Post operative respite care 2 Chronic disease management Date of last inspection 22 Feb 2005 Brief Description of the Service: Kenilworth Manor is a converted manor house, which has been extended to provide accommodation for 34 service users. The home is located in a quiet residential area of Kenilworth and is close to the town and its amenities. The home is registered to provide accommodation and care to service users over the age of 65 who may require nursing care. The home has specialist registration for the provision of terminal care. The current owners are Kenilworth Manor Ltd. The accommodation is provided on a number of floors with access to these via a passenger lift or stairs. The homes décor, furniture and furnishings are homely and of a very high standard. This is an overview of what the inspector found during the inspection. This inspection was unannounced and took place over one day. This was the first visit for this inspection year. Staff co operated fully with the inspection. The registered manager was present throughout the inspection. The inspection process involved a tour of the home, talking with the manager, examining records and care plans, observation of care practices along with discussions with residents, staff and two General Practitioners visiting on the day of the inspection. What the service does well: What has improved since the last inspection?
Since the last inspection the home have commenced the implementation of the Gold Standards Framework for Palliative Care, which are national best practice standards. This includes specialist training for staff members. The manager and staff have continued to improve procedures for the management of wound care in line with best practice guidelines. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10 and 11 There is a suitable care planning system in place, which provides the staff with the necessary information to meet individual residents needs. Health needs of residents are met with evidence of liaison with health and social care professionals on a regular basis. Procedures for the management of medications ensure the safety of residents. Personal support is offered in such a way as to maintain residents’ privacy and dignity. EVIDENCE: Individual care plans are available. Inspection of the records of three of the residents showed that the health, personal and social care needs are identified and planned. Care plans and risk assessments are reviewed on a regular basis. Evidence of resident and relative involvement in care reviews was seen on those files examined. One resident spoke of some concerns with regards to her specific plan of care the manager was dealing with this at the time of the inspection. Care files viewed showed involvement of members of the multidisciplinary team in assessing and meeting residents’ care needs. Two GP’s visiting during the inspection highly commended the care provided at the home. Systems for the management and administration of medications were observed and were satisfactory. Staff were observed treating the residents with dignity and respect during the inspection. Without exception residents said that the staff were very kind and caring and that they enjoyed living in the home. One resident said “that nothing was too much trouble for the manager and staff. It was like home from home.” The home is a pilot site for the implementation of the National Gold Standards Framework for Palliative Care. Evidence was observed of staff training in palliative care issues and liaison with Medical staff and Macmillan Nursing services. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13, 14 and 15 People living in this home are supported to maintain family links and friendships and continue to be part of the local community in which they live. Lifestyle in the home promotes and respects residents’ rights and responsibilities. Dietary needs of residents are extremely well catered for with a balanced and varied selection of food available that meets residents’ tastes and choices. EVIDENCE: There are no restrictions placed on visiting unless requested by residents. Four of the residents said that visitors are welcome at any time and that family and friends can arrange to take meals with them when visiting should they wish. Several of the current residents continue to access the local town and its facilities. One resident said that she regularly attends town and had been out for a meal the previous evening with family. Local churches visit the home and provide regular services and communion for those residents who may be unable to visit the church. The inspector ate lunch with the residents. The home continues to maintain very high standards with regards to the provision of meals and the surroundings in which they are served. The presentation and quality of the food was excellent. Residents commented enthusiastically about the quality of

Menus seen were nutritionally balanced and varied with good choices available. The home has achieved a good hygiene award. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 The standard of décor and furnishings is high with evidence of ongoing planned improvement and maintenance. The home presents as very comfortable and homely for residents. EVIDENCE: Kenilworth Manor is a converted manor house, which has been extended to provide accommodation for 34 residents. The accommodation is provided on a number of floors with access to these via a passenger lift or stairs. The décor, furniture and furnishings are homely and of a very high standard. The gardens are mature and well maintained and easily accessible to residents. Residents said that they enjoyed the gardens and took the opportunity regularly to walk/sit in the garden areas. The home was observed to be clean and tidy and free from odours at the time of the inspection.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 ,29 and 30 There is a complement of staff that have worked at the home for many years and staffing levels offer a good consistency of care within the home. The procedures for the recruitment of staff are satisfactory and protect the residents. Training is well planned and good opportunities are available for all staff. EVIDENCE: Duty rotas seen for the period of a month demonstrate that staffing is maintained within previously agreed levels. Staff do additional shifts to cover for sickness and annual leave. Staff and residents told the inspector that the staffing levels were suitable to meet current care needs of residents. The home is a placement for student nurses with three students commencing placement following the inspection. Letters from previous students were complimentary about the quality of care provided at the home and the quality of the learning environment. The inspector examined the records of three staff members, which included those who had recently been appointed. Each file contained evidence of suitable CRB checks, references and all other information as required by the standard. Training records were observed and demonstrated that good opportunities are available for staff training. The home has a good NVQ training programme with several of the care staff are currently undertaking NVQ ‘s.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 and 38 The quality management systems in place ensure that the home is run in the best interests of the residents. The health and safety of residents and staff are promoted and protected. EVIDENCE: The manager has implemented a number of formal quality audits with evidence of these seen during the inspection. Residents said that staff consult them about their personal circumstances and day-to-day things in the home. There are regular meetings where residents are consulted on day-to-day activities, food and outings etc. Questionnaires are sent out to relatives and residents on an annual basis. The results of the most recent survey were observed. Responses were very positive and complimentary about the home.

Reports of visits by the Fire Officer, Environmental Health Officer and Health and Safety Officer were observed during the inspection.
