Conditions of registration: 1. Residents to be 55 years and over Date of last inspection 21st July 2005 Brief Description of the Service: Lawrence House is a Kent County Council run Home providing care, rehabilitation and step down care for up to thirty people. There are sixteen places for those with failing mental capacity. The Home was purpose built in the 1980s and is a single story building set in pleasant and safe gardens. The accommodation is divided into homely living units. It is well staffed and the recuperative care unit has the benefit of on-site occupational therapy services. Lawrence House is situated in the residential area of Shorncliffe. Cheriton High Street is close by and provides amenities such as post office and shops. This is an overview of what the inspector found during the inspection. This unannounced inspection was carried out over approximately 6 hours. The Inspector spoke with the manager, (whose registration application has recently been submitted), 3 staff and 4 of the current 25 residents. The Inspector toured the building and inspected a number of records, policies and procedures. What the service does well: What has improved since the last inspection? The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6 Prospective residents, and/or their representatives, are provided with the information they need in order to make a decision on whether the home will suit them. The health care, emotional and social needs of prospective residents are assessed prior to their admission to the home. EVIDENCE: The individual records which are kept for each resident included a comprehensive pre-admission assessment with input from various health care professionals. There are now occasions when the assessment is carried out by the local Kent County Council care management team rather than by the manager or a member of staff employed within the home. However, the inspector was satisfied that there is appropriate liaison between care managers and the home and that the home would not admit someone if it was felt that their needs could not be met or that their needs would compromise the care and safety of other residents. Residents said that they had been given information regarding the home and that the home lived up to their expectations. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10 The health and social needs of the residents are well provided for by the home. Personal support is offered in a way that protects the privacy and dignity of the residents and promotes their independence. There are clear and comprehensive systems for the management and administration of medicines and the home promotes good health EVIDENCE: There are care plans in place for all residents which include reference to physical, personal and social care needs, individual aims and objectives, and evidence of reviewing residents’ needs. Risk assessments have been prepared and staff referred to specific guidelines which have been prepared for use in certain situations. The medication administration procedures have been reviewed since the last inspection and medication was seen to be appropriately recorded and stored. All of the residents and relatives spoken to told the inspector that the staff are helpful and sensitive and, whilst able to joke with residents, still treat residents with respect. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and 15 The home promotes the well-being, independence and autonomy of the residents and helps them to maintain control over their lives. Residents have access to a range of recreational interests. Residents are assisted with maintaining contact with their previous way of life, and their families and friends are made welcome. Residents’ tastes, choices and dietary needs are well catered for, with a varied selection of food and drinks available during the day and evening. EVIDENCE: All of the residents spoken to said that they are frequently encouraged to say what they would like, in terms of personal care, activities, menus etc., and that any suggestions are acted upon. Residents also confirmed that visitors are always made welcome and offered hospitality. The meal served during the inspection was seen to be appetising and nutritious and residents confirmed that the food is generally of a very good standard. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 The home’s complaints process is satisfactory and there is evidence to show that residents’, or others’ concerns are taken seriously and responded to appropriately. Staff have a good understanding of adult abuse issues and of the home’s procedures to protect residents from abuse. EVIDENCE: The service user guide includes the home’s complaints procedure. Residents confirmed that they would feel comfortable in mentioning anything to the Managers or staff and confident that any concerns would be resolved. The Manager and other staff referred to procedures and ongoing training in adult protection and showed a willingness to consult with local authority care managers and adult protection co-ordinators. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Although there are no serious safety issues, and the home is clean, pleasant and hygienic, the standard of internal maintenance in some areas is affecting the standard of the indoor environment. Residents have access to specialist equipment and their own bedrooms suit their needs. EVIDENCE: The residents’ bedrooms were generally of a reasonable standard and some have had new carpets and furniture. However there are some areas around the building, such as the “blue” lounge which are in need of attention. The corridors have been decorated in bright cheerful colours but again this is spoilt by the marked or damaged wallpaper, door frames and woodwork. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28, and 30 Staffing, in terms of both numbers and competency, is appropriate to the current needs of the residents. Staff are well trained and supported, and staff morale is high EVIDENCE: Staffing at the time of the inspection comprised the manager, team leader, 7 care staff and kitchen and domestic staff. The manager and staff confirmed that there is a good level of induction and further training and that all necessary checks on staff are undertaken prior to employment. All of the residents spoken to confirmed that day staff and night staff are kind, sensitive, helpful and cheerful. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): ,33,35 and 38 Residents benefit from a well managed home where there are appropriate procedures in place to safeguard their general well-being. There are sound and effective quality assurance systems in place. Generally, there is a high level of attention paid to health and safety issues EVIDENCE: Residents confirmed that they are consulted and kept informed of any developments affecting themselves or the home in general. Records are generally well maintained, and systems and procedures are regularly reviewed. The staff said that health and safety is regarded as a serious issue, that it is covered during their induction training and there is on-going training for staff in core health and safety issues.
