What the care home does well

The Home provides a spacious, homely, well-decorated and well furnished environment. The Home provides toiletries and a free chiropody service for the residents. The Home liaises well with local healthcare professionals and ensures their advice is carried out in the Home.

What has improved since the last inspection?

More bedrooms have been re-decorated and fitted with new carpets. The home has improved the way it looks after the health of the residents. There is now a shady, paved area in the garden with tables and chairs. Some of the residents have enjoyed sitting here during the summer. The home has improved the way in which it responds to complaints

What the care home could do better:

The manager should check to ensure only those residents who want to get up early, do so.

Service users under 65 years of age to be restricted to one (1) whose DOB is 22/08/1946. 24th May 2005 Date of last inspection Brief Description of the Service: Mont Calm Residential Home, Lydd, formerly known as The Manor House, is a care home, registered for twenty-two older people with failing mental capacity. It is situated in the small coastal town of Lydd. There are some local shops and a church nearby. There is easy access, by road, to the larger towns of New Romney, Hythe and Folkestone. The registered provider for the Home is Mr Steven Castellani. Mrs Susan Page is the manager for the Home and her application for registration with the CSCI is currently being processed. This inspection began at 1.30pm and lasted for three hours. The inspector spoke to the, the manager, four residents, one relative and two members of staff during the course of the inspection. A tour of the Home was undertaken and direct and indirect observations were made throughout. Key documentation, including a sample of care plans, was examined in detail. The Home continues to make progress in achieving the National Minimum Standards and to improve the environment. All the residents appeared well cared for on the day of inspection. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 10 & 11 The home threats the residents with respect and values their dignity. It has improved the way it cares for their health and manages end of life issues very well. EVIDENCE: A sample of care plans was examined. This confirmed that all appropriate healthcare professionals have been consulted and their advice implemented. All the residents were in good health, appeared well cared for and were well dressed on the day of inspection. A relative said that he is happy with the care the home gives and that they always make him welcome when he visits. Mrs Page, the manager, said that the home aims to care for residents at the end of their lives providing they can meet their needs. Staff were observed to treat the residents with kindness throughout the time of inspection. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13 & 14 The Home works to provide a lifestyle in the Home that meets the expectations of the residents and their supporters. The home helps the residents maintain contact with their friends and family. The home has improved the way in which it supports choice, however, some work is still required in this area. EVIDENCE: An activities co-ordinator visits the Home three times a week. Life histories of the residents are recorded in the care plans. A relative said that he is always made welcome and that he can visit at any time. Mrs Page talked about the way the home recently gave support to a familiy of a terminally ill resident. It was clear that every effort was made to help the family through a very difficult time. Some staff said that they think that some residents are still getting up too early. This means that they are tired and often fall asleep in their chairs during the day. They said that these residents sometimes look uncomfortable. The home should also consider if it would be appropriate for some residents to rest on their beds, rather than sleeping in chairs, if they look uncomfortable. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 The complaints system is satisfactory and the home has improved the way it responds to complaints. There are sound policies and procedures in place in respect of the Prevention of Abuse. EVIDENCE: The CSCI received one complaint since the last inspection. This was partially substantiated. The home acted in a timely way to rectify the concerns and learned valuable lessons from the handling of this complaint. Staff said that they are aware of the need to protect the residents from abuse. They would not hesitate to report concerns if they arose. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 23, 24, 25 & 26 The environment of the home is god, providing the residents with a comfortable and homely place in which to live. EVIDENCE: Since the last inspection further re-decoration has taken place. Several bedrooms were inspected. All were well decorated and tastefully furnished. Work in the garden is nearly complete. There is now a shady patio area with garden furniture. Some of the residents have been able to enjoy this area during the summer months. The area is soon to be planted and a sensory garden with scented plants is planned. A tour of the Home confirmed that all areas were clean and hygienic. There were no offensive odours noted. No health and safety hazards were noted on the day of inspection. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 29 & 30 Staff demonstrate clear understanding of their roles and responsibilities and there is a good level of training. Staff morale is mostly good but some do not always feel valued by the registered provider. Recruitment policies and procedures in the Home are satisfactory. EVIDENCE: Organisational structures for the Home define clear roles and responsibilities and staff are aware of these. Generally staff morale is good, however, some said that they do not feel valued. Recruitment policies and procedures are sound. A sample of staff files was examined. This confirmed that the appropriate checks have been carried out. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 38 The home is well maintained. This ensures that the residents have a safe place in which to live. EVIDENCE: There are sound health and safety policies and procedures. Staff are aware of these and their responsibilities in respect of health and safety. Since the last inspection glass has been removed from the door to the rear staircase. An improved arrangement to allow access to the key in case of fire, has been put in place. No health and safety hazards were noted on the day of inspection. Documentation inspected was found to be in order and up-to-date.
