Persons of either sex, aged 65 years and over, who require care by reason of old age only (not to exceed 14 persons) One named person, over the age of 65 years, who requires care by reason of dementia The total number of service users to be accommodated in the home must not exceed 14 persons 30th August 2006 Date of last inspection Brief Description of the Service: Oaklands is a care home, registered for fourteen older people. The category of dementia, for one specific service user only, is included in the homes conditions of registration. The providers of the service have made an application to the Commission for Social Care Inspection (CSCI) to extend their registration so that they can accommodate more people with dementia. The current range of monthly fees, as detailed in an Inspection Questionnaire completed on 18th March 2006, are £340 - £358 per month. Additional charges were listed as £10 for chiropodist every six weeks, hairdressing £6.50 every six weeks and toiletries £6 a month. The home is in the town of Brightlingsea, Essex. The property is situated on the main road into the town and close to all amenities, the seafront and the beach. Mr & Mrs Cahoolessur are the Directors, and Mrs Cahoolessur is the Registered Manager. The home has fourteen single rooms, with the bedrooms located on the ground floor and the first floor. Access to the first floor is by stairs, a chair lift and a passenger lift. Communal areas are situated on the ground floor and consist of a large lounge and a dining area. The home also has a smaller lounge and a conservatory dining room. Both the kitchen and laundry areas are located on the ground floor in the central area of the property. There is driveway access to the property, with some parking areas and garden areas at the front of the home. Gardens to the rear of the home are large, mainly laid to lawn, with some garden seating. This is an overview of what the inspector found during the inspection. The inspection took over 4 hours. The manager was present throughout. A tour of the building took place 7 residents; one relative, three staff were spoken with as part of this process. The visit mainly focused on all Key standards and all of the requirements from the last inspection report. Random samples of records, policies and procedures were inspected. The home needs to ensure that all chemicals are kept in a locked cupboard according to the COSHH (Chemical or substances hazard to health) regulations. Also creams kept in residents rooms need to have lockable storage supplied to comply with medication regulation and guidelines. A Quality assurance system is in place, however this needs to be developed further by collating information; an action plan put together and this needs to be forwarded to the CSCI office. The home needs to evidence work carried out by appropriate means of recording things such as choices of activities tried given and maybe refused.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Prospective residents and their relatives are given a service user guide which informs them of the services provided and they also are encourage to visit Oaklands prior to any admission. This allows prospective residents and there families make an informed choice as to whether or not Oaklands will met their needs EVIDENCE: All care plans included initial assessments and were carried out prior to admission. There was evidence that individuals and their families had visited Oaklands and that a service user guide had been given. Service user guides have recently been reviewed. All care plans have recently been re-organised and gives a comprehensive break down of each residents needs including associated risks. The policies and procedures of the home provide evidence that it meets the requirements expected for the admission of any new residents. Oaklands does not offer intermediate care.

Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Care Plans reflect and highlight all residents’ individuals’ needs. The care plans include all aspects of an individuals needs. The review of clients care plans is evident. EVIDENCE: Observation during the inspection showed residents to be well cared for, clean and properly looked after. Residents and a relative spoken with felt that the quality of care was good. Residents can have BT lines installed in their own rooms if they wish, there is also the main office phone which can be transferred to individual rooms for privacy. Residents mail are kept for the individual to collect, if support is needed then the staff or relatives give this assistance. A Monitored medication dosage system is in place for each resident. Medication is stored in a lockable cabinet and the administration records were being maintained in accordance with agreed procedures. Record sheets had been correctly recorded and signed for.Oaklands has a controlled drugs cabinet and controlled drugs register which is compliant and issues raised at the last inspection regarding correct labelling and storage had been retified.Training records indicated that staff have had medication training and

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Activity plans are in place. Relatives and friends are encouraged to have regular contact with the home. A variety of regular nutritious meals are being provided. EVIDENCE: There is an activity co-ordinator and staff spoken with evidenced that choice of activities are offered. These included group and individual activities, inside and outside of the home. Residents spoken with informed me that they did have a choice. However activity choices and refusal by individuals needs to be recorded appropriately and evidence that staff try to encourage that individual are in place. Good history and likes dislikes of individuals are kept as an aid to staff working with groups and individuals. Residents and relatives spoken with evidenced that visits happen without any restrictions attached. Relatives felt that the manager and staff in general keep them well informed of any issues regarding their relative. Menu’s looked nutritious and reflected residents tastes. Food smelt and looked appealing and was presented nicely. Meal times were unrushed and had a relaxed atmosphere. Staff were seen supporting residents discreetly to eat meals. Staff were seen to encourage other residents to use their limited skills by letting them eat unhurried and not worrying about mess caused by allowing them to maintain this independence. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has a complaints procedure and each service users and relative has a service users guide that highlights the complaints procedure. The homes policy and procedures and training of staff protect residents from abuse. EVIDENCE: Since the last inspection there has been no complaints. The manager and staff are available when relatives visit and if they need to raise any concerns or sensitive issues privately they can do so. Residents and a relative spoken with on the day of inspection were complementary about the care provided within the home and felt all staff are approachable. A number of staff spoken with regarding the protection of adults evidenced to me that they knew the correct response and procedures to follow. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The outside environment is pleasant, attractive and provides appropriate and practical usage for the residents of Oaklands. The home environment provides a clean, comfortable and safe environment in which to live in. The home needs to ensure that cleaning materials are kept under Chemicals or substances hazrdous to health regulations. EVIDENCE: The home has fitted a new water tank, thermostatic control valves to keep hot water temperatures at a safe level. All radiator covers have now been fitted. An occupational Therapist has done an environmental assessment and staff have received mandatory, POVA and specialised dementia training. The manager and team have worked hard at achieving the requirements from the last report.

Also creams kept in residents rooms need to have lockable storage supplied to comply with medication regulation and guidelines. Individual rooms are personalised and choice of colours and furnishings are of service users choice. The environment was homely comfortable and practical for the use of service users at Oakland’s. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 and 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The number of staff on duty, their experience and skill is able to meet the needs of residents. Recruitment records are in place and to a good standard. Staff have had mandatory and specific client related training. EVIDENCE: At the time of inspection the skills mix of the staff group was being used positively in meeting the various needs of residents. Staff rotas evidenced that there is adequate cover throughout the day and night. Paperwork for staff recruitment was looked at and overall was to a good standard. The staff team have had a number of recent training packages, which includes mandatory and ‘specific’ client need led training. NVQ training is planned through out this and next year. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35 and 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Management systems are good and the home is run in the best interests of residents. Evidence to show that teamwork have developed the service and improved the standards of care. The management does respond robustly & rectify matters of health & safety when identified. There is a quality assurance monitoring process implemented. EVIDENCE: The manager of the home evidenced through discussion good understanding of their individual responsibilities and knowledge of the care standards, regulations and the Care Standards Act 2000. The manager is hands on with the residents of the home, which sets good examples for staff to follow and

The manager, proprietors and staff were observed to have good communication between the team. The home is in the process of developing a quality assurance system. Once all surveys are back the manager will collate and make a plan of action according to the outcomes and will forward this to the CSCI.
