What the care home does well Primley View appeared homely and comfortable. The staff were welcoming and approachable. Appropriate interactions between staff, people living in the home and visitors were heard during the inspection. People were heard knocking on doors before entering rooms. Staff were knowledgeable about what care needs each person had and said relevant information is given during the handover session at the beginning of each shift. What the care home could do better: The safety of people must be the main priority of staff at the home. This must be achieved by ensuring systems and records are in place for staff to know how to safely move and transfer people. Records should be clear about which equipment is to be used and how each transfer is to be performed and staff should follow this plan. Staff should not manually lift a person from the floor unless in an emergency situation and should follow the care plan and training given. Staff must promptly report accidents to the GP or call emergency services if required, especially if a person has complained about pain. Staff should also ensure that family and next of kin are informed promptly of incidents that occur at the home. All Management and staff at the home should be aware of how to correctly report any possible incidents or allegations of abuse according to locally agreed policies. This will ensure any decisions made about investigations are made by a multi disciplinary group of professionals including the home. This will also ensure any possible criminal investigations are not affected. Record keeping must improve in the home. Care Plans must be written in a person centred way which shows how that person should be cared for. Records, assessments and plans should be detailed, accurate, dated and signed. Staff at the home should also ensure that any reporting to the health and safety executive using the RIDDOR (Report of injury and dangerous occurrence) form should contain accurate date and time information. Record keeping by the manager of the home should also improve. She should keep detailed accurate records of any investigation performed at the home. It offers panoramic views of Torbay from Thatcher Rock to Berry Head. The home provides care for elderly people who have a physical illness or disability that need respite or full time nursing care. There is a trained nurse on duty Care Homes for Older People Page 2 of 9 Brief description of the care home 24 hours a day. A shaft lift goes to each floor that people are accommodated on. This home is comfortably furnished throughout creating a warm, friendly and relaxed environment. All of the rooms contain a nurse call bell system and television point, some have their own telephone sockets. There is a small patio area to the rear of the building that is accessible to wheelchair users. The current fees vary and depend on the level of care each person requires. All people living in the home are issued with a contract and some detail of the funded nursing care contribution. The reason for this random inspection was because of information received by the CSCI which led us to believe that regulations may have been breached. The information related to an incident that occurred in the home. We found that despite repeated previous assessments for a person needing a hoist and two staff to transfer, this person was transfered without such equipment. Current care Plans were not written in a clear way to show how staff actually cared for this person and how they transferred this person. Care Plans did not clearly show what equipment was to be used but staff spoken to were able to verbally explain what equipment was needed for people in the home. We found evidence of an incident that occurred in the home, which led to three staff lifting a person from the floor without using equipment indicated in care plans and assessments. This was also performed despite staff having received moving and handling training. We also found that following an incident two register nurses recorded on three separate occasions events of a person complaining of pain following an incident. These reports of pain were not passed onto a GP or hospital until 48 hours following the incident. The person had sustained a significant injury. A report of an incident was reported to the health and safety executive using a RIDDOR (Report of an injury or dangerous occurrence. We found the date and time of information included on this form was inaccurate. We also looked at another incident that occurred in the home and read that one incident resulting in admission to the accident and emergency department was not reported to family until two days later. Whilst performing this random inspection, we found several examples where records, notes and care plans were not completed accurately. We found examples where documents were not dated or signed, we found examples where risk assessments had been changed with no example of the review. We could not see care plans for an incidence that had occurred in the home. We saw documents that had not been dated. However, we also saw examples of good and adequate record keeping. In the process of performing this random inspection we found evidence of another incident which had not been reported correctly to the safeguarding team. The action taken by the Provider and manager at the home is not in line with locally agreed safeguarding procedures followed by social services, Torbay council, police and CSCI. Records were not clear and contained conflicting information about the cause of the incident. The matron said there were no records kept of the investigation conducted, but that staff had written statements. An issue about a member of staff was discussed. There were no records maintained on employment issues raised.
