Conditions of registration: None Date of last inspection 2 March 2005 Brief Description of the Service: The home can provide personal care for 6 people who have learning and physical disabilities. The home cannot provide nursing care.The home is situated in Whickham and is a large detached bungalow divided into two units all with single bedrooms. It is set within its own extensive grounds with car parking facilities to the front of the house. It is on a bus route which makes it easy for relatives and friends to visit and the home has its own transport. There is a variety of community facilities reasonably close by including churches, doctors surgeries and a large shopping area in the centre of Whickham.The bungalow is purpose built with all of the necessary facilities, The layout and design of the building provides easy access throughout and bathrooms and toilets are suitably adapted for physically disabled people. This is an overview of what the inspector found during the inspection. The inspection took place on the 17 August 2005 by one inspector (Eileen Hulse), the date and time of the inspection was announced to people who use the service including the two Managers and staff and was carried out as part of the annual inspection programme. Prior to the inspection, questionnaires were sent out to all of the service users and their families. 6 were returned from service users that had been completed with the help and support of staff, some service users have complex needs and are not able to verbally express how they feel about the service. 5 completed questionnaires were received from relatives. There were no relatives visiting at the time of the inspection but completed questionnaires from families appeared they are satisfied with the service provided to their relatives in general. However, two relative’s made comments where they felt the service could be improved upon. Relatives comments included: • • • • • • ‘I feel a separate lounge for privacy would be nice’ ‘I have not been updated lately about my relatives care’ ‘Often relief staff therefore no consistency’ ‘I am satisfied with the care although I was concerned about the mass exit of regular staff’ ‘My relative is happy living at Lavender Rd’ ‘Two senior workers are very committed and have really good plans to keep improving life for those who live here’ The inspection took 5hrs to complete that included 1hr to prepare for the inspection. The focus of the inspection was to gain insight into the quality of life and services received by service users who live in the home. Time was spent chatting with service users, observing the practice of staff throughout the inspection and talking with the Manager and staff who were on duty. Some records were inspected including service user assessments, care plans, POVA records and staff training records. A tour was made of the communal areas of the premises accompanied by a staff member. What the service does well:
Service users were seen to be relaxed and comfortable with members of the staff team and observation throughout the day evidenced that the needs and choices of the service users were carried out at all times. The staff know the service users well and this helps when service users are making decisions and choices on a day to day basis. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 Good detailed assessments are in place for all service users that form the basis of their care plan to ensure that all care needs are met on a day to day basis. EVIDENCE: Records evidenced that all service users that currently live in the home have well maintained and up to date assessments that have been completed by Care Managers. Six monthly reviews are held to discuss in detail if any changes to the package of care are required, the issues are then recorded and implemented within the plan of care. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6 and 7 The care plans contain adequate information to guide the practice of staff that helps to ensure that all service users needs are identified and met at all times. Good communication methods are used between the service users and staff that help service users to make decisions that affect their lives. EVIDENCE: Each service user has an individual plan of care. It includes elements of care that service users can expect to receive and necessary graphics are used to help the service user understand what is being written. Behaviour management plans are in place and these have been devised in conjunction with the staff team and professional input from the clinical psychologist. Service users in Lavender Road have very complex needs, however, the skills and experience of the staff team enable them to communicate well with service users and this is evident within the individual plans of care and through observation during the inspection. Service users each have a key worker and progress reviews take place every six months to discuss any changes taking place or if a change in goals to be achieved is highlighted. The records show that the service user, care manager, key worker, relatives or friends and professionals are involved in this process. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 15 Service users are well integrated within the local community and use many of the local and leisure facilities to ensure they are involved in appropriate facilities of their choice. EVIDENCE: Records and discussions with the staff on duty evidenced that service users lead full and active lives. Service users visit the bowling club, library and leisure centre and the staff access up to date information from the library so that service users have a choice of what they choose to do on their days off from the day centres. The home is on mailing lists for the Theatre and cinemas and is situated close to the village community centre where service users take part in bingo sessions and service users are invited to any events taking place there. The home also holds a coffee morning every three months and the neighbours are regular guests of the service users at this event. The home have an open door policy on visiting and relatives and friends are made welcome and encouraged to visit people at home. Friends and families are invited to all social occasions held within the home. People are supported to maintain contact with their family and friends and this is outlined in the home’s Residents’ Charter. Some people regularly go out to see their relatives

The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 and 19 The staff team have been trained in the use of the specialist equipment used within the home, this protects service users by ensuring safe working practice at all times. EVIDENCE: All moving and handling techniques are well detailed in the service users individual plans of care with records well maintained and up to date risk assessments in place. All service users are able to choose what they want to do, as an example, this includes choosing their preferred times of going to bed and how they choose to spend their leisure time. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 23 Appropriate systems are in place to protect vulnerable people against abuse and staff have received training in this area so they are able to identify any signs of abuse and therefore protecting the service users. EVIDENCE: Some of the staff team have received Protection of Vulnerable Adults Training and arrangements are in place for the remaining staff team to access this training. Up to this time no incidents or suspected incidents of abuse have taken place. In discussions with staff they stated they felt confident they could recognise or suspect if an abuse situation was taking place and would know what to do about it. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24 and 30 The home is clean, nicely furnished and generally well maintained. However, the two lounges are in need of re-decoration to ensure that a high standard is maintained and that services users live in a comfortable environment. All areas of the home were found to be very clean with no odours in the home. EVIDENCE: Plans are in place to have two bedrooms redecorated in the coming weeks and service users have been involved in choosing the colour schemes for their individual bedrooms. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32 United Response produce job descriptions for all staff employed by the organisation. Staff appear to understand their roles and responsibilities and this helps to ensure the needs of service users are met at all times. EVIDENCE: Staff are given information to enable them to understand their own and other roles and responsibilities of the staff team. Policies and procedures are discussed within team meetings and codes of practice produced by the General Social Care Council have been distributed to all the staff in the home. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37, 39 and 42 The home must have Managers in place that have been registered with the Commission for Social Care Inspection. This requirement has been outstanding for some time and must now be addressed urgently. The home have no formal quality assurance system in place and therefore they cannot measure the quality of the service provided by the home. However, Staff have received mandatory training and a number of short courses to ensure the health, safety and welfare of service users, and everyone coming into the home. EVIDENCE: The home currently has two prospective Managers that are not yet registered with CSCI. However recent applications have been submitted and interviews will take place when all the necessary checks have been carried out. Both Managers have attained NVQ Levels 2/3 and are currently completing the NVQ Level 4 in care and the Registered Managers Award. Both Managers have
