Conditions of registration: 1. The home is registered to accommodate 15 people in the category PD (18 65 years) for a maximum period of 6 months. 2. A maximum of 4 people may be accommodated who have a physical disability and a learniing disability. (PD/LD). 3. A maximum of 1 person who has a physical disability or physical disability/learning disability and is over the age of 65 years (PD(E) & PD/LD(E)) may be accommodated at any one time. 4. The three bedrooms in flat 3 that are less than 12 square metres must not be used to accommodate wheelchair users. 5. The total number of service users must not exceed 15. Date of last inspection 17.1.05 Brief Description of the Service: Pine Lodge is a home run by the local authority. The home currently offers rehabilitation and respite care to people with a physical disability who may have an associated learning disability. All accommodation is ground floor. The unit is divided into three ‘flats’ two designated to respite care the other to rehabilitation for a maximum stay of six months. Extensive work has been carried out to upgrade the building and the home now has accessible facilities for wheelchair users. Each flat has a lounge /dining area and kitchenette. There is a communal area used by all three flats and a main kitchen and laundry. Service users access local day/work placements or stay within the home during the day.The home is close to local amenities such as the post office, shops and library. This is an overview of what the inspector found during the inspection. This inspection took place over 4 hours during the afternoon and early evening and was carried out as part of the regular inspection visits required by law. During the visit all service users were spoken to and 4 were interviewed in greater depth. Discussion took place with the manager and three of the staff on duty. A tour of all the communal areas and some bedrooms took place and staff, fire and accident records were inspected. No requirements or recommendations were made at this inspection. What the service does well: What has improved since the last inspection?
Since the last inspection the building work has been completed, the standard of the rooms, facilities and decoration is very high. This is now a comfortable and homely place to live. There are good facilities for wheelchair users, the kitchens have low-level cupboards, sink, cooker and fridge, and there is an accessible laundry. The bathrooms provide plenty of space for staff to assist people who use a hoist and baths that are suitable for all levels of physical need.

Additional staff have been recruited as the numbers of service users has increased there are extra staff that the home can call on in emergencies. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 & 9 The attitude and approach of the staff team is to create an environment where service users are responsible for their own lives, empowering people rather than taking over. EVIDENCE: Service users said that they are able to make decisions about their lives and staff are available to support them if they request assistance. Everyone said that they choose what time to go to bed and what time to get up, where they wish to take their meals and when they want a bath or shower. One service user said that he felt like a prisoner in the last home he used for respite care however went on to say that things are completely opposite at Pine Lodge where he feels fully involved in all aspects of his care. Service users and staff confirmed that service users take risks as part of everyday life. All risks are assessed and action taken to reduce these wherever possible. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 & 16 &17 The service puts the rights and preferences of service users at the heart of the service, however the staff must be mindful that some preferences of service users for example eating together in the communal area may create an ‘institutional’ feel which the staff have worked so hard to eradicate. EVIDENCE: Service users said that they continue with their usual daily activities when they spend time at Pine Lodge. They said they feel part of the community and go to the shops and local club. Where service users require staff assistance to go out due to their physical needs then this is arranged. The improvements to the environment particularly the bathrooms and toilets have greatly improved the privacy and dignity for service users. Access for wheelchair users is now very easy and there is sufficient room for people to be comfortably assisted with their personal hygiene. One person sent written comments and said that during their stay they found the food was excellent offering a choice of three different options at every mealtime. During the inspection the majority of service users ate their evening meal in the central communal area and were not using the dining areas in their own flats. Staff confirmed and service users said that it is individual choice as to where they take their meals. A member of staff said that she was concerned that those service users who are staying at Pine Lodge for rehabilitation and access into the community were not benefiting by taking their meals in the communal area. A service user who is new to the home and who arrived that day said that she would ‘go along with’ wherever anyone else sat to take their meals. Staff should ensure that it does not become custom and practice for service users to take their meals together in the central communal area, but encourage the expectation that meals are taken in individual flats in the usual domestic way with people coming together for particular occasions. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 Personal support is delivered in accordance with service users’ preferences and in a way that protects the dignity and privacy of service users. EVIDENCE: Service users said that staff are sensitive and discreet when they assist them with their intimate personal care needs. It was evident that those service users who are not able to carry out their own personal care are well supported by staff, who pay attention to detail. For example one service user appeared well turned out, had been shaved that day; his hair had been brushed, he was clean and was wearing clothes that were fresh and had been ironed. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24 & 30 Recent major investment has significantly improved the standard of the building and the environment is now very good, the home provides service users with a comfortable and homely place to live. EVIDENCE: The building work that was carried out throughout 2004 has now been completed and all three flats are in use. All areas are contemporary and create a modern and pleasant environment. Curtains, furniture and soft furnishings have been used thoughtfully to create a variety of areas for service users use. The bathrooms are now easily accessible by wheelchair users; there is plenty of room for staff to comfortably assist those who use a hoist and specialist baths that are suitable for people with all levels of physical needs. Each bath has a shower attachment for those who prefer this facility. Service users said they are very pleased with the facilities. Each flat has a kitchen that is suitable for use by a wheelchair user. Again fixtures and fittings are modern and of good quality. Service users now have

Wheelchairs have marked some of the walls in the corridors. The manager is implementing an ingenious idea and the lower parts of the corridor walls are being covered in MDF that has been routed and very effectively creates the look of tongue and groove panelling at a fraction of the price. This is not only protecting the walls but enhancing the domestic/contemporary style throughout the home. The manager said there are some finishing touches such as pictures that are not yet in place however these are in the process of being bought. There is a high standard of cleanliness throughout; service users commented that they are pleased with the levels of hygiene in the home. One service user who made written comments raised the issue of service users smoking in the central recreation area; they are not a smoker and did find this unpleasant during their period of respite. It is acknowledged that this is a problem, however as the numbers of service users at any one time increases and people are encouraged to use the communal areas in individual flats then this may alleviate the problem. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35 There is a good staff team who are committed to providing a good service and keen to participate in regular training both in-house and externally. EVIDENCE: As all three flats are now functioning additional staff are rostered to work on each shift. There is now 3 or 4 care staff on duty throughout the day and evening and two staff at night. In addition to this there is a senior member of staff throughout the day and evening, the manager, domestic staff and the cook. Care staff have been recruited over recent months in anticipation for all flats to be in use. The home also has a bank of relief staff to call on in emergencies. Records demonstrated that there is a robust recruitment process that ensures all relevant checks are made on new staff and have the right qualities and competencies for the job. Two away days are planned in the near future for staff to attend in two groups where training will be delivered. The relief member of staff confirmed that she is included in all training and has completed training in fire procedures and moving and handling and has recently done a course in the Protection of Vulnerable Adults (POVA).

JUDGEMENT – we looked at outcomes for standard(s) 38 & 42 The manager is supported well by the senior staff in providing clear leadership throughout the home, everyone is aware of their roles and responsibilities and work to the visions and values of the service. EVIDENCE: Staff confirmed that they are enabled and encouraged to express their views through regular staff meetings and supervision. A service users meeting is planned and service users said that they would have no hesitation in speaking to staff or the manager if they had any concerns. One service user said that they had written to the manager after their stay, some suggestions they made were acted upon and changes have been made. A new service manager is now in post and will be overseeing the running of the home on behalf of Northants County Council. The registered manager said she has met her new line manager and considers they will have a good working relationship. Fire, accident and incident records were complete and up to date. Where issues had been highlighted action was taken to resolve matters. All required checks are carried out and staff confirmed they had received training in matters around health and safety.
