This inspection was carried out on 5th September 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Those who live in the home felt that they were treated with respect and their right to privacy supported. People living at the home said that they were consulted about the quality of the service provided. Those who live at the home said that they had a range of activities to choose from. Relatives said that they were always made welcome and given information on the needs of those who live at the home.

What has improved since the last inspection?

Eight areas for improvement were identified at the last inspection. It was found at this inspection that these had all been addressed. Information on the needs of people living at the home had been updated so that it covers all the areas where they needed support. The safety of those who live at the home has been enhanced by ensuring that all fire doors can be closed in the event of a fire. The staffing level had been reviewed and staff re-allocated to ensure a better skill mix to meet the needs of those living at the home. Staff were supported to meet the needs of those living at the home.

What the care home could do better:

This inspection identified two areas where the home needs to make improvements. There need to be measures to ensure that night staff respond sensitively and appropriately to people living at the home. The fire safety risk assessment needs to be updated to ensure that all fire risks are identified.

The home must advise the registering authority at such times as the specified service user attains 65 years of age or vacates the home. Date of last inspection 28 April 2005 Brief Description of the Service: Nairn House is owned by BUPA. Nairn House is a large purpose-built care home providing accommodation for 61 older people who require residential and nursing care. The service is provided on three floors and is serviced by a passenger lift. Each service user has a single bedroom with en-suite facilities. An office, nurse station, lounge and dining room are situated on each floor, in addition to toilets and bathrooms. The home is located close to shops and with access to public transport. This unannounced inspection was undertaken as part of the annual inspection process. The inspector also sought to confirm that the eight areas for improvement found at the last inspection were addressed. The inspection took place over one day. Sue Kane the manager assisted the inspector. The inspector spoke with five service users, two relatives and four staff. The inspector toured the building and examined a range of records relating to the care and management of the home. The standard of care continues to improve. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. JUDGEMENT – we looked at outcomes for standard(s) 4 Service users assessed needs are being met. EVIDENCE: Relatives and service users said that the staff understood their needs. Since the last inspection care plans have been updated to include more information on the needs of service users. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 8 10 Service users have sufficient information on how their needs are met. Service users medical needs are being met. Service users privacy is maintained. EVIDENCE: The manager explained that care plans are being updated. The inspector saw examples of these care plans, which contained more details of the needs of service users. Professional involvement in the management of pressure sores was recorded. There were detailed care plans on the treatment and prevention of pressure sores and continence promotion. Service users said that staff respect their privacy and always knock on their bedroom doors. Service users also said that staff took time to ask how they wished to be assisted. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 13 14 Service users are provided with sufficient and varied activities to meet their needs. Service users are supported to maintain contact with relatives and other representatives of their choice. Service users are not always able to make choices about how they live in the home. EVIDENCE: Service users said they had been provided with activities and entertainment. The activities organiser who has been recently appointed explained that she was organising a range of activities and also doing one to one work with service users. The inspector saw that an activities programme is in place and the interests of individual service users are recorded. The inspector saw staff interacting with service users and providing activities. Relatives spoken to said that visitors were always made welcome and kept informed of the needs of service users. Service users spoken to said that they were offered a choice of food and could choose to take part in activities run in the home. One service user told the inspector that night staff were unresponsive to her when assisting with personal care. This was discussed with the registered manager who agreed to take this issue up with the night staff concerned. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 Service users live in a safe environment. EVIDENCE: The inspector saw that all fire doors were either closed or held open with a self-closure device. All areas of the home were well maintained. Redecoration of the home is continuing. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 27 Sufficient staff are available at all times to meet service users needs. EVIDENCE: The registered manager explained that since the last inspection a review of the staffing level had been carried out. The staff have been organised so that there is a greater mix of skills. Service users spoken to felt that there were more staff available throughout the day. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 36 38 Service users are consulted about the quality of the service provided and encouraged to make suggestions for improvement. Staff are appropriately supervised to ensure that service users receive the care and support they need. The fire safety risk assessment needs to include how all fire risks in the home will be responded to. EVIDENCE: The inspector found that there were systems in place to ensure that service users and relatives are consulted about the quality of the service provided. Actions to improve the service are identified. The inspector spoke with staff who confirmed that they had supervision regularly. The inspector saw notes of supervision and found that these showed that supervision provided the support staff needed. The inspector found that the fire risk assessment did not contain detailed information on how fire risks were to be prevented.
