What the care home does well

The majority of records, documents, policies and procedures that were required during the inspection were accessible, accurate and up to date. It was indicated that the home was organised and well run, and that the health, safety and welfare of the service users were being safeguarded. Overall, the home was found to be clean, hygienic and well maintained. The atmosphere was calm, pleasant and homely.

What has improved since the last inspection?

All nine requirements that were made at the last inspection had been met. These related to environmental issues, records, staff training, quality assurance summaries and fire safety checks.

Two have profound disabilities. Although the home is not purpose built it has all the aids and adaptations the service users need. It is on two floors and there is a lift. All the bedrooms are single rooms and they are an adequate size for wheel chair users. The home is owned by Ealing Family Housing Association and managed by Life Opportunities Trust. It is on a housing estate and is a long walk to shops and other local amenities. There is a minibus for taking service users out of the home. There is a Sensory Room for the provision of stimulation to the service users. Activities are provided in house and outings are arranged. Two service users are taken to a weekly club for people with learning disabilities. The service users socialise with service users who live in homes managed by Life Opportunities Trust nearby. There were no vacancies on the day of inspection. This unannounced inspection was carried out between 12:00am and 3:45Pm on 14th December 2006. A total of three care support workers were at the home during separate morning/afternoon and afternoon/evening shifts at the time of the inspection. It was reported by a care support worker that the Registered Manager was receiving training at a separate venue. Three service users were attending the day centre. One service user was being supported in an activity within the community. During the course of the inspection, the home’s records, policies, procedures and documents were examined and observations were made. Three care support workers were spoken with and a telephone discussion was held with the Service Manager. Care support workers were co-operative and provided appropriate assistance throughout the inspection. What the service does well: What has improved since the last inspection? Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. JUDGEMENT – we looked at outcomes for the following standard(s): 2. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The needs of prospective service users are appropriately assessed prior to admission. EVIDENCE: Assessments that had been submitted to the home by placing Authorities in relation to prospective service users were within service users’ files that were inspected at random. It was indicated also, that relatives, carers, social workers and medical professionals were involved in assessing the suitability of the home in meeting separate identified needs and aspirations. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7 and 9. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Care plans and risk assessments are being appropriately drawn up and service users are assisted with making decisions, where possible. EVIDENCE: The care plans of the service users that were viewed at random were found to be satisfactory. Individual personal, social and health care needs had been identified and appropriate action plans and set goals were put into place. Care plans were reviewed on a monthly basis. Risk assessments relevant to specific activities identified within care plans had been carried out. These included use of cot-sides, choking and pushing wheelchairs. Risk assessments were reviewed annually or following accidents or incidents. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15, 16 and 17. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The service users participate in appropriate activities and contact with relatives and/or friends are being maintained. Varied and wholesome meals are provided to the service users. EVIDENCE: It was reflected on records viewed that service users were supported while participating in various shared and individual activities within the community. These included meals out, shopping, day trips, swimming and annual holidays. Service users also attended Religious services, evening clubs and day centres. An open visiting policy is operated at the home and contact with relatives and/or friends are encouraged and facilitated. The records indicated that service users regularly visited relatives at weekends. The Inspector was informed by a care support worker that the service users were unable to undertake housekeeping tasks due to profound physical difficulties. The menus were reflective of varied and nutritional meals being provided to the service users. A care support worker confirmed that two service users required feeding. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 and 20. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The personal and health care needs of the service users are being met satisfactorily. The home’s policy and procedures on medication are appropriately detailed. EVIDENCE: It was confirmed by a care support worker that the service users were fully reliant on support and assistance with their personal care routines. However, these tasks were carried out by same gender care support workers and in privacy within bedrooms or bathrooms. Separate health care needs were identified within care plans. It was indicated that service users were accompanied during medical appointments and received access to GPs, Physiotherapists, Occupational Therapists, District Lowdell Close, 186-188 DS0000027064.V321627.R01.S.doc Version 5.2 Page 14 Nurses and Speech and Language Therapists. Regular dental and optical checks were arranged. The home’s medication policy was in place and the records were reflective of medication training being delivered to the care support workers. The storage, disposal and administration of medicines were satisfactory and medication reviews were carried out on an annual basis. It was reported by a care support worker that the service users lacked the capacity to self-administer their own medication. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 22 and 23. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The complaints procedure is satisfactory and the service users are being protected from abuse. EVIDENCE: The complaints procedure was clearly written, concise and accessible to the service users and their relatives. No complaints had been made to the home following the last inspection. The home’s adult protection procedures and policies and procedures on the protection of vulnerable adults were in place. The London Borough of Hillingdon Multi-Agency policies and procedures on the protection of vulnerable adults were also available. The records were indicative of training on the protection of vulnerable adults being delivered to the care support workers. Personal allowances are safeguarded at the home. Financial records relating to individual service users were viewed and were satisfactory. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24 and 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is safe, pleasant and well maintained. Two requirements under Standard 24 relating to environmental issues have been met. EVIDENCE: The communal areas at the home were adequately spacious, comfortably furnished and suitable for shared or individual activity. A maintenance plan dated 2006/7 was in place. The hallways, kitchen and staff toilet had been redecorated and there new flooring had been laid in the laundry and boiler room. Overall, the home was found to be clean, hygienic and well maintained. The atmosphere was calm, pleasant and homely. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34 and 35. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Care support workers are suitably trained and qualified for meeting the needs of the service users. The policies and procedures relating recruitment are satisfactory. Requirements under Standards 34 and 35 relating to staff records and staff training have been met. EVIDENCE: Individual training programmes were in place and it was indicated that new staff members received induction training. Lowdell Close, 186-188 DS0000027064.V321627.R01.S.doc Version 5.2 Page 19 It was evidenced on Training Certificates that staff training delivered during 2006 included Protection of Vulnerable Adults, Managing Diversity, Administering and Safe Handling of Medication, Moving and Handling and Fire Safety. The Registered Manager was not at the home during the inspection and recruitment files were not accessible. However, a telephone discussion was held with the Service Manager. She confirmed that recruitment files were held at the home and contained all required documents such as CRB disclosure certificates, application forms, references, photo-identification and signed contracts/statement of terms and conditions. It was reflected on training records that four care support workers had achieved level 2 National Vocational Qualification in care. Three care support workers expressed satisfaction with the level of training they received at the home. This judgement has been made using available evidence including a visit to this service. The Registered Manager is appropriately qualified. Quality assurance exercises have been carried out satisfactorily. The safety and welfare of the service users are being safeguarded. Requirements under Standards 39 and 42 relating to quality assurance, fire risk assessments and fire drills have been met. EVIDENCE: Lowdell Close, 186-188 DS0000027064.V321627.R01.S.doc Version 5.2 Page 21 It was reflected on Training Certificates that the Registered Manager had successfully completed training in Managing Diverse Teams and Management Leadership. It was also indicated that he was in the process of obtaining Level 4 National Vocational Qualification in care management. Care support workers reported that the Registered Manager was open, approachable, helpful and supportive. The Service Manager confirmed that self-monitoring of the home had been carried out and that appropriate summaries were completed. Surveys that had been completed by relatives and stakeholders were evidenced on documents viewed. It was indicated on records viewed that up to date health and safety checks had been undertaken. These included professional testing of gas safety, portable appliance, legionella and fire alarms. Weekly fire safety and water temperature checks, monthly fire drills and clearly detailed fire risk assessments. Environmental risk assessments including control of substances hazardous to health, use of tail lift/clamping in minibus and transfers using the hoist, had been undertaken.
