Conditions of registration: May provide residential personal care for up to 26 persons, aged over 65 years, with dementia. The Responsible Individual to visit the home on a weekly basis until a registered manager is in place. The two additional bedrooms are not to be used until minimum standards have been achieved and checked by the inspector. Date of last inspection 14 December 2004 Brief Description of the Service: Osborne House is a pleasant period building with panoramic views over the Bristol Channel. At the time of writing the Commission is awaiting an application from a newly appointed Nurse Manager. This is an overview of what the inspector found during the inspection. This announced inspection took place to investigate a complaint originating from a previous employee. The inspector looked in some depth at the areas of complaint. The inspector found evidence that the management team were proactive and attempting to modernise care practices prior to the complaint. The inspector found evidence of good practice and failed to substantiate the complaints, the inspector also noted that the areas of concern had been identified and assessed by the management team prior to the commission inspecting. The inspector notes a newly appointed manager is due to start at the home within three months with a view to implementing a changing model of person centred care. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3,5 The homes statement of purpose reflects the team’s philosophy of care and meets national minimum standards. New service users are admitted after a comprehensive process. Prospective service users are given the opportunity to visit the home prior to coming for a trial visit. EVIDENCE: The inspector audited the statement of purpose and service user guide. Both meet the requirements of the national minimum standards. The inspector audited 2 admission assessments; both were comprehensive assessments of the service needs at the point of admission, meeting national minimum standards. Statement and documentary evidence demonstrated that prospective service users and their families are invited to visit the home prior to a trial period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,10 Service users needs are set out and addressed in a plan of care. Service users are treated with respect and dignity with staff respecting their privacy. EVIDENCE: The inspector audited three plans of care. All were based around a comprehensive assessment of service users needs based around a person centred approach. Where possible the plan is drawn up with the involvement of the service user. Documentary and staff statement evidence demonstrated that staff treat service users with dignity and respect. On inspection service users were addressed by their preferred name and when bathed or toileted their privacy was respected. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13,15 Service users are supported to maintain contact with friends or family. Service users receive a nutritious well balanced diet. EVIDENCE: Service users are supported to contact their family by phone or in writing to maintain contact. The home encourages visits from friends and family with an open visiting hours policy, (within normal social hours). The inspector observed a pleasant appealing meal to be served to service users in pleasant congenial surroundings. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 Any complaints are dealt with seriously and in good time by the management team. Service users are safe guarded from abuse. EVIDENCE: The inspector noted on investigating a complaint that home exceeded national minimum standards when dealing with a complaint. The management acted proactively, identifying potential areas of difficulty and addressing them promptly. The management team have used the commission as a resource and have been completely transparent in all dealings with the commission. The inspector also noted that an employee complaint originated as a result of improved care practices and fully supports the management teams course of action. The home has appropriate policies and procedures in place to safe guard service users from abuse. Staff interviewed were clear about what constituted abuse and their course of action should they have concerns. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19. 20,24,26 The home is pleasant well maintained and homely. The environment meets National Minimum Standards. EVIDENCE: The inspector notes the scale of environmental improvements by the current management team in order to maintain and improve this very pleasant home. The inspector noted a program of regular maintenance and improvements. The grounds are well maintained and the general environment is safe throughout. Communal space meets or exceeds National Minimum Standards. Service users bedrooms are furnished and equipped to meet service users needs. The premises have recently had a warm air circulation system installed and were clean, pleasant and odour free throughout.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29 The home has sufficient staff on duty to meet service users assessed needs. The home has a thorough recruitment policy to protect vulnerable adults. EVIDENCE: The inspector audited the staff rota and took staff statement evidence for staffing. The management team have altered the shift times for the night to morning overlap period. This was changed in response to staff requests for more staff at the beginning of the morning. The inspector notes the home has sufficient staffing to meet the needs of service users. Staffing must be kept under constant review by the management team and be responsive to the varying needs of service users. Two recent staff files were audited. Both contained appropriate references, identification and CRB information. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32,37, 38 The home benefits from an open, positive and inclusive management approach. The home has appropriate records to protect service users. The health and safety of service users and staff is protected. EVIDENCE: The management team communicate a clear direction to staff via a range of strategies to improve and review care practice. These include questionnaires to staff and visitors and a managerial review and meetings with staff. The complaint being investigated gave clear evidence that the management processes were clear and transparent and that the management team was engaging staff with clear strategies to encourage innovation, creativity and development within the team. The home exceeds this standard.

The management ensure safe working practices through appropriate health and safety, fire and manual handling in addition to having completed a full health and safety audit of the premises. This included review of hot surfaces, water temperatures and a comprehensive risk assessment of the premises.
