This inspection was carried out on 29th June 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home is very well managed with a manager and staff team who put the wellbeing of the residents first and foremost. Residents say they are really treated very well in the home. Although the home is in a rural setting and has in-house day service facilities the resident also have many opportunities to access the wider community. The forthcoming events and activities are varied and frequent, taking account all residents interests. The home has regular residents meetings, which are very important to the residents and from reading the minutes the inspector ascertained they are very pro-active, the meetings are well attended and chaired by a member of staff.

What has improved since the last inspection?

Risk assessments regarding smoking, tissue viability and the management of challenging behaviour have been reviewed and now in place.

What the care home could do better:

No recommendations or requirements were made following this inspection. However, the registered manager should ensure the home is clean, with no offensive odours.

The maximum number of adults to be accommodated is forty (40) Date of last inspection 5 October 2004 Brief Description of the Service: Parkgate Manor is a large detached house and provideng residential care for up to 40 adults over the age of 30, with a mild to moderate learning disability. The home is set in extensive grounds, approximatley 2 miles from the town of Battle. Residents rooms are on the ground and first floors, and there are a variety of attractive commonual rooms. The gardens are also a well-used resource. The home has its own day centre, offering a range of opportunities in artistc, domestic and work related activities. It also enables access for residents to a wide variety of community events and educational facilities. This unannounced inspection took place between 10.00 and 14.00 with the inspector made very welcome to the home. The inspector was impressed by the high standards of care and support and of the excellent qualities of the management team and staff. During the course of the inspection, five residents were happy to speak with the inspector both privately and within a group, passing on news about their lives, recent outings and the night before disco. Also four staff were interviewed and they all spoke positively about the home, their roles within the home and the training their had undertaken. The inspector spent time in the office with the registered manager and the assistant manager reviewing care plans and other documentation. This was followed by a full tour of the home including viewing a selection of residents’ bedrooms. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4 & 5 Prospective residents are provided with a good level of written information about the service, and there are comprehensive procedures, in place, which include formal and informal information collected and the opportunity for prospective residents to visit the home. EVIDENCE: The resident group has tended to be stable, however, a new resident had just arrived a few days before this inspection. All the documentation including the gathering of information to make the new resident’s care plan was seen by the inspector. It was evident that the resident had, had access to suitable written information to inform her of the service and what it is like to live at Parkgate Manor in the form of a welcome pack, which includes the statement of purpose and service users guide Parkgate Manor H59-H10 S21184 Parkgate Manor V222821 170505 Stage 4.doc Version 1.20 Page 8 Individual Needs and Choices The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,8,9, & 10 Residents’ care and support needs are well managed enhancing residents’ independence and overall quality of life. EVIDENCE: Residents told the inspector that the staff knew their needs well, as was seen during the inspection. Care plans were inspected and indicated that the standard and depth of recording is very good. In hearing about peoples news, the inspector was assured that needs are regularly reviewed with any changes managed in discussion with the resident and other relevant people. The residents have monthly meetings and from the minutes seen it was evident to acknowledge that this was a forum for residents to discuss the running of the service, any changes and plans for the future. A discussion with one resident evidenced the level of support given within a sound risk assessment process. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,13,14,15,16&17. Residents have every opportunity to live a fulfilled, responsible, valued and socially active life. EVIDENCE: The residents on the day of the inspection were seen to be occupied in various activities. The inspector visited the day centre and spoke to the residents participating in art and craft activities. It is evident that the residents dictate the pace of life, with more elderly people taking the day slowly, sitting quietly content in the home’s lounges and in the garden under erected gazebos, with the staff offering support and refreshments. The residents’ notice board was full of all the activities planned for the coming months. The manager explained to the inspector that some of the external courses attended by the residents had now stopped for the summer, and therefore the home has the summer months forthcoming events planned around trips out, holidays and entertainers coming to the home. The home produces a Parkgate News information newsletter for residents, families and friends, and the inspector was given the July/August 2005 edition, which confirmed that the residents has a variety of activities planned for the summer. This newsletter requested the filling in of a quality assurance survey, to ensure that the home is providing a good service. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 & 19 Residents’ health and care needs are very well managed and take into account residents’ individual preferences and needs EVIDENCE: Residents feel that they are treated as individuals and that this is respected in care giving and support. Health care is well managed with residents encouraged to take responsibility as far as they are able. A resident spoke of her eye operation and how her sight had improved, she told the inspector that the manager and staff had supported her and she felt very lucky to be living at Parkgate Manor. Health records indicated frequent visits to the chiropodist dentist and optician and the manager confirmed that the home had a good relationship with the local GP surgery. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 & 23 Residents are able to raise concerns and be assured that these concerns will be listened to and acted on. The Residents are protected as far as possible from the risks of harm or abuse. EVIDENCE: Residents have a range of opportunities to voice concerns and worries about the service. It was evident from the residents’ meeting minutes, that these meetings are one place where residents felt comfortable to raise concerns. A resident informed the inspector that if they had worries they would speak to the manager. The home has comprehensive policies and procedures on the prevention of abuse; staff understanding of such policies was explored during discussion with them and they were found to have a good understanding and awareness. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25,2728 &30 Residents enjoy living a Parkgate Manor, it is safe, nicely decorated and although it is a large house, it is well maintained and homely. EVIDENCE: Parkgate Manor is a large property, but maintains a homely atmosphere with good quality furnishings and subtle decor throughout. Bedrooms are generally single but there are a few double rooms, which the manager confirmed that the residents have agreed and enjoy sparing together. All the bedrooms seem by the inspector were furnished to an acceptable standard with many personal items belonging to the residents. The communal areas are spacious and provide many areas for quite relaxing and watching of the television. This inspection was carried out on a very hot day and the home was airy with doors and windows open, and many residents were enjoying the garden under the shade provided by gazebos, it was also noticed that cold drinks were provided throughout the day. There was a slight odour on the top corridor and in a double bedroom not serious enough to be offensive, also areas around some light switches were stained. The manager agreed to review the cleaning of these areas and would be discussing these areas with the cleaning staff. JUDGEMENT – we looked at outcomes for standard(s) 32 & 35 Residents quality of life is enhanced by the support and expertise of the staff employed. EVIDENCE: Residents spoke very highly of the manager and the staff team. It was felt the manager and staff were very approachable. It was evident that staff listened to residents and supported rather that directing their lives. The staff interviewed all expressed how much they enjoyed their work. Residents have a range of abilities and wishes and staff receive in house and external training and direction to work with individuals their preferred manner. The manager confirmed that the staff are undertaking NVQ level 2/3 training, and this training is on track with 50 of the care staff trained. JUDGEMENT – we looked at outcomes for standard(s) 37,38 & 42 Residents are safe in the home, being well managed and maintained. EVIDENCE: A competent and well-qualified manager manages the home. She has qualifications in nursing and management, which are equivalent to the NVQ, level 4 in care and management and surpass this standard. Resident and staff spoke very highly of the management of the home, and from speaking to residents the manager is a consistent feature in their lives. The staff are expected to follow the principles of care that will benefit the residents to develop their potential and have a good quality of life. The documentation in relation to the health and safety of the residents was found to be in good order and well maintained.
