This document summarises the inspection findings of the CSCI in respect of Larchwood Nursing & Residential Home. The inspection findings relate to the National Minimum Standards (NMS) for Care Homes for Older People published by the Secretary of State under the Care Standards Act 2000. The Regulations applicable to the inspected service are secondary legislation, with which a service provider must comply. Service providers are expected to comply fully with the National Minimum Standards. The National Minimum Standards will form the basis for judgements by the CSCI regarding registration, the imposition and variation of registration conditions and any enforcement action. The report follows the format of the NMS and the numbering shown in the report corresponds to that of the Standards. INSPECTION VISITS Inspections are undertaken in line with the agreed regulatory framework with additional visits as required. This is in accordance with the provisions of the Care Standards Act 2000. The report is based on the findings of the specified inspection dates.Larchwood Nursing & Residential HomePage 4 BRIEF DESCRIPTION OF THE SERVICES PROVIDED. Larchwood Nursing and Residential Home is situated on the outskirts of Norwich, within easy access to a large supermarket, post office and local shops. It is owned by the company of Bondcare. It is a two storey building with access to the first floor by shaft lift and stairs. The home can accommodate up to 48 older people, with 25 nursing needs and 23 residential needs, within 36 single and 6 shared bedrooms. All the bedrooms have en-suite facilities. There is an enclosed patio area with seating and well maintained raised flower beds, that are accessible to wheelchair users. The home has ample car parking facilities at the rear of the premises.Larchwood Nursing & Residential HomePage 5 PART A SUMMARY OF INSPECTION FINDINGSINSPECTORS SUMMARY (This is an overview of the inspectors findings, which includes good practice, quality issues, areas to be addressed or developed and any other concerns.) This inspection was conducted as an unannounced inspection during the lunch time period. Several service users and relatives spoke to the Inspector and gave some positive comments regarding their care within the home. Choice of Home (Standards 1-6) Standards 1,2,3,4,5,6 were checked and met. The home has a statement of purpose and service users guide in place and these are available for all prospective service users. There has been a change of manager and these details will be included in these documents once registration has been concluded. Service users are encouraged to visit the home prior to admission. Health and Personal Care (Standards 7-11) Standards 7,8,& 9 were checked and met. Service users have a care plan in place to enable personal, social and medical needs are met. The home is supported by local GP practices. Daily Life and Social Activities (Standards 12-15) Standards 12 & 15 were checked and met. The home displays a selection of activities offered during the week. An Activities Co-ordinator arranges the majority of social events. During the inspection there was a quiz with several service users and relatives taking part. Service users were offered a choice from the menu. Complaints and Protection (Standards 16-18) Standard 16 was checked and met. The home has a complaints procedure in place, some service users were aware of this procedure. The manager is currently investigating one complaint, using the homes complaint procedure. Environment (Standards 19-26) Standard 19 was checked and met. The corridors have recently been redecorated with further decoration to be arranged. The home was found to be clean and tidy. Staffing (Standards 27-30) Standards 27 & 30 were checked and met. The home is adequately staffed with experienced and competent staff. The home has a trained nurse on duty over twenty four hours. The manager is also a trained nurse. Bondcare have their own Training Co-ordinator and NVQ and mandatory training has continued. Management and Administration (Standards 31-38) Standard 31 & 36 were checked and met. The manager has introduced supervision sessions with staff. The home has adequate health and safety procedures in place. There were no requirements from this inspection. The home has a statement of purpose and service users guide available for prospective service users. The new manager is compiling a large print and an audio tape of both these documents, and should be commended.Larchwood Nursing & Residential HomePage 11 Standard 2 (2.1 2.2) Each service user is provided with a statement of terms and conditions at the point of moving into the home (or contract if purchasing their care privately). 3 Key findings/Evidence Standard met? Service users receive a copy of the terms and conditions of residence on admission to the home. The contract now includes the room number that is to be occupied by the service user. A sample of the terms and conditions were seen and found to be satisfactory.Standard 3 (3.1 3.5) New service users are admitted only on the basis of a full assessment undertaken by people trained to do so, and to which the prospective service user, his/her representatives (if any) and relevant professionals have been party. 3 Key findings/Evidence Standard met? All prospective service users are visited either in their own home or hospital by the manager to undergo a general assessment to ensure the home can meet their needs. Assessment details are included within the care records after admission. Assessment documents from the placing agencies are also included within the care records and found to be detailed and informative. Standard 4 (4.1 - 4.4) The registered person is able to demonstrate the homes capacity to meet the assessed needs (including specialist needs) of individuals admitted to the home. 3 Key findings/Evidence Standard met? The new manager has initiated some changes within the home. A trained nurse is on duty over the twenty four period and new training has been recently introduced to ensure the nurses continue to develop their professional development. One care staff has developed her role to undertake and arrange social activities. The manager discussed further plans for future development with care staff skills. Standard 5 (5.1 5.3) The registered person ensures that prospective service users are invited to visit the home and to move in on a trial basis, before they and / or their representatives make a decision to stay; unplanned admissions are avoided where possible. 3 Key findings/Evidence Standard met? Service users are invited to visit the home prior to admission. If this is not possible, relatives are invited in their place. Some service users who spoke to the Inspector stated that they had visited the home and stayed for a meal or to take part in the days activities. Service users make decisions about their lives with assistance as needed. Service users, where appropriate, are responsible for their own medication, and are protected by the homes policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect.Standard 7 (7.1 7.6) A service user plan of care generated from a comprehensive assessment (see Standard 3) is drawn up with each service user and provides the basis for the care to be delivered. 3 Key findings/Evidence Standard met? Service users have a written care plan and this is drawn up with either the trained nurse if they have nursing needs, or the key worker if they have residential needs. Each care plan contains health, personal and social needs and short term goals. Risk assessments to reduce falls, washing and dressing, nutritional and pressure sore reduction are in place to allow staff to implement a safer lifestyle. The care plans contained daily records and were reviewed on a monthly basis A sample of care plans were seen and found to be satisfactory. Examining care plans indicated that the health needs of service users continued to be monitored.Larchwood Nursing & Residential HomePage 14 Standard 9 (9.1 9.11) The registered person ensures that there is a policy and staff adhere to the procedures for the receipt, recording, storage, handling administration and disposal of medicines, and service users are able to take responsibility for their own medication if they wish, within a risk management framework. 3 Key findings/Evidence Standard Met? There is no change to the medication. The trained nurses administers all medication to service users. There were no service users who self medicate during the inspection, although a locked facility is provided in each bedroom to allow this. A sample of the MAR sheets were found to be satisfactory. Medication is stored securely. The Controlled Drugs were not checked on this occasion. The Social Co-ordinator organises activities during the day, taking into account a variety of service users interests, hobbies and disabilities. During the inspection a quiz had been organised with several service user and relatives taking part. Service users commented that they enjoy taking part with some of the organised activities. The care plans record individual hobbies and social activities. Some service users continue with outside interests, and go out with relatives or staff when the weather improves. The meals are included in the service users guide. The menu is displayed in the dining room and annexe. Service users have a choice from the menu. The cook caters for a variety of special diets, including soft diets and any service users who require assistance with their meals are helped in a discreet manner. The menu has been adapted since the previous inspection to cater for the more frail service users. Snacks and drinks are offered at regular intervals throughout the day. The home is clean, pleasant and hygienic.Standard 19 (19.1 19.6) The location and layout of the home is suitable for its stated purpose; it is accessible, safe and well maintained; meets service users individual and collective needs in a comfortable and homely way and has been designed with reference to relevant guidance. 3 Key findings/Evidence Standard met? The home was found to be clean and fairly tidy when the Inspector visited. The corridors highlighted in the previous report have recently been redecorated. The manager has a plan in place for further redecoration and refurbishment for the next six months. The many communal lounges and dining rooms were in use during the visit, although some service users preferred to stay in their rooms. The lounges remain unchanged since the previous inspection. The gardens were tidy, with one service user enjoying the outdoor patio area. Standard 20.
