This inspection was carried out on 30th November 2009.

It is an annual review prepared by CQC after examining previous reports and information from the provider. At the time of this report, CQC judged the service to be Good.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

We give a quality rating following a full review of the service. Number of places (if applicable): Under 65 Over 65 34 0 0 34 Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service Annual Service Review Page 2 of 6 Malvern House is a privately owned care home offering residential accommodation and support to 34 elderly people, including those with dementia. There are 2 double bedrooms and 30 singles. All have wash hand basins and several have ensuite toilets. There are two lifts and access is suitable for those with limited mobility. There are three bathrooms, with assisted baths. There is a conservatory, patio and attractive garden planted to lawn and flowers. The home is situated on a main road in a residential area of Chingford (in the London Borough of Waltham Forest) with access to local shops and transport links. The current fees range from £460- £612 per week. Information about the service is available at the front reception area of the home, including inspection reports, to people living in the home and to other stakeholders. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection. This included: The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Information we have about how the service has managed any complaints. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection report. What has this told us about the service? At the last key inspection on 26/11/2007 the service was assessed as being a good service and this was still our judgement at the last annual service review that took place on 04/09/2008. In March 2009 the directors of the provider organisation changed and a new responsible individual and manager of the service were appointed. The new the manager is in the process of registering with the Commission. Since March 2009 there has been one complaint about the service that was made to the Commission, which was properly investigated by the home using its complaints procedure. There have also been three safeguarding adults referrals since March 2009, all of which were properly investigated under the L.B. of Waltham Forests safeguarding adults procedure. Two of these allegations were judged to have been substantiated following the investigation by the local authority and improvements, particularly with regard to recording, have been agreed with the home. The service sent us their annual quality assurance assessment (AQAA) when we asked for it. The AQAA contained a range of detailed information and indicated that the home is working hard to develop the service. The AQAA told us of the improvements the service had made in the previous twelve months. These have included: improved work on reminiscence using music; introduction of improved recording for regarding contact with a GP; introduction of coffee mornings to help residents express their opinions about the home; new furniture and equipment for the garden; staff training, including in food hygiene, safe administration of medication and Parkinson disease and introducing six monthly reviews of policies and procedures. The AQAA told us that planned improvements to the home over the next twelve months would include: redecorating the home; continuing to encourage comments and suggestions from residents regarding social activities; complaints, staffing and the management and administration of the home. Annual Service Review Page 4 of 6 The home continues to let us know about things that have happened since our last key inspection and they have shown that they are working hard to managed the issues properly. The home continues to work well with the Commission, however, given the changes in management arrangements at the home and the recent safeguarding investigations we have decided to bring the next key inspection forward. What are we going to do as a result of this annual service review? We are going to amend our inspection plan, and will carry out a key inspection by 31st May 2010. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
