Conditions of registration: 1. 2. Persons of either sex, under the age of 65 years, who require care by reason of a learning disability (not to exceed 20 persons) 16 persons to be accommodated in Jamesons Residential Care Home, 2 persons to be accommodated in 32 Turner Road, Colchester, Essex and 2 persons to be accommodated in 58a Nayland Road, Colchester, Essex 4th May 2005 Date of last inspection Brief Description of the Service: Jamesons is a two-storey detached property situated in Fordham, a village on the outskirts of Colchester. The home is registered to provide care and accommodation to 20 adults with learning disabilities. Jamesons is owned by Mr and Mrs Catantan and managed by Mr Catantan who has many years experience of caring for this service user group. The accommodation in the main house is over two floors. Bedrooms are on the first floor for 12 service users, with each service user having their own room, one of which has ensuite facilities. Communal areas, including lounges and dining rooms are on the ground floor. The remaining four service users are accommodated within a newer, single storey extension to the rear of the home. Service users living within this area of the home have a more independent lifestyle and each of these single rooms benefit from ensuite facilities. The home is set in approximately 4 acres of land, most of which is accessible to the service users. Set within the grounds are a number of outbuildings that are equipped to offer daytime activities for those living at the home as well as up to six individuals who attend Jamesons from the community for day services. This is an overview of what the inspector found during the inspection. This inspection took place in one day, over 6 hours. In the course of the inspection a tour of the premises was made, 6 services users were met, discussions were held with 3 if these service users, discussions were held with the shift leader, the day care co-ordinator, the cook, 1 director and the residential manager/managing director and 3 support workers. 3 files were care tracked, 2 staff files were reviewed, as was health and safety information, policies and procedures. Service users said “(the staff) look after me” “I choose what garden”. do” “nice Staff were clear that “I’m really happy here,” “load’s of training” and of the manager “brilliant man, really understands people”. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3 Prospective service users are given clear and comprehensive information about the services provided in the home prior to moving in. There are opportunities to visit and move in on a temporary basis. EVIDENCE: The Statement of Purpose is comprehensive. The Service Users Guide is written in plain English and contains pictures for those who have conceptual difficulties. There is an audio version of the Service Users Guide. In conversation with the manager it was clear that effort is made to ensure that the staff try to make the service information accessible to all. The shift manager and residential manager outlined a clear process of helping individuals become familiar with the service through visits, prior to residence. The documentation is clear that a placement is initially for 6 weeks at which time there is a meeting to confirm whether a person is happy to staff or not. Care plans demonstrate that these meetings take place and that decisions are made and recorded. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 8, 10 Service users were aware of their care plans, that things change in the plans and that they had choice in what they do. EVIDENCE: In discussion with service users, they were aware of their care plans. Individuals spoke of the decisions that they had been involved in i.e. going to college or not, “I choose what do”. The service users are involved in choosing menus, in staff appointments and holiday arrangements. Service users are also consulted and involved in the day to day running of the unit, as evidenced by observation, the service users meeting notes and through discussion with service users and staff. In discussions staff modelled their way of dealing with sensitive material and checking with service users if material could be shared with the inspector. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,21 Service users receive personal support in all aspects of their lives, including physical and emotional health. EVIDENCE: Service users spoke of feeling helped by the staff and this was observed to happen. Care plans detail needs and what needs to happen to address needs. There was also evidence on files and through discussions of the range of medical and pars-medical staff who are mobilised to met identified needs. In discussion with staff there is awareness of the importance of planning with the service users in relation to their ageing, which includes planning for moving on at 65 years old and dealing with terminal illness. JUDGEMENT – we looked at outcomes for the following standard(s): 22, 23 Service users are protected from abuse and their views are listened and acted upon. EVIDENCE: The unit has a strong commitment to staff training, which includes training to protect vulnerable adults from abuse. The staff training programme and staff files shows training completed. Staff spoke of the training that they had received. There is a complaints procedure and a complaints and compliments book. Currently there are no outstanding complaints. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 25, 28, 29, 30 Service users have a homely environment, suited to their needs and choose their individual bedroom style. Shared spaces complement and supplement the residents’ own space. There is a need to provide paper towels in bathrooms and toilets to prevent cross-infection. EVIDENCE: All the rooms seen, individual and communal, were of a high standard of decoration and fit for purpose. One service user prized his room “nice room.” The communal spaces were well-furnished and showed attention to detail. Around the campus there are port-a-cabins and a barn. Each building has a designated purpose and is decorated and set out for the appropriate activity. The bathrooms and toilets now have liquid soap, but no hand drying facilities or in the case of the staff toilet/shower room a shared towel. This is a cross infection matter, which must be addressed. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 34, 36 Appropriately trained, competent and qualified staff supported service users’ needs. EVIDENCE: In discussion with staff and managers everyone was clear of their role and responsibility within the unit. The staff team were observed to work together and spoke of working as a team. There are regular staff training events, distance learning is encouraged and records show that there is a full induction of new staff. Individual staff spoke of how supported they feel in carrying out their work. The staff files show that there is a robust recruitment procedure, which includes the taking of references and Criminal Record Bureau checks prior to a person taking up posts in the organisation. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39, 40, 41, 43 Service users benefit from the ethos, leadership and management within the home, which promotes their welfare EVIDENCE: The leadership style within the home is open, positive and based on the needs and welfare of the residents being paramount. Staff praised their manager for his openness and availability. Workers spoke of having a positive experience of work within the unit, going to work was a good experience. The atmosphere within the home was active but not frenetic. All staff knowing what their task was, where they should be and what they should be doing. The policies and procedures are clear and have the residents best interests at their heart. All records looked at were up to date and are held securely. Observation through the day of inspection showed administrative systems to be in place and that the records held within the system to be meaningful in the delivery of service within the unit. All health and safety certificates were in place and up to date.
