Conditions of registration: 1. Conditions of Registration 1. Younger adults will only be accommodated in the nursing unit and will only be admitted if they require palliative care and the placement is not considered to be long term. 2. Service users admitted under the category of terminal illness must be accommodated in the nursing unit. 3. Service users admitted under the category of physical disability (over 65) must be accommodated in the nursing unit or on the ground floor of the residential unit. 4. Service users will only be admitted into the first and second floors of the residential unit if they are able to descend the stairs in the event of a fire evacuation. When a service user looses this ability the Fire Risk Assessment must be reviewed and the Fire Safety Officer and the Commission informed about any interim arrangements, while the appropriateness of the placement is reviewed. Date of last inspection 15th August 2005 Brief Description of the Service: Newstead House is a Victorian property with a modern two-storey adjacent wing built in 1988. It is situated north of Hereford City approximately one mile from the town centre. It is owned and managed by Somerset Redstone Trust. In the original building the home offers accommodation for a maximum of 14 residents requiring support with personal care. The adjacent two-storey wing offers accommodation for a maximum of 33 residents requiring nursing care. The service is able to offer care for adults of both sexes who have a physical disability, are terminally ill or are frail due to old age. This is an overview of what the inspector found during the inspection. The inspection took place during the morning time was spent with the registered manager and her deputy, a number of residents and staff were spoken to during the course of the morning. This inspection focused on requirements from the previous inspection visit and inspecting the standards not inspected last time. What the service does well: What has improved since the last inspection?
The registered manager and her deputy continue to address the shortfalls highlighted at the last inspection; this has been made difficult by the continuing major building work ongoing in the home. During this period the registered manager and her staff have ensured residents care has not been compromised and they have been kept safe and warm. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 Residents who were self medicating were not doing so within a risk framework. EVIDENCE: Medication policies and procedures were inspected on both units. There were two residents who were self medicating they were doing so without an up to date risk assessment and any audit procedure to ensure they complied with their medication regime. Medication to be given as and when had set times in the medication chart. Medication procedures for the disposal of medication complied with new legislation regarding disposal of medication including controlled drugs. Residents self-medicating had lockable storage space in their bedrooms. Medication records seen did not have any signature gaps, this was commendable. A fluid chart seen during the inspection had not been updated for several hours the previous day, this lack of documentation for frail residents is unacceptable. The registered manager said she invites resident’s families to have input into care plans; to date residents and relatives have not taken up this offer; discussion around documenting this took place. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 & 15 Residents are enabled to live as full a life as possible within their physical and mental disabilities. EVIDENCE: The home has an open visiting policy and visitors were seen coming and going during the inspection. The home has a Holy Communion service once a month and children from the local primary school visit at Christmas to sing carols. Residents in the nursing unit are frail; those in the social care unit are more active and able to leave the building, for organised trips out. Lunch on the day of inspection was appetising and residents were offered choices for all meals. The kitchen was inspected and was clean; records for the previous day were requested. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25 & 26 A number of environmental risks, risk the health and safety of the residents in the home. EVIDENCE: The home has ongoing major building work in progress; this includes a new call bell and fire bell system, which is due to be commissioned this week. A number of areas in the home required refurbishment following the building work including re-decoration and new carpets. Concerns following a tour of the premises were based on unacceptable risk to residents these included unlocked cupboards containing hot water pipes, a number of exposed hot water pipes, a free standing heater which had not been risk assessed. A number of fire doors were wedged open by means of a stool and in one case a pack of toilet rolls. There were no lamp shades in toilets. There was an unlocked full-length window on the first floor with no window restrictors (the room was unoccupied and was locked following the request of the inspector).

Some bedrooms on the nursing wing appeared small if the resident required hoisting. Infection control policies were adhered to and in the past the home had had access to the infection control nurse and her expertise. Bedrooms were personalised, not all bedrooms had lockable storage space. The registered manager said residents were risk assessed for a key to their bedroom these records were not seen. Communal areas in some areas of the home would benefit from refurbishment following the building work and a number of carpets required cleaning or replacing. Residents who were in bed or in rooms were listening to appropriate television and radio programmes and looked comfortable. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 30 Staff have a full induction and ongoing training programme in place, which ensures they have the knowledge to care for residents. EVIDENCE: There is an induction programme and all staff attend training in moving and handling before moving any residents. A training matrix in place evidenced staff received continual updates in mandatory training. The home employs thirty-five care staff of these twenty have an NVQ qualification and two staff are currently training. The foundation programme is based on TOPPS and underpins NVQ training, the home was implementing a new pack for new staff which looked comprehensive. New staff had the benefit of a senior member of the care staff team to mentor them during this period. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): The registered manager is experienced and runs the home in a manner, which ensure residents are kept safe and secure. EVIDENCE: The registered manager has the necessary experience to run the home well. There is a committed staff team and the home uses agency staff to cover holidays and sickness. Staff receive regular supervision from other members of the team. Resident’s monies are kept safe and records are audited weekly. The home has a quality assurance programme in place this requires further development into a yearly report which should be made available to residents and prospective residents.

The home has a legionella risk assessment in place and recent changes in the supply of water have now decreased the risk of legionella.
