amenities. The home consists of a two-storey building with a newer ground floor extension and conservatory. All of the home’s bedrooms are single, and twenty of the bedrooms have ensuite facilities. There are two passenger lifts as well as a chair lift. The home has extensive gardens that are well maintained and easily accessible. This is an overview of what the inspector found during the inspection. This inspection was unannounced and took place over one day between the hours of 3.50pm and 7.00pm. This was the first visit for this inspection year. Staff co operated fully with the inspection. The senior carer on duty or the deputy manager was present throughout the inspection. The inspection process involved a tour of the home, talking with the deputy manager and senior carer, examining records and care plans, observation of care practices along with discussions with residents and staff. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 5 Residents are assessed before moving into the home. They are provided with the opportunity to visit the home to assess the quality, facilities and suitability of the home before they move in. EVIDENCE: The files of three residents were reviewed during this inspection. The admissions procedure included an assessment by the home completed before admission and provided the opportunity for the prospective residents and/ or their relatives to visit the home. Records showed that assessments had been completed and this provided the necessary information for the home to be able to provide the care needed. The community care plans provided by the social worker, as part of the individual needs assessment process, were seen within one resident’s care plans. A discussion with one of the most recently admitted residents evidenced that they had visited the home before making a decision to move in permanently. Other residents advised that either they or their relatives had visited the home to look round before they moved into the home.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 and 10 All residents have individual plans of care, which sets out their health, personal and social care needs in detail and enable staff to ensure that these needs are met. Personal support is offered in such a way as to maintain residents’ privacy and dignity. EVIDENCE: Three residents plans of care were seen at the inspection. These were clear and comprehensive and included risk assessments. Care plans and risk assessments are regularly reviewed and updated. Residents received dental, chiropody care and saw the optician. The home supported residents to attend outpatient appointments. All residents are registered with local General Practitioners (GP). Residents spoken with felt that their health care needs were being met advising that they saw the dentist, chiropodist, optician and GP or District Nurse when necessary. Staff were observed responding to residents in a sensitive manner. During the inspection it was observed that privacy and dignity were being afforded to residents, and there was very good interaction with staff. Care staff were seen knocking on doors before entering. All residents spoken with told the inspector that they were treated with respect, and that the staff were very kind and helpful. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 and 14 Contact has been maintained with relatives and friends of residents. Opportunities to access the local community has been made available. EVIDENCE: Those residents spoken with advised that they have good contact with their relatives and some go out with their family on a regular basis. Residents advised that their family and friends are welcomed at the home and are invited to attend parties and other celebrations. Some residents are able to leave the home on their own and continue with their day-to-day life in the community. Residents were seen to be relaxed and happy to be living in the home. The majority of residents were spoken with during the inspection and comments made include, ‘it’s lovely being here, I’m well looked after’ and ‘the staff are very good to us, they can’t do enough for us.’ Although the provision of recreational activities was not inspected on this visit residents spoken with were generally happy with the activities provided, but one resident said that when she was ‘sometimes bored as there wasn’t enough going on to keep her busy.’ James Hirons Home DS0000004250.V255533.R01.S.doc Version 5.0 Page 11 Complaints and Protection

Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 23 and 24 The standard of the environment within this home is good providing residents with an attractive and homely place to live. EVIDENCE: The home presents as comfortable and homely for residents. The communal areas and some bedrooms were seen during the inspection. There are three lounges and two conservatories available to residents. A split level dining room is also provided. Communal areas are in good order and the home was warm and comfortable. Observations of bedrooms visited indicate that residents are encouraged to bring in personal items to individualise their own space and that bedrooms are furnished based on their individual needs. A resident spoken with said she was very pleased with her room and ‘couldn’t ask for better.’ The home was observed to be clean and free from offensive odours. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 Service users are being supported by a skilled and knowledgeable staff team who understand their needs and wishes. EVIDENCE: The inspector observed the duty rotas for the period of a month. These demonstrated that although care staffing numbers were being maintained this was sometimes achieved by agency staff to cover shifts. The senior carer advised that if possible the same staff are requested in order to ensure consistency of care/service by suitably skilled staff to residents. A discussion demonstrated that correct procedures were being followed by the home when using agency carers, thereby ensuring the residents are protected by the home’s employment policies at all times.
