What the care home does well

Service users` rights are respected and responsibilities recognised in their daily lives to enable continued independence. Service users are offered plenty of fresh food with lots of choices to enable a balanced and healthy diet. A comfortable and safe standard of accommodation is provided for the service users, which meets their needs. The home is managed efficiently and service users benefit from a well run home with their views being sought on the running and development of the home.

What has improved since the last inspection?

The lounge has been refurbished and the pipe work repaired ensuring a homely environment for service users.

What the care home could do better:

As some individuals` needs have changed recently a re-assessment of everyone`s needs would be beneficial to ensure the home can still meet service users` needs. All staff must receive training in adult protection and fire training to further protect service users

Service users must be at least 45 years of age on admission Date of last inspection 26th July 2005 Brief Description of the Service: 296 Old Worting Road is a large property set in a quiet suburban street. It is within easy reach of the local facilities with a bus route to the centre of Basingstoke. Together is the registered provider and the registered manager is Yvonne Rabson. The home is registered to provide care and accommodation to seven service users who have mental health issues. The home comprises of seven single bedrooms, one sitting room, kitchen/diner and laundry facilities. The garden is well maintained, providing additional recreational space. 296 Old Worting Road encourages service users to retain their own privacy and endeavours to support them in reaching their own personal goals. 296 Old Worting Road (Pines) DS0000012310.V274822.R01.S.doc Version 5.1 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This unannounced inspection took place over two and a quarter hours and the inspector also went back on 26th January to look at staff records for a further hour. The manager was available on the second day of the inspection and the staff and service users also assisted with both visits. Many of the core standards were inspected during the previous visit on 26th July 2005; therefore referral to both reports will give a full overview of the service. An opportunity was taken to look around parts of the home, view some records and talk to three staff and three service users. Relatives were not visiting on the day of inspection. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 The home has a system of assessment, which identifies prospective service users needs and how they will be met but all areas of an individual’s life needs to be assessed and recorded to identify all aspects of their lives. EVIDENCE: At the last inspection it was discussed with the manager that all areas of an assessment should be completed to show all aspects of a person’s life has been assessed even if a particular area is not applicable to them. This has not happened and as some individuals needs have changed recently it was suggested all service users living in the home have a re-assessment at the earliest opportunity. 296 Old Worting Road (Pines) DS0000012310.V274822.R01.S.doc Version 5.1 Page 9 Individual Needs and Choices The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 Service users do make decisions about their lives with assistance as needed. EVIDENCE: Service users spoken to said they are supported to make decisions and this includes going out for a walk whenever they want. Another service user said they are being supported to eventually move on to more independent living. On the day of the inspection one service user was being supported to plant flowers in the garden, as they wanted to have a memorial for their cat that had recently died. There are also one to one meetings with keyworkers and these meetings are recorded. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 17 Service users’ rights are respected and responsibilities recognised in their daily lives to enable continued independence. Service users are offered plenty of fresh food with lots of choices to enable a balanced and healthy diet. EVIDENCE: There is a timetable in the dining area showing who is responsible for what jobs around the house. This was seen during the inspection. Service users said they are treated with respect and staff would only come into their bedrooms if invited. Staff were seen supporting service users to do what they want such as going out for a walk or looking after the cat. 296 Old Worting Road (Pines) DS0000012310.V274822.R01.S.doc Version 5.1 Page 11 The menu was seen and there are two choices with a tick to indicate what choice has been made. On the day of the inspection plenty of fresh fruit and vegetables were available. Service users prepare their own breakfast and lunch and staff prepare the evening meal. One service user said they would make themselves something if they didn’t like what was on the menu. One service user was seen making themselves a sandwich on the day of the inspection. 296 Old Worting Road (Pines) DS0000012310.V274822.R01.S.doc Version 5.1 Page 12 Personal and Healthcare Support The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20 The home has good systems in place that ensures the medication needs of service users are met. EVIDENCE: The home has a policy on medication and a copy of the Royal Pharmaceutical Society Guidelines. All the relevant documentation is in place relating to receiving, administering and disposal of medication and the inspector viewed this. All staff receive training by having a six monthly assessment completed by the manager and answering relevant questions on the medication process. Two service users require controlled medication and the relevant process in undertaken. Some service users self medicate and there is a care plan in place detailing agreement and that regular reviews are in place. 296 Old Worting Road (Pines) DS0000012310.V274822.R01.S.doc Version 5.1 Page 13 It was discussed with the manager that for the one service user who takes ‘as required’ medication but would not necessarily request this, documentation should be available for staff detailing when this should be offered. 296 Old Worting Road (Pines) DS0000012310.V274822.R01.S.doc Version 5.1 Page 14 Concerns, Complaints and Protection The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 and 23 Arrangements for protecting service users and responding to concerns are satisfactory. EVIDENCE: Most areas of these standards were assessed during the last inspection but the manager confirmed there have been no new complaints and any complaints would be kept in individual files. It was discussed that a system needs to be in place to ensure the complaints can be located easily. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 A comfortable and safe standard of accommodation is provided for the service users, which meets their needs. EVIDENCE: The home is well maintained and suited to the service user’s needs. It is decorated to a standard that creates a comfortable and homely ambience. The lounge has been redecorated with new sofas and a new carpet. The service user spoken to said it looked much better. The pipe work in the house has also been repaired. Service users benefit from clarity of staff roles and responsibilities. Service users are supported by competent and qualified staff. Service users are supported by an effective staff team. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34 and 35 There is a good skill mix of well trained staff to support service users. The relevant recruitment practices in place ensure service users are protected. EVIDENCE: The inspector viewed three staff files and all contained a criminal records bureau check, application form, references and a health check. There was also evidence of regular supervision and staff spoken to confirmed this. Staff have many years experience in this field of work and have received core training and some have received training relevant in the area of mental health. It was discussed with the manager that each staff member should have an individual record of training making it easier to see what training has been completed. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39 and 42 The home is managed efficiently and service users benefit from a well run home. Service users views are sought to ensure they are involved in the selfmonitoring, reviewing and development of the home. The health, safety and welfare of service users is generally well promoted but staff must be well trained especially in the area of fire safety. EVIDENCE: The registered manager has the skills and experience to manage the home and service users spoken to said they like the manager and feel she is approachable and easy to work with. 296 Old Worting Road (Pines) DS0000012310.V274822.R01.S.doc Version 5.1 Page 18 There are monthly one to one meetings with service users and their keyworkers to gain the views on the running of the home and this information is passed onto the manager for action. Regulation 26 visits are undertaken on a monthly basis and staff and service users are involved in these visits and this is documented. Service users views are sought annually through questionnaires and this information is then used as part of the annual review of the home. Views are also sought from staff, relatives and professionals. The health and safety of the home is well maintained and most areas were assessed during the last inspection.
