Date of last inspection Brief Description of the Service: Langley Lodge provides nursing care and accommodatio for up to a maximum of twenty-six older people including up to a maximum of two people who have been diagnosed with a terminal illness. The home is situated in a quiet residential area of Westcliff on Sea, close to the sea front, Chalkwell Park and the local shops and amenties on London Road. Langley Lodge is a large well-maintained older style property. The home provides fourteen single bedrooms and six double bedrooms. People living at the home have access to a small dining area, large lounge / conservatory and a large well maintained garden area. This is an overview of what the inspector found during the inspection. This was a routine unannounced inspection carried out on 21st March 2006 between the hours of 11.30 and 15.30. Carolyn Delaney, Lead Inspector for the home, carried out the inspection. Records including care plans and assessments in respect of residents’ needs and risks to health, safety and welfare were examined. Four residents and three relatives were spoken with during the course of the inspection. The relatives of four residents at the home were contacted by post so as to offer them the opportunity to make comments about the services provided by the home. A summary of the comments made will be included in the final version of the report. Five members of staff, including the homes manager were spoken with during the course of this inspection so as to determine their awareness of the needs and wishes of the people living at the home and the homes policies and procedures. Key standards as identified in the intended outcomes sections of this report are inspected at least once every twelve months. Where key standards have not been inspected on this occasion they will have been inspected at the previous inspection. Langley Lodge provides a well-maintained, comfortable and homely place for up to twenty-seven older people who require nursing care. The home has a team of staff many of who have worked at the home for a number of years and this ensures continuity of care. Staff keep up to date and accurate information about the nursing and general care needs of the people who live at the home so that all staff are aware of each persons specific plan of care. Risks to resident’s health and welfare are assessed and care taken to minimise these risks. There have been no complaints made about the services and care provided by the home for over a year and all those spoken with on the day of the inspection said that they were very happy with the care provided.

Overall the home is managed in the best interests of the people who live there and their needs and wishes are met. What has improved since the last inspection? The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2&3 People are only admitted to Langley Lodge following a detailed assessment of their nursing and care needs so as to ensure that the home can best meet these needs. People who live at the home are provided with a contract of general terms and conditions in relation to their placement. EVIDENCE: Each individual is provided with a contract of terms and conditions in respect of the placement when they move into the home. The pre-admission assessments for four people who live at the home were examined and these indicated that a comprehensive and detailed assessment of each persons nursing, safety and general care needs was undertaken before they were offered a place at the home. Those residents and their relatives who were spoken with during the inspection all commented that the home met their needs and that they were happy with the care provided

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10 & 11 Information about each persons nursing, safety and general care needs is clearly recorded and kept up to date. Staff ensure that residents receive at appropriate times medicines and treatments as prescribed for them. Residents are treated with respect and their wishes taken into consideration when care and treatment is provided. Resident’s wishes for care and treatment in respect of end of life and arrangements following death are not always recorded. EVIDENCE: Staff record detailed information about each person nursing and general care needs and this information is reviewed and amended according to changes in the individuals condition or treatment. Where risks to a person’s health or safety are identified there is a clear plan so as to minimise these risks.
Langley Lodge DS0000015540.V253934.R01.S.doc Version 5.0 Page 11 It is noted that a number of people had been admitted to the home from hospital with severe pressure area damage and that these people had been treated appropriately and the advice of Tissue Viability Specialists sought as required and that these individuals pressure sores had improved greatly. There were detailed records maintained in respect of visits made to residents by other healthcare professionals including general practitioners and physiotherapists. The home has clear and detailed policies and information regarding the safe administration, storage and disposals of medicines. Medication Administration Records (MAR), which were sampled, indicated that staff acted so as to ensure that each resident receives medicines, which have been prescribed for them. One resident receives oxygen therapy as required and this should be clearly recorded in the person’s plan of care. It was noted that resident’s wishes in respect of the care and treatment they would wish to receive and their preferred arrangements following death were not always recorded. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 14 & 15 The range and frequency of opportunities provided for stimulating and meaningful activities does not always meet the needs and wishes of all of the people who live at the home. Residents are assisted and encouraged to make choices about how they wish to receive care and treatment. Residents are provided with a well-balanced and varied diet, which meets their dietary requirements and wishes. EVIDENCE: The majority of residents who were spoken with said that they were happy with how they spent their days. There was evidence that staff were aware of their preferences for receiving care and that the routines and practices in the home were flexible so as to accommodate individuals wishes. One residents’ relative commented that the resident was often bored, as she did not have any company in the afternoons and evenings. The residents’ sight and hearing had deteriorated which made it difficult to participate in some of the activities provided by the home. It was not clear that this had been taken into consideration or that alternative opportunities for stimulation and occupation had been offered.

On the day of this inspection residents were offered the choice of fresh fish or sausages which all said they enjoyed. It was noted that three people had been admitted to the home with PEG feeds who were now enjoying taking food and fluids orally. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 Langley Lodge has a detailed policy and procedure for dealing with complaints. There have been no complaints made since the last inspection. All staff working at the home receive training and information so as to ensure that they act so as to protect the people living there from harm or abuse. EVIDENCE: All staff working at the home have received recent training provided by the local Borough Social Services in respect of the protection of vulnerable people and staff practices are monitored and supervised so as to minimise the risk of harm or abuse of any person living at the home. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 26 Langley Lodge provides a safe, clean, comfortable and homely environment for those people who live there. EVIDENCE: Langley Lodge provides accommodation in fourteen single bedrooms and six shared bedrooms, which may be personalised according to each individual’s wishes. The communal areas are very homely and comfortable in nature with a large bright conservatory / lounge area which leads out into a well-maintained garden. Residents may take their meals and receive visitors in the dining room or their bedrooms if they choose. Two bedrooms on the ground floor have doors, which lead out into separate patio style areas of the garden.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 & 30 Staff are employed in suitable numbers and with appropriate skills and knowledge so as to meet the needs of the people living at the home. EVIDENCE: All staff working at the home have appropriate off duty time and do not work excessive hours without a break so as to ensure that they are fit to carry out their duties and best meet the needs of the people who live there. The staff rota indicated that two registered nurses and four care staff worked at the home for the morning duty, one registered nurse and three care staff worked the evening duty and one registered nurse and two care staff worked the night duty. In addition the home employs dedicated catering, domestic and maintenance staff. There had been no new staff recruited to work at the home since the last inspection. There were training files maintained in respect of each person working at the home and there was evidence that each individuals specific training needs and any particular wishes were recorded. Staff who were spoken with during the inspection could demonstrate a good knowledge of the residents needs and preferences and residents and their relatives commented that they were very

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 36, 37, 38 Langley Lodge is well managed in the best interests of the people who live there. The home is well maintained so as to protect the safety of residents, their visitors and the people who work there EVIDENCE: Langley Lodge continues to be managed in an open and positive manner and each of the regulatory requirements identified at the last inspection have been met. Staff liaise with residents and their families so as to best meet the needs of the people living at the home. Staff receive supervision and support so that they can carry out their duties in the way that best serves the interests of the people living at the home. Some

All records as required and requested were available and maintained in good order. The home is well maintained and records were available in respect of the repair and maintenance of gas, fire, electrical and mechanical equipment used in the home.
