What the care home does well

A very comprehensive activities programme is in place. A new activities coordinator has recently been appointed to assist in this area and residents are involved in the choice of activities and entertainment as well as in fundraising efforts. Good contacts have been made with the local community, with both school children and adult groups involved in visiting the home. All of the residents seen appeared well groomed and well cared for. Those spoken to expressed their satisfaction at the care provided and comments such as " couldn`t be better" and "the staff are very good " were made.

What has improved since the last inspection?

Three resident rooms have been redecorated to ensure a homely environment. Staff recruitment has improved leading to decreased usage of agency staff. This ensures better continuity of care for the residents.

What the care home could do better:

The provision of care for those residents with a diagnosis of dementia continues to be provided in an integrated manner with the physically frail elderly. This leads to frustrations for the non-demented residents who are disturbed by their behaviour and noise, and also to difficulties in providing specialist activities and supervision for this group. The environment in the home has not been adapted to meet the needs of residents with dementia, and may therefore prove hazardous to them, with such things as electric kettles and unsecured exits. The company has been required to address these issues.

The accommodation for service users is set out on the ground floor. The first floor locates the Registered Manager’s office and a small area used for training and a staff room. Service User’s rooms are divided into small individual areas called flats. Each flat has a small lounge and dining area and the Service User’s bedrooms. All rooms are single occupancy. There is a communal lounge area and a larger foyer area where Service Users frequently choose to sit. The home has a wide corridor that is called the street, where tables and chairs are placed for the Service User’s use. Located in this area are a bar and a shop for the Service User’s use. The home is surrounded by pleasant gardens and is situated in a residential area close to the town centre and other local amenities Lancum House Care Home C51 C08 S60026 Lancum House V234165 260705 Stage 4.doc Version 1.40 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. Two hours were spent prior to the inspection reviewing previous requirements and recommendations, comments from residents and relatives and collating information provided by the service. The inspection took place over a period of four hours as part of the statutory inspection programme. Two residents were chosen in order that their experience in the home could be monitored. This included looking at their records, talking to them and also to the staff concerning the care received. In addition to this staff training and medication records were seen. Eleven comment cards had been received from residents, one comment card from relatives and information was available from a questionnaire completed by the providers of the service. Eight of the residents who commented were entirely happy with the service provided. Two were unhappy with the activities provided and one was unhappy with the choices offered at tea- time. Teatime menus were discussed with the Registered Manager, who stated that individual choices could be catered for on request. The relative, who commented, felt that the new company were not as friendly as the previous providers and were too conscious of company image, although there was no evidence of this at the time of the inspection when there was a happy, friendly atmosphere in the home. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 and 3 Residents are aware of their terms and conditions of residence.The needs of the elderly frail residents in the home are compromised by the integration of dementia clients throughout the home. The needs of dementia clients are not always met in the home. EVIDENCE: Copies of Terms and Conditions were available in the resident files checked. Comprehensive assessments had been completed on the service users case tracked and included physical and social needs. There is no separate provision for residents with a diagnosis of dementia in the home. No specific activities are provided for this group and the environment is not always suitable. For example kitchenette areas have electric kettles and washing up chemicals freely accessible to these residents. The home has many outside doors, which present a risk to these residents who may wander out and come to harm in the community. One elderly resident stated that she spent the afternoon in her room as the constant repetition got on her nerves. A requirement was made in this respect. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 and 10 Good systems are in place to ensure that all aspects of resident need are identified and documented for staff guidance, to ensure that these needs may be met in the home, but residents with a diagnosis of dementia are not always cared for in an appropriate setting. EVIDENCE: A selection of service users were chosen to case track. Records for these demonstrated that comprehensive care plans were in place for all of their assessed needs. Clear instruction was available concerning individual needs, choices and preferences. Specialist mattresses, cushions and other aids had been provided according to assessed need. All service users are registered with a local General Practitioner and evidence of doctor and Professions Allied to Medicine visits was seen in the case files seen. Records of hospital appointments were also available. The District nursing service provides nursing care to those who require this. Those service users case tracked had Medication profiles in place and had been assessed for their ability to self medicate. Service users spoken to confirmed that they were treated with dignity and respect and staff were observed to knock before entering individual units as well as service user rooms. As stated above, the specialist needs of those residents with a diagnosis of dementia are not always catered for in the home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15 Social Activities and meals are both well managed, creative and provide daily interest and variation for people living in the home. EVIDENCE: Records of individual choices were seen in the files checked. Service users and staff confirmed that they are able to choose their time of rising and retiring and where in the home to spend their day. Residents were observed to be sitting in their individual rooms or in the communal areas according to individual preference. One lady said she spends the morning in the lounge and then watches television in her own room in the afternoon, owing to the disturbance from residents with a diagnosis of dementia in the lounge. A small shop is available on the premises staffed by volunteers. The inspector was informed that fund raising takes place to provide activities and that a residents committee decides on the entertainment to be arranged. Preparations were in progress for a planned Pig Roast, a trip to the races, a trip to the seaside and Halloween and Christmas parties. Links have been forged with a local primary school, ballet school and Lions Club who provide entertainment for residents in the home. Visitors are welcome in the home at any time and may be seen either in communal areas or in their own rooms. One lady was enjoying a visit from her daughter at the time of the inspection. All of the resident comment cards received confirmed that staff treat them with dignity and respect. Staff were observed to knock before entering individual flats and resident rooms. Menus provided as part of the pre inspection information, demonstrate that a varied, nutritious diet is provided. Residents spoken to confirmed that the standard of food was good and that choices were offered at all meals. Meals are served in the dining areas in individual flats and are served at attractively set tables. One resident who completed a resident comment card, stated that the variety offered at teatime was not very imaginative as one of the two choices on offer was always sandwiches. The registered manager was taking steps to address this area but stated that other alternatives may be provided if individuals request this. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 The company takes all complaints seriously and acts upon them. Residents are protected from abuse. EVIDENCE: No complaints have been received by the Commission for social Care Inspection since the last inspection. The homes own records demonstrate that no formal complaints have been received but that concerns raised by residents, visitors and staff are taken seriously and investigated in order to resolve the problem. The company has a complaints procedure, which is on display in the home. Staff are trained in the recognition and reporting of abuse. Two training refreshers have been organised for the month of August. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 23, 24, 25 and 26. Residents live in homely surroundings, which are well maintained and clean. EVIDENCE: Communal lounges are kept clean and homely, with comfortable furnishings and designated smoking areas. External grounds are kept attractive and tidy. Shaw Health Care has plans to modernise and upgrade facilities in the home as part of a programme of development agreed at registration. The inspector viewed the bedrooms of the service users who were being case tracked. These were decorated in a homely manner with evidence of small items of personal belongings, such as photographs and ornaments on display. Three residents rooms have been redecorated since the last inspection and residents had chosen the colours. A full time handyman has recently been employed and he is busy redecorating areas of the home which have become shabby, to ensure a pleasant facility whilst the upgrading and redevelopment are awaited. The Registered Manager stated that he has had a budget allocated for the replacement of some armchairs and carpets within the home. The home was well lit and warm and comfortable throughout. Laundry is carried out in an area separate from the kitchen. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, and 30 Staff training and development is encouraged in the home, to ensure that a high standard of care is provided to residents. EVIDENCE: Duty rotas provided as part of the pre-inspection questionnaire, demonstrate that staffing levels are calculated according to resident dependency and are adequate to meet their needs. Staff recruitment has improved since the last inspection, leading to a reduction in the use of agency staff, and so improving the continuity of care provided. A training matrix is available to demonstrate staff attendance at training sessions. The company employs a training manager to oversee the training in this group of homes. New staff are required to attend an induction and foundation training to comply with the Training Organisation for Personal Social Services guidelines. The Team Leader on duty undertook to arrange training on a specific area identified at the time of the inspection to ensure that staff were aware of best practice. The pre inspection questionnaire identified that 43 of staff currently hold a National Vocational Qualification in care, and the Registered Manager confirmed that three staff are currently working towards this qualification, with a further two registered to commence in September. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, and 36. The Registered Manager ensures that the home is run efficiently, in a way that serves the best interests of the residents. EVIDENCE: The Registered Manager has many years experience in caring for this resident group and holds a National Vocational qualification level 4 in care as well as the Registered Manager’s Award. He operates an open door policy, and staff and relatives freely approaching him during the inspection demonstrated this. Monthly residents meetings are held at which they are welcome to air their views. The plans for the redevelopment of the home are on display in the main entrance so that every one may be aware of the proposed changes. Staff receive formal supervision every four to six weeks and a system of annual appraisal is being introduced in October.
