The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home continues to co-ordinate well the four distinct service areas; the quality of care is extremely high and outcomes for service users are good. The service has been consistent in ensuring service users needs are well met and this high standard of care has continued.

What has improved since the last inspection?

As noted previously the home provides a very high standard of care to service users; this level of care has been successfully maintained. The inspection findings evidence that all outstanding requirements had been met, so those areas identified as being problematic at the previous inspection, namely evidencing service user contracts, appropriate recording of medication information and the appropriate vetting of staff have now been resolved.

What the care home could do better:

As mentioned previously, the home offers service users a high quality of care which has been maintained over a significant period of time. All outstanding requirements made at the previous inspection have been successfully resolved. The service is to be commended for achieving and maintaining a service which consistently has excellent outcomes for service users.

The home offers support, personal care and accommodation for a maximum of 37 service users. It offers four distinct services; long term residential care, intermediate care, respite care and services to those recently discharged from hospital and assessed as having no immediate need for hospital care (Delayed Discharge Programme). The home is in the process of reducing its long term residential care provision in order to offer more beds to service users admitted via the delayed discharge policy. It is the service’s long term plan to eventualy phase out long term provision and offer services to slightly younge elders than the registration category currently permits. A variation to the home’s current services will need to be applied for. The home is situated in Clapton within the London Borough of Hackney, in very close proximity to the local general hospital. Bus links are good and local shops and amenities are located nearby. The unannounced inspection of 25 Median Road took place on the morning of March 6 2006. The sole purpose of the inspection was to review the home’s progress in addressing outstanding requirements (3) made at the previous inspection that was conducted in July 2005. The inspection process included discussion wuth the registered manager and other staff and the review of five service users files. The inspector would like to thank all staff and service users who contributed and co-operated with the inspection. It was noted at this inspection that all outstanding requirements had been addressed satisfactorily and no additional requirements or recommendations were made. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. EVIDENCE: The previous inspection had highlighted the needs for service user files to evidence written agreements/contracts. The inspector reviewed five service user files, all of which evidenced written agreements. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 Service users, where appropriate, are responsible for their own medication and are protected by the home’s policies and procedures for dealing with medication. EVIDENCE: The previous inspection had highlighted the need for staff to pay greater attention to the recording of information on MAR cheets. On this occasion, the inspector reviewed the medication information in detail for five service users. Information seen was clear and comprehensive, there was evidence of staff members countersigning records in every instance. Key codes on MAR sheets were apropriately used and where changes to medication regimes had been recommended, documentation and instructions were clear. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. JUDGEMENT – we looked at outcomes for the following standard(s): 29 Service users’ needs are supported and protected by the home’s recruitment policy and practices. EVIDENCE: The previous inspection had included the review of a number of staff files, some of which failed to evidence key information such as proof of identity. The registered manager commented that since the last inspection the department’s Human Resources team had conducted a review of all staff files and requested staff supply all required documentation. Staff personnel files are maintained off site at the service’s head office in Hackney. The registered manager confirmed that all staff had provided information as required and that she satisfied that staff files contained full information as required.
