This inspection was carried out on 14th February 2006.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Poor. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The bedrooms at the Old Rectory are well personalised with residents` own items such as televisions, ornaments and pictures. Staff were friendly and attentive to the residents and had a good knowledge of the residents needs and the character and history of the residents in their care. The home manager is experienced and competent. The home was clean and free from any unpleasant odours. Relatives spoken with said that they "could not fault the care" "staff are very kind and work hard" "the staff are marvellous"

What has improved since the last inspection?

The style of plans were much improved, individualised and painted a picture of each resident. There is a notice board in the main entrance hall with the names and photographs of all staff working at the home to aid the residents to recognise them. New carpet has been purchased for both upstairs and downstairs corridors and the upstairs corridor has been redecorated as required at the last inspection. Five new chairs, new bed tables and eight wheelchairs with specialist footrests and equipment to assist the transfer of residents have been purchased. Six bedrooms have been totally refurbished with carpets, beds, wardrobes and chest of drawers. The vanity units identified at the last inspection have been replaced or repaired. Two of the bathrooms have had a new bath, toilet, wash hand basin and flooring.

What the care home could do better:

Social activities are still not well managed as there are days within each week where no activities take place. Residents are frequently left to sit in small groups without any distraction or stimulation from staff. This was a requirement at the last inspection and has not been met. The present location of the dining room in a link corridor does not provide a homely or positive location and thought of how this can be improved must be built in to the planned extension. The windows at the home are ill fitting and require repair or replacement. Reports of unannounced visits by the proprietor or his representative must be sent to CSCI. There were no recorded visits at the home since July 2005. A requirement was made at the last two inspections regarding this and these have not been met.

Maximum of 35 persons accommodated aged 65 years and above Maximum of 1 named service user D.O.B 01/03/42 be accommodated in the category DE 14th December 2004 Date of last inspection Brief Description of the Service: The Old Rectory is registered to accommodate 35 service users aged over 65 years of age with a diagnosis of dementia. The home is a detatched house which has been extended and converted. Accomodation consists of twenty three single bedrooms and six double bedrooms on two levels. Access between floors is via a passenger lift or stairs. None of the bedrooms at the home have en-suite facilities, wash-hand basins are fitted in all rooms.The home is situated in the village of Capenhurst with easy motorway access. Long and short term placements are offered and the home employs a team of 1st level registered nurses and support workers for the provision of nursing and personal care. This unannounced visit of the home took place over five hours and was carried out as part of the yearly inspection process. A tour of the home was carried out and care records, fire records and staff training files were inspected. The service history of the home and the previous inspection report were read in preparation for the inspection. Staff on duty, residents and relatives were spoken with during the inspection. As the residents are often unable to express their views due to the nature of their health problems, more emphasis was placed in observing and checking records in order to form judgements. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,9. Care plans at the home are detailed and address the changing care needs of the residents. Medication is administered, recorded, stored and disposed of correctly to maintain the safety of the residents living at the Old Rectory. EVIDENCE: Five care plans were examined and these were completed using the improved style of plans. The care plans were much improved, individualised and painted a picture of each resident. All plans of care were detailed and assessed areas of need such as mobility, falls, moving & handling, continence, pressure area & tissue viability, nutrition and general dependency making reference in each area assessed of the effect of the residents’ mental health on their behaviour and vulnerability. All were up dated and reviewed on a regular basis. A record was also made of support from, and visits by other health professionals such as GP’s and consultant psychiatrist. Detailed information given regarding relatives has been incorporated in to the care plans. Resident choices were documented such as times of rising and retiring and likes and dislikes of food. Medication recording, management, storage and disposal were inspected and found to be satisfactory. Medicine Administration Records were examined and found to have been completed correctly. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 Social activities are not taking place to ensure some stimulation is in place for the residents living at the home. Relatives and friends can visit at any time and resident choice is recorded in the care plans. EVIDENCE: The home has employed an activities co-ordinator since the last inspection but they have been of sick since December 2005. Social activities are still not well managed and there are days within each week where no activities take place. Residents are frequently left to sit in small groups without any distraction or stimulation from staff. This was a requirement on the last inspection and must be addressed. Activities are particularly important for people with dementia. It was discussed with the care manager that as some residents are not able to join in the more structured type of activities that a more individual approach is applied. Visiting at the home is open and relatives and friends of residents are an important aspect of the home. Residents are not taken out in to the community at present. Choice is recorded in the care plan with diet and food preferences and the likes and dislikes of residents are discussed with the relatives. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Complaints recorded are not confidential and the action taken is not recorded to ensure that residents and relatives are confident that their complaint is acted upon. EVIDENCE: The home has a complaint book to record complaints, however, this is kept in the office and can be accessed by residents, staff and visitors therefore complaints made are not confidential. There is no recording of investigations or of action taken by the home following a complaint to ensure it does not reoccur. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,26 Some areas of the home require attention to ensure that residents live in a safe and well-maintained environment. The home is clean and free from unpleasant smells. EVIDENCE: The home has met some requirements from the last inspection regarding the environment. Eight bedrooms have been completely refurbished with new furniture, carpet, curtains and wallpaper. The identified vanity units from the last inspection have been mended or replaced. The carpet in the corridors on both floors has been replaced and the upstairs corridor has been redecorated. Sensory boards have been placed along one of the corridors to give the residents at the home a tactile experience. This was a requirement at the last inspection and has not been met. There was evidence that the lawn had been mowed and the garden area looked generally tidier. The home was clean and free from unpleasant smells. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 30 Training is in place to up-date all staff in meeting residents care needs. EVIDENCE: Staff at the home, receive essential training such as moving and handling, fire training and health and safety. Other training has taken place in communication and infection control. A course on challenging behaviour for care staff is to commence April 2006. Two staff have been trained to train staff in safe handling of residents. NVQ training in care is in place at the home and over fifty per cent of care staff have completed the qualification, which is in line with the national recommendation for care homes. A computer has been obtained from the Cheshire Care consortium to assist staff with training. The care manager has achieved NVQ level 4 in management. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,35,36,38 Some areas of management need to improve to maintain the safety of the residents living at The Old Rectory. EVIDENCE: The registered manager is an experienced nurse but has not yet achieved NVQ level 4 in management. The home does not have responsibility for any residents’ personal finances. Informal supervision of the staff takes place on a daily basis and formal supervision sessions are recorded. The proprietor or his designated representative must visit the home on a monthly basis, unannounced, to check on all health and safety, property and equipment and staffing issues. There were no recorded visits at the home since July 2005. The fire log was checked and staff training has taken place and been recorded.
