Care plans do not provide sufficient information about individuals care needs and how these are met by staff. Risk assessments also have insufficient information about how risks are minimised and the actions to be taken by staff. The daily records varied in content and quality of recording. It was not always clear what actions had been taken after incidents occurred. Reviews of individuals care are not always being carried out. Staff are generally able to describe peoples care needs, but the lack of consistent recording means that care can be compromised. Staff training in moving and handling has not been provided for all staff. Some new staff have not completed moving and handling training. One staff member who moved from another service had not completed training that reflected the needs of the people living at Pengarth. This person had previously worked with people who were more mobile than those living at Pengarth. There was evidence that induction training is not always completed for staff who have worked for the organisation previously. As the people living at Pengarth have very specialist needs it is important for staff to complete specific induction training. Complaints records did not provide information about how matters were investigated and what the outcome of this was. Two complaints were recorded, howeve another complaint concerning the service was received at the organisation headquarters and the details of this complaint, its investigation and outcome were not in the records at Pengarth. What the care home does well: What they could do better:
Care plans should have more information about individual needs and how these are met. Risk assessments need to be clear and show how risks are managed and minimised. Staff training in moving and handling that is relevant to the people living at Pengarth must be completed by all care staff. Induction training must be completed by all staff. Complaint records must include information about any complaints, the investigation and the outome of the complaint. This should include whether or not the complainant is happy with the outcome.
