This inspection was carried out on 7th June 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home is exceptionally good at providing a homely environment and care that suits the needs of the residents. The home is also good at recognising when those needs cannot be met and does not admit anyone to the home whose needs cannot be met. The home responds well to all regulatory activities. The provider keeps up to date with current practices and legislation. Although the home is only registered for three service users the provider ensures that her own and that of the one member of staff`s professional development is pursued. The home is very well maintained, and the providers work hard to provide a safe environment. The home promotes a sense of community and encourages all forms of social interaction. Many activities and outings are offered throughout the year and the residents are enabled to make choices in relation to their every day activities.

What has improved since the last inspection?

A new system is now in place for the handling and administration of medication this has proved a big improvement.

What the care home could do better:

It is difficult to state what this home could do better, however the provider/manager thinks they should offer more activities. Future inspections will assess progress made.

It is situated in the middle of the historic village of North Elmham. The village is approximately five miles from Dereham, seven miles from Fakenham and sixteen miles from Norwich; which can be reached by a regular bus service. The Home is registered to offer care to three people in a small caring family. The Providers make every effort to ensure that the Home retains a homely atmosphere and the Service Users feel very much a part of a family. All rooms are individual and service users can negotiate to have some of their own furniture, there is a communal lounge with television. There are many local clubs including Friends and neighbours, Bowling Club, Horticulture Society and Extend. The Service Users are given many opportunities for trips out. Since the last inspection the conservatory has been enlarged, much to the delight of the residents. This was an announced inspection that took place over 2 hours. The provider of this exceptional service was present for the inspection and all three of the residents were spoken to during the inspection process. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,9 The health care needs of the residents are identified and met. The residents receive their medication safely. EVIDENCE: The three care plans were inspected and they indicated the care that was needed to meet the needs of the individuals, there was evidence of continuous reviews and evaluation of care. The Inspector noted a number letters from various agencies stating that the provider/manager communicated well with the local surgery and promotes the well-being and independence and quality of life for the residents in her care. Other letters described the provider’s commitment to caring for the residents and her pursuit to ensure that they received care that was far and beyond what was expected in a care home. The residents are also enabled to receive any resources that help them maintain their independence. It is a much safer system and the provider/manager and the part time carer have both had instruction in relation to this new system. They have also attended a study day on medication so that they have a better understanding of the process; certificates were seen for this. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 Social activities are managed extremely well, they are varied and creative. Residents are encouraged to make choices in relation to their daily lives. Links are maintained with the local community. EVIDENCE: The three residents discussed with the Inspector the variety of activities offered to them; they all felt that they very much suited their needs and that they were always given the choice of what they wished to do. One resident commented that they “pleased themselves what they do, and can go out as much as they wished”. A number of outings take place especially during the summer months; they had visited the Sea World at Hunstanton and also enjoyed a trip on a local North Norfolk train to name but a few of the outings. The provider ensures that she takes one resident to visit her friend once a week and collects the friend the next week to visit the home. The residents said they very much-enjoyed sitting in the newly extended conservatory and watching the birds and the two elderly tortoises who also live at the home. The residents expressed their satisfaction in all that the providers arrange for them. Two of the residents are local people; hence many of the visitors live locally and bring into the home all news of local events and happenings. Visiting is not restricted in any way and indeed is encouraged. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 The providers have ensured that the residents are protected against all forms of abuse. The home does not have a history of complaints, however the residents are confident that their concerns would be listened to and taken seriously. EVIDENCE: The residents were insistent that if they have any concerns they were always listened to. They felt that they could approach either of the providers, but could not visualise why they might have to. When asked if they had any concerns one resident commented that they were extremely well cared for and that he had only come for two weeks but had been there two years. A most comprehensive complaints procedure is in place, it is easy to understand; it was seen by the Inspector. The provider/manager has constantly kept herself up to date with all the relevant legislation and certificates were seen relating to study days that she and the carer had attended. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,26, The environment is extremely well maintained and safe. Areas inside and outside are easily accessed and safe, ramps have been installed so that the residents do not have to tackle steps. EVIDENCE: The providers have an on going plan for redecoration and as soon as something requires attention it is dealt with; this is obvious due to the general ambiance and state of all décor and furnishings. The home was found to be very clean and tidy and the residents shared with the Inspector that the home was just as they wished it to be. One resident stated that they were lucky to live in such pleasant surroundings. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 29, The residents are protected by the homes policies and procedures that are in place. EVIDENCE: This is a very small home that does not have to regularly employ new staff, only one very part time carer is in post who relieves the provider/manager when required. All this persons appropriate checks have been carried out and these were seen by the inspector. All policies and procedures seen by the Inspector were up to date and relevant to the home and its residents. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32,33,38 This is an extremely well managed home, that is totally run in the best interests of the residents. Practices are in place to ensure the safety of the residents. EVIDENCE: Anecdotal evidence gained by chatting with the residents led the inspector to be totally convinced that this home is run well and consideration always given to the needs of the residents. One resident stated that the providers were very good and nothing was too much trouble and that they could not live in a better place and clapped their hands. They went on to state that “the care was so marvellous they could not even start to describe how wonderful it was” A quality control survey was carried out to ascertain the observations of the residents and their relatives. The home obtains advice in relation to fire safety and instruction in the event of a fire. Risk assessments are in place, COSHH regulations are adhered to and a policy was seen for this. The home has achieved a Good Food Award and the certificate was seen.
