The minimum staffing levels to be maintained for Unit 1 (Eardisley), with 16 places for older persons requiring personal care to include a maximum of 6 dementia places (DE/E) must be 360 separately rostered care hours per week. The minimum staffing levels to be maintained for Unit 2 (Logan Jack & Staunton Wings), with 32 places for care with nursing to include a maximum of 3 physical disability (PD), 1 terminal illness (TI) , 1 terminal illness (TI/E) and 27 older persons (OP) must be 981 separately rostered care hours per week. All care staff employed to provide care for service users, with dementia and as part of the Home`s mandatory training, must begin NVQ Level 2, Unit CLI & W2 within the Care Homes for Older People

Date of last inspection Brief description of the care home Kington Court can provide personal care and accommodation for 16 older people, 12 intermediate care places, which includes two social services re-ablement beds, eight for older people and four for people under the age of 65, as well as 20 nursing places for older people. In total it is registered to accommodate 48 residents. The home is owned by Blanchworth Care Ltd and is located on the outskirts of the small market town of Kington. It is close to shops, and other amenities. The home was purpose built and opened in June 2003. It consists of a two-storey building, which was built to provide a Health and Social Care centre for the town and surrounding rural area. The homes facilities are entirely situated on the first floor of the building. They are divided into three units; Eardisley (personal care), Logan Jack (intermediate) and Staunton Wing (nursing). The home offers 46 single rooms and one double room, all with ensuite toilet and shower facilities. There is a passenger and a service lift provided. The home has gardens at the rear and to the side of the building that are accessible via the ground floor lounge/dining area or outside via an entrance off the car parking area. The home is accessible throughout by people with a physical disability who require the use of a wheelchair. A range of equipment is also provided for residents with physical disabilities. There is no information in the statement of purpose and service users guide about the range of fees charged by this service; fee levels are available on request. We, the Commission, carried out this random inspection of Kington Court to check the current management arrangements for the service. This involved us making an unannounced visit and spending just over two hours at the home. During this time five staff were interviewed, staffing rotas were checked and an overall impression was obtained of the home. There were twenty seven people using the service on this day. The last key inspection of the service, which was undertaken on the 4th of June 2008, found that there were good quality outcomes for people using the service. There had previously been issues at the home however that had resulted in poor outcomes for some residents and also related to staffing levels and training. This had resulted in referrals being made under local authority safeguarding procedures and to the service being closely monitored by the Commission. We had subsequently issued four statutory requirements notices and a notice of proposal to withdraw the homes registration. Having recognised that significant improvements were made it was subsequently decided to withdraw the notice of proposal for cancellation. Information has recently been brought to our attention however indicating that the service may be vulnerable again due to management arrangements and because the home is now operating at a much higher capacity than it was at the last key inspection. What the care home does well:
The manager Mrs Nicola Warman was registered in respect of the service on the 22nd of August 2008. Mrs Warman is supported in the management role of the home by the deputy manager, six qualified nurses and two senior care staff. Although there is currently one vacancy for a qualified nurse staffing rotas show, and staff interviewed confirm, that management and senior staff are able to cover the home. Mrs Warman normally works four days a week with her hours supernumerary to those of the nursing and care staff teams. Staff report that they feel well supported by Mrs Warman and the deputy manager who they say are approachable and always available to give them advice and support, both when they are at the home and on-call. Staff also say that the providers area manager visits the home regularly and provides good on call support to the home. Whilst Mrs Warman and the deputy manager normally work the same shifts, and have holidays at the same time, the senior nurse who provides cover in their absence says that additional visits and support are provided by the area manager when they are not available. The evidence obtained indicates that the home is appropriately staffed, having at least seven staff on duty between 7.00am and 7.00pm; four between 7.00 and 10.00pm and three waking night staff. Whilst some agency staff are being deployed to cover shifts, this is usually no more than one person a day during the day time and one person covering a night shift two or three times a week. The same agency staff are also working regularly who know the home and residents well, which is good and should help to promote consistency of care provided. Care Homes for Older People
Page 4 of 8 Those areas of the home visited are well maintained, decorated and furnished. They were all also found to be warm, clean and fresh. There was a calm and relaxed atmosphere throughout the home and staff did not appear to be rushed. Staff were observed to interact respectfully and in a friendly way with residents. Relatives and professionals who were visiting were made welcome by staff and seemed to be comfortable at the home.
