Conditions of registration: 1. 16 single rooms and one double room Over 39 years, 18 people with learning disabilities Over 65 years, 18 people with learning disabilities One service user with a physical disability One service user with dementia whose date of birth is 24.02.1934. 16th March 2007 Date of last inspection Brief Description of the Service: Kingswood Lodge is located in the centre of Gillingham and is close to the main railway station and other local transport. It is a short walk from the high street and other amenities. There is car parking to the rear of the property and the garden is attractively laid out with raised flowerbeds, a lawn and seating area. The home provides accommodation on three floors with sixteen single rooms and one shared room. There is a lift to the first floor. There is a games room and a choice of sitting rooms. There is a spacious dining area with a conservatory attached. Information relating to the fees and other charges may be obtained from the provider. This is an overview of what the inspector found during the inspection. An unannounced visit was made to the premises on 22nd November 2007 as part of the Key inspection of Kingswood Lodge. Time was spent talking with the staff on duty and some of the service users. The opportunity to discuss with all the service users, their experiences and opinions of the home was limited by their involvement in activities and their inclination and ability to communicate or not. Observations indicated that they were settled in the home, were comfortable in the company of staff and had plenty to do. Throughout the visit, the staff’s attention to the service users’ needs, their patient, friendly and respectful manner and their treatment of each service user as an individual were observed and noted. A range of documentation and records was viewed and an accompanied tour of the premises was undertaken. Additional information and detail was supplied by the organisation in the Annual Quality Assurance Assessment. Survey/comment forms were distributed prior to the visit. Eleven service user, five relative and one care manager forms were completed and returned. The comments were generally positive and included: “ X is very happy there” “The staff genuinely care “ “It’s a happy environment” “Staff are pleasant and helpful but I feel they are overworked” “ X is cared for excellently and has a good life at the home” Suggestions for improvement included: a few more varied activities; health update reports to parents. Not all NMS were inspected at this visit. Unless noted, only the core standards were inspected in each outcome group. What the service does well:
There is a well thought out range of activities available based on the individual needs of the service users. These include activities at home, trips out and attendance at the local centres. The staff work positively with the service users helping them to communicate their needs and develop their skills. Service users’ views and opinions are sought and considered wherever possible

Prospective service users and their representatives may visit the home before admission and are provided with information to help them make a decision about moving in. Assessments are undertaken to ensure that the Home can support service user’s needs This judgement has been made using available evidence including a visit to this service. EVIDENCE: Staff spoken with confirmed that prospective service users and their representatives are invited to visit the home prior to admission. They may use the opportunity to meet and spend time with the existing service users and staff, view the accommodation and find out about the routine and lifestyle they could expect to experience at Kingswood. Assessments are undertaken prior to, and subsequent to admission as part of the care planning process. Senior staff may visit the prospective client in their current accommodation as part of the assessment. Quality in this outcome area is good The care planning system is generally clear and consistent and provides staff with information and guidance when supporting residents. Risk assessments are undertaken in order that service users can participate in activities with the appropriate level of support and supervision This judgement has been made using available evidence including a visit to this service. EVIDENCE: The home has developed reasonably well-structured and informative client files. The content and detail provided enables new and existing staff to better understand and effectively support the needs of service users. The Manager reported that the emphasis in the care plan documentation is moving towards ‘person centred’ information, and guidelines will relate less to physical care of the individuals and more to supporting social, educational and therapeutic goals and aspirations.

Service users are helped, as far as possible to make day-to-day decisions affecting their lives with regard to activities, outings, routines and food choices amongst other things. Service users were directly observed expressing preferences and making choices prior to going out with staff to the shops and when discussing the evening meal. Risks are viewed positively by the home and assessments are undertaken in order that service users can participate in activities with the appropriate level of support and supervision. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16,17. Quality in this outcome area is good The home provides a range of leisure and social activities based on personal needs, wishes and interests. Family and friends are welcomed to the home and contact with the local community is maintained. Service users enjoy a varied and balanced diet. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Staff in the home appear to have worked positively with the service users to establish likes, dislikes and interests. They are supported by the home to provide, and help service users access, a range of activities and events for both recreational and therapeutic purposes. At the time of the visit a number of service users were with staff, visiting shops and having lunch out. Two

The Manager reported that she has endeavoured to foster a positive and active culture within the home, one that respects an individuals own needs, inclination and abilities. Regular bi-monthly house meetings are held where it is hoped that every one will contribute to the topics discussed and to the decision making process. In response to the last inspection report particular attention was paid to determining service user preference for activities and holidays. As a result some will go as a group to a holiday centre whilst others are more comfortable remaining closer to home and having special days out. The home has also provided computing facilities for one service user in particular and for others to use as well. Family and friends are welcome to visit. Service users confirmed that they enjoy their meals. Three meals are provided every day, one of which is a cooked meal. Snacks and drinks are available at all times. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20. Quality in this outcome area is good Service users are treated with respect and their privacy respected. Healthcare monitoring and medication systems are in place to promote the well being of service users. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Service users are treated with dignity and respect and levels of privacy are maintained. The service users require assistance with aspects of personal hygiene and care. The manager endeavours to ensure that staff are instructed and supervised to provide this thoughtfully and sensitively. The healthcare needs of service users are monitored and addressed. The home has developed positive relationships with the local healthcare agencies that provide support and advice. Specialist staff are consulted as required and the outcome of visits noted in case files.

The home has procedures in place for dealing with medicines. It was reported that the Manager deems staff that currently administer medication, competent by experience and qualification. Systems are in place to protect service users, as far as possible, from the risks of harm or abuse This judgement has been made using available evidence including a visit to this service. EVIDENCE: It was reported that protection of vulnerable adults training is offered by the Organisation. Staff spoken to indicated an understanding of issues relating to the protection of vulnerable people. They are aware of their role in ensuring people living in the home are protected from abuse in all its forms and that they are the service users main key to having their views heard. It was confirmed, verbally and in comment forms that service users had been made aware of the appropriate procedures and would happily approach staff and management if they had any concerns or complaints. Service users at live in homely, comfortable and seemingly well-maintained environment. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Kingswood Lodge is located in the centre of Gillingham and is close to the main railway station. Local amenities are within walking distance, as is public transport. The home does however have it’s own transport. The home provides accommodation on three floors with sixteen single rooms and one shared room. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34,35. Quality in this outcome area is good. There are sufficient staff on duty to support the service users. Staff are clear about their roles and responsibilities and are offered good training opportunities to develop skills and expertise Staff are encouraged and supported to attend induction and statutory training courses. The management indicated a good awareness of the need to ensure adequate checks are made on all potential staff to determine suitability and protect the Service Users. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Staff were observed interacting with the service users in a friendly and non-patronising manner. They indicated an understanding of the service users’ needs and managed behaviours in a patient, calm friendly manner. Service

It was reported that day time staffing levels of four per shift plus the Manager are adequate for the demands of the current group. This would be reviewed and revised should the need arise or the size of the group change. According to information received 10 of the 16 staff have completed or are working toward their NVQ award level 2 or above. The home provides induction, ongoing mandatory and client specific training. The manager reported that the induction programme embraces the Common Induction Standards from Skills for Care. The training matrix indicates the completion or due date of training identified as being; statutory; essential or developmental. The only areas showing low attendance were COSHH and Dementia awareness. Staff records were viewed and indicated that the recruitment procedures are thorough and include interviews, acquiring of references and Criminal Record Bureau checks. The home appears to be well run in an inclusive manner that allows for service users and staff to contribute to the daily life and routine. The home is supportive, encouraging the ongoing development of the service users. EVIDENCE: Kingswood Lodge DS0000028918.V352609.R01.S.doc Version 5.2 Page 22 The Manager appears to be a well-informed, experienced and well-organised individual who ensures that service users are consulted as appropriate. Staff seem aware of their roles and responsibilities and are encouraged to contribute to the development of the service. It appears from discussion and observation that the Management is approachable and supportive. She operates an open door policy and frequently meets with staff individually and collectively on a formal and informal basis. An open and inclusive atmosphere appears to have been established within the Home. A representative of the organisation regularly visits the home to monitor the standard of care and relevant documentation and ensure the premises are in good order. Quality assurance surveys are sent to service users and their relatives annually and the owner produces a report for the home of the findings of this exercise. It was reported that relatives are fully informed, involved and consulted on any matters that impact upon the experience and quality of life for their relative. The health, safety and welfare of service users, staff and visitors is addressed in induction and training and through written guidance. Information in the pre inspection questionnaire indicates that the maintenance checks and certification are satisfactory and up to date.
