Lancum House is a purpose built residential care home for the elderly, formerly owned by the Northamptonshire County Council Social Services Department, and now owned by Shaw Healthcare. The accommodation for service users is set out on the ground floor. The first floor accommodates the managers office, and an area used for training and as a staff room. Service users rooms are divided into small individual areas called flats. Each flat has a small lounge and dining area, all bedrooms are single occupancy with ensuite facilities. There is a communal lounge area and a larger foyer area where service users frequently choose to sit. The home has a wide corridor that is called the street, where tables and chairs are placed for the service users use. The home is surrounded by pleasant gardens and is situated in a residential area close to the town centre and other local amenities. The home caters for elderly residents of both sexes with both physical and dementia related illness. The current fees are in the region of 450 pounds per week according to assessed needs.

What the care home does well Prescriptions are photocopied each month before being sent to the pharmacy to be dispensed, so that an up to date record of each person`s prescribed medicines is available. What the care home could do better: We found a number of gaps in administration records and saw that the daily check designed to correct these mistakes was not always being done. Care Homes for Older People Page 3 of 8 What we found: We watched medicines being administered to people in two areas of the home and saw that a safe procedure was followed. We looked at medicine records in these two units, and at the storage and recording of controlled drugs. We found that a previous requirement about handling medicines, with a deadline of 22nd October 2009, had not been met. We learned that prescriptions are photocopied each month before being sent to the pharmacy to be dispensed, so that an up to date record of each persons prescribed medicines is available. We saw that the receipt of medicine into the home was recorded on each persons medicine chart. We also saw records of the disposal of medicines. We checked the homes controlled drugs register and a persons medicine chart and found that the administration of controlled drugs was correctly recorded. However, we found that two controlled drugs entered in the CD register were not in stock in the home. These drugs were no longer required but there was no record of their disposal. We looked at records of administration on medicine charts and found that people did not always receive their medicine as prescribed. One person had missed an important medicine on two evenings because he had been asleep. A dietary supplement was only being given to another person in the morning, instead of three times a day, as prescribed. There were a number of gaps in administration records and the daily check designed to correct these mistakes was not always being done. There was no written guidance (protocol), in addition to the information on the pharmacy label, to help staff decide when medicines prescribed to be taken as required should be given. One persons medicines had recently been changed by the doctor. The changes were not clearly explained in the persons care plan or on their medicine chart. The disposal of the medicines no longer required by this person was recorded. We found that a few medicines were labelled take as directed by the pharmacy. The medicine refrigerator was operating at the correct temperature but no minimum/maximum thermometer was available for checking the temperature over 24 hours.
