It offers panoramic views of Torbay from Thatcher Rock to Berry Head. It caters for elderly people who have a physical illness or disability that need respite or full time nursing care. There are three lounges and 27 bedrooms, two of which are double bedrooms and seven that have en suite facilities. Several of the bedrooms enjoy sea views. A shaft lift goes to each floor that the Service Users are accommodated on. The majority of the bedrooms are single occupancy and all contain a nurse call bell system and television point, some have their own telephone sockets. There is a small patio area with seating to the rear of the building that is accessible to wheelchair users. Individual and group activities are encouraged with the help of an activities provider. The services of a dentist, optician, chiropodist and hairdresser are available by appointment. Relatives and friends may visit at any reasonable time. Trained nurses are on duty 24 hours a day and are supported by carers, catering and domestic staff. The owner has several other home situated in the South West. This is an overview of what the inspector found during the inspection. This inspection was unannounced. It took place over 2 hours and 45 minutes and was conducted with the matron. The focus of the inspection was to assess the key standards that were not assessed at the last inspection and to discuss a recent complaint made. Each service user has a written contract/ statement of terms and conditions with the home. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8&9 The health care needs of the residents are identified and met. The medication at the home is well managed, promoting good health. EVIDENCE: During the inspection members of the CARRIE team (short term admissions to prevent admission to hospital) were assessing some patients. Their subsequent discussion with the matron confirmed the staffs’ commitment to delivering the best care possible tailored to meet individual need. Equipment for the treatment and prevention of pressure sores was seen during a tour of the home. The care plans examined had nutritional information and manual handling assessments included in them. During the inspection the communication between staff about the changing needs of patients was observes as being effective. The matron showed the inspector the systems in place for medicines administration. Due to the layout of the home the medicines are kept in locked boxes (one for each floor) these are stored in a locked treatment cupboard. The nurse takes the appropriate box with her to the patient to administer the

The local pharmacist delivers the medicines in a ‘Nomad’ system weekly and carries out 6 monthly inspections of the homes systems. The matron said the home has a good working relationship with the pharmacy. The matron has arranged an outside contractor to dispose of out of use medicines as recent legislation requires. At the time of the inspection one patient self administers inhalers only. The matron was heard discussing the review of medications of a patient with a member of the CARRIE team (respite admissions) in order to be able to meet his needs more appropriately. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 &15 The staff encourage patients to remain as independent as possible allowing them to make personal choices and maintaining some control over their life. The staff have a good understanding of the patients cultural, social and recreational needs and the importance of maintaining contact with family and friends and engage the patients in a variety of ways. The meals in the home offer both choice and variety and cater for special dietary needs. EVIDENCE: It was clear, during the inspection, that the matron and other staff know their patients very well and encourage them to spend their time as they wish lending support and care as required. Information about activities in the home is circulated to patients on the day of the activity and they are given an opportunity to join in if they wish. The matron said that some patients go out with relatives or friends sometimes. Relatives were seen coming and going freely during the inspection. The staff were heard being friendly and welcoming to visitors and the matron said they have a good relationship with the current relatives that visit. The matron and cook said that the cook sees new patients following admission to discuss their likes and dislikes and any special needs. The choices that the patients had made for their meals on the day of the inspection was seen, and included a variety of things not just items that were on the menu. The tea time list is destroyed after being used, the matron was advised to keep a copy of the list for future reference when carrying out quality assurance activity. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 The home has a satisfactory complaints system. Staff have a knowledge and understanding of adult protection issues which protects residents from abuse. EVIDENCE: A recent complaint investigated by the matron confirms that the homes laid down complaints procedure and policies were followed as required. Details of the investigation were recorded as necessary and the outcome forwarded to the CSCI for review. The complaints procedure is displayed in the entrance hall and is included in information given to patients admitted to the home. The policies and procedures file contains up to date policies concerning adult protection procedures. The matron is confident that the staff have an awareness of what abuse is and was able to show the inspector the information staff are given as part of their induction and ongoing training. Staff have to sign to say they have received the information. A copy of all the information is also kept in the office accessible to staff at all times. JUDGEMENT – we looked at outcomes for the following standard(s): 29 & 30 Patients needs are met by a satisfactory mix of nurses, carers and ancillary staff who are trained and competent to do their job. The homes recruitment policy and practice protect the residents. EVIDENCE: The matron said that before advertising a vacant post she looks at the current skill mix to decide what grade of staff is required. She said she uses a standard recruitment procedure which includes getting 2 written references, one of which must be from the last employer, and a CRB check. She carries out the interviews herself at the home. Once in post the induction process depends on the individual and their previous experience. There is a checklist that all staff have to complete before being allowed to work unsupervised. They have a three month probationary period before being issued with a permanent contract. Over 50 of carers have an NVQ level 2 or 3. Some overseas staff are employed at the home. The matron confirmed that she is confident that the home has all of the correct documentation for these people and sees their paperwork before their appointment. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 33 & 35 The home reviews aspects of its performance through a programme of self review and consultation, which includes seeking the views of the residents, relatives and staff. The systems in place in the home promote and protect the welfare of the patients and staff. EVIDENCE: A representative of the company carries out Regulation 26 visits (when the owner is not in day to day charge of the home). The report is forwarded to the CSCI as required. Questionnaires are sent to relatives and patients periodically to get their views on the service provided. The matron said that people see her on a daily basis and often informally talk to her about issues or concerns. The persons who manage patients money for them are billed individually for things such as hairdressing and chiropody or newspapers.
