This inspection was carried out on 5th January 2006.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

This home provides a safe and homely environment for its residents who were all very appreciative of the staff, many of whom have been there several years. Although most of the home requires redecoration and refurbishment all of those that were spoken to agreed how lovely the staff were and how well looked after they were. Comments were made "nothing was too much trouble" and that " it was a lovely place to live "and "you can always say what you like" and" everyone always gets on well together." All the residents commented how much they had enjoyed the recent celebrations that had occurred over the Christmas period and how lovely all the decorations had been.

What has improved since the last inspection?

Since the last inspection staffing levels have been increased and staff commented how much better this was and that it was allowing them to spend more time with the residents. Arrangements have been made to fit automatic door closers to bedroom doors to ensure residents safety in the event of a fire and some routine redecoration is occurring. The representatives of the home have been heavily involved in trying to update the management processes in operation and comply with current legislation and all of the comments received from residents reflected their commitment to the home. Documentation, including a Statement of Purpose and Service User Guide are currently being updated and staff teaching programmes are being considered. There had previously been concerns raised about the financial viability of the home however this appears now to have been resolved and work can now commence to comply with the requirements of The Commission.

Accommodation is arranged in a mixture of single and shared rooms over three floors. There is a large lounge and dining room and an attractive well-maintained rear garden, which is well used by the residents. All areas of the home are accessible to residents, including those who may have limited mobility, and a passenger lift, specialist adaptations and equipment have been provided. The home is situated in a quiet residential road in Surbiton and there is off street parking to the front of the property. This second, unannounced, inspection of the year 2005/2006 took place over four hours and was conducted with the representatives of the home, as the current owners are no longer living in England and the Registered Manager has recently left. There were lengthy discussions regarding the future management of the home and the progress being made to comply with previously issued requirements. During the visit a partial tour of the premises was undertaken, several of the residents were spoken to and also five staff members. Discussion centred on ensuring that residents felt safe and well cared for during this period of transition and that they felt their needs were being met. During the course of the two visits, all of those standards considered by The Commission to be key to the inspection process, have been assessed and this report should be read in conjunction with the one produced following the inspection undertaken on 21st September 2005. Since the last inspection, no complaints have been received about this service. What they could do better: As detailed in this report much of the home still requires redecoration and refurbishment. Carpets are worn and shabby, wallpaper needs replacing, some ceilings need repainting and wheelchairs are still being stored in the sitting room. The home is without a Registered Manager although strenuous efforts are being made to recruit a suitable person and the clinical responsibilities of the home are being very well managed by the senior nurses. Once this role has been filled there will need to be a commitment to staff training and development including supervision and appraisal. In view of all of the problems experienced recently by the home the timescales of any requirements that were not directly related to the health and safety of resident’s has been extended to allow them to be complied with. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9,10 The arrangements that are in place for delivering personal care ensure that residents can be sure that their dignity and privacy will always be respected and medication procedures are in place to protect them. EVIDENCE: During the inspection staff were observed treating residents with respect and dignity and residents confirmed that this was always so. Comments were received about how caring staff were and that they “were always kind to everyone”. All personal care is undertaken in resident’s rooms and screening is provided in shared rooms. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Activities within the home are organised in a way that suits the residents and visitors are always made welcome so that relationships can be maintained. Meals are served that are suitable and nutritious and so maintain the health and wellbeing of residents. EVIDENCE: Residents confirmed that activities are provided which suit their needs and they are able to choose whether they wish to participate. Some explained that they prefer to remain in their rooms and watch television or read. They said that their relatives and friends are always made welcome when they visit. Residents agreed that the food served was very good; they are visited by the housekeeper in the morning, and informed of the meals that will be served and offered a choice. Most of the residents eat together in the dining room although they are able to stay in their rooms if they wish to. A tour of the home illustrated that residents have been able to bring personal possessions in with them from home. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, Although it is clean, a programme of redecoration and refurbishment would make the home a more pleasant environment for residents to live in. EVIDENCE: The majority of the home, both bedrooms and communal areas, still requires redecoration and refurbishment. Carpets are worn and shabby, wallpaper needs replacing, some ceilings need repainting. No rooms have a lockable facility for the use of the residents. Wheelchairs are still being stored in the sitting room and could pose a safety hazard. On the day of the inspection the home was clean and tidy and free from odour. Automatic door closers, which operate in the event of a fire, need to be fitted to the bedroom doors of those residents wishing to keep them open however, a date to undertake this work has now been arranged. JUDGEMENT – we looked at outcomes for the following standard(s): 27, Residents can be sure that there are always sufficient numbers of staff on duty to meet their needs. EVIDENCE: Since the last inspection staffing numbers have increased and staff agreed that this allowed them to spend more time with residents. Once the management arrangements are finalised in the home there will need to be an emphasis on staff training, supervision and appraisal and these issues are already being considered. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,34 Residents cannot be confident that they currently live in a home, which is managed by a suitably qualified and experienced person who will safeguard their interests although they can be reassured that the home is financially viable and that they will be able to remain there a long as they need to. EVIDENCE: The home is currently without a Registered Manager although strenuous efforts are being made to recruit a suitable person. Clinical responsibilities are being very well managed by the senior nurses in the home. Since the last inspection, residents can be reassured that documentation has been produced as evidence of the financial viability of the home and this has been seen at meetings with the homes representatives. Recent visits have been received from The Fire Safety Officer and The Environmental Health Officer and an assurance was given that the minor concerns raised had been dealt with.
