If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service Jamesons is registered to provide care and accommodation to 24 adults with learning disabilities. 16 people are accommodated in the main house, a two-storey detached property situated in Fordham, a village on the outskirts of Colchester. The accommodation in the main house is on two floors. 9 service users have their own rooms on the first floor (one with en suite facilities, 3 service user have rooms on the ground floor. Communal areas, including lounges and dining rooms are on the ground floor. The remaining 4 service users live in a newer, single storey extension to the rear Annual Service Review Page 2 of 7 1 6 0 3 2 0 0 9 of the home. These service users have a more independent lifestyle and en suite facilities. The home is set in approximately 4 acres of land, most of which is accessible to the service users. Set within the grounds are a number of outbuildings that are equipped to offer daytime activities for those living at the home as well as up to six individuals who attend Jamesons from the community for day services. An additional 6 service users live off the main site, in separate annexes, supporting more independent living. A fourth Annex registered for the 2 service users is currently occupied. Jamesons is owned by Mr and Mrs Catantan and managed by Mr Catantan who has many years experience of caring for this service user group. At the time of inspection, the weekly charge for a room ranged from £1018.00 to £1425.42. This was subject to individual assessment and a review of needs after 3 months. The cost of dry cleaning, hairdressing, holidays, fee-based recreational pursuits, personal items, such as toiletries and newspapers, and specialist services, such as chiropody, were not included in the overall charge. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We (CQC) looked at all the information we have received, or asked for since the last key inspection. This included: The annual quality assurance assessment (AQAA) that was sent to us in December 2009 by the service. The AQAA is a self assessment that focuses on how well outcomes are being met for people using the service. It also gives us some numerical information about the service. The document is a legal requirement. We send surveys to the provision asking that they be distributed to stakeholders. We requested that they be completed and returned to us. This would provide us with information about the stakeholders views of the provision. We received 12 completed surveys from staff members, 17 from residents and 2 from health care professionals. We have included some of the comments within this report. Information we have about how the service has managed any complaints. What the provision has told us about things that have happened in the service, these are called notifications and are a legal requirement. We spoke with the manager by telephone on 15th March 2010. The last key inspection took place on 16th March 2009. What has this told us about the service? We reviewed the information within the AQAA, assessed the information on the service record, read the surveys and we spoke with the manager on the telephone. Within the AQAA under brief summary of our comments made within this Annual Quality Assurance Assessment and asked what the service does well, the service had recorded, Jamesons offer a high standard of residential care, in a homely and very informal atmosphere. We ensure that our service users are continually involved in a variety of daily activities with changes. We respect the dignity, privacy, the right to be heard and the right of each of or service users to be an individual including the terms laid out in our Equality and Diversity Policy. We welcome ideas from our service users and their families working as partnership. When asked how we have improved in the past 12 months, the service had recorded We have taken note of the comments of our inspecting agencies and where required we have taken the necessary steps to improve our standards in order to offer the best value for money. Under what we could do better and how we are going to do this the home had recorded, We will continue to monitor our polices and procedures and embrace new initiatives ensuring that change is managed in an affordable manner. The manager told us that the provision had a stable core group of staff. Staff had Annual Service Review Page 4 of 7 access to good training opportunities. We were informed that there had been no formal complaints received since the last inspection and no safeguarding adults referrals had been made to the local authority. We were informed that the provision had a good professional working relationship with all health and social care professionals. We were told that the provision was meeting the assessed care needs of all residents that were accommodated. From our records, we note that the provision contacts us when appropriate and necessary. We received the following comments from service users, Im able to see (name given) when I want to...I like the activities...I like my house that I live in...I get on well with all the staff. We received a good numerical response from residents and would acknowledge that many of the surveys could not be completed in full because of limitations. Where surveys could not be completed, the home had attached the following statement due to the cognitive impairment it was not possible to carry out this survey objectively as the client lacks capacity to understand what is being asked, even with help. We would acknowledge that other surveys had been completed by an advocate on behalf of individual named residents. Within the healthcare professionals survey it was recorded good personal care...good communication and liaison with GP services and other healthcare professionals...good at letting us know in advance if a patient cannot make an appointment. We received a very good numerical response to surveys from staff. Staff were open and transparent about their views and opinions abut the service. It was clear from the information within the surveys that staff are accustomed to being asked about the quality of the care provided. We noted that there were many positive things said about the service, for example provides good activities...residents well cared for...safe environment...the home is warm...cleanliness is good...family atmostphere...we have meetings regularly so we know what is going on...work as a team...managemnt and staff are easy to talk to...variety of activities for service users...provides training and courses for staff...regular meetings and supervisions...we have polices and procedures...we respect other peoples ideas, values and principles. We noted that some staff indicated that there were areas for development. For example, staffing levels, activities for residents, resources for residents, changes in the menu, clinical waste storage and better adherence by staff to the No Smoking policy. We discussed these aspects with the manager. The manager assured us that since the surveys had been sent (December 2009) these matters had been addressed. The manager told us about the provisions quality assurance policy. We were informed that all stakeholders are surveyed on a regular basis and the last Quality Assurance report was dated December 2009. The manager told us about the effective independent advocacy service that continues to be available to all residents. We do not have any current concerns about Jamesons. From the information available, the service continues to provide positive outcomes for residents. What are we going to do as a result of this annual service review? We can inspect the home at any time if we have concerns about the quality of the service or the safety of the people who use the service.
