What the care home does well

The Home offers a relaxed, calm environment. The staff offer care to the residents by showing respect and ensuring their privacy. The Home is kept clean and is decorated to a high standard.

What has improved since the last inspection?

Some of the doors now have automatic closures in the event of the fire alarm being triggered. The scaffolding at the dining room window has now been removed and this area is light and bright.

What the care home could do better:

One area within the Home has a concern with two steps going up and down, which could cause a problem to some residents. At present a childlike gate is in place, which is not seen as appropriate but is to be rectified shortly.

Within the grounds there are forty-three bungalows providing sheltered housing, although these are not part of the registration.The building has three storeys and offers accommodation for twenty service users in fourteen single and three double bedrooms, two of which are occupied as singles at present. There is a passenger and three stair lifts, two lounges, a dining room and attractive, accessible gardens, with garden house and toilet, which is easily accessed. Quebec Hall is not suitable for service users with severe mobility problems, as a consequence of the frequent change in floor levels. This was an unannounced inspection, which took place on a Sunday morning over one and a half hours. The Deputy Manager was able to show the Inspector around the Home and although not on duty was on call. Six staff were on duty to include the cook and kitchen assistant with eighteen resident at present residing at Quebec Hall. This inspection was to ascertain the lifestyle of residents at the weekend by observation and discussion. Records were not looked at, therefore, only some standards were inspected on this occasion. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Although no assessment paperwork was looked at on this inspection the residents were able to say that the care they require is met at Quebec Hall. EVIDENCE: Records were not looked at on this occasion but on talking to the residents. (5 in total) all of them were very contented with the care they receive. Each one was able to discuss the support they get and how it suits their needs. Most of them were already introduced to the Home by day visits, the provision of lunch and had transferred from the sheltered bungalows within the grounds. The fact that the Home and staff know the potential resident well gives them a good insight into the care provision required and could transfer the information directly onto a care plan. The Manager or Deputy Manager (Deputy was spoken Quebec Hall Christian Eventide Home DS0000027266.V257732.R01.S.doc Version 5.0 Page 9 to) will do an assessment on potential residents who are interested in moving to Quebec hall and have not had involvement in the past. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 On speaking and listening to interaction with residents it is evident that people are treated with dignity and respect. EVIDENCE: Throughout the visit it was evident that the staff are respectful and offer care with dignity and retain peoples privacy. Residents spoken to were full of praise of a dedicated staff team. The day of the visit the inspector saw staff knocking and asking if it was alright to enter the bedroom and these rooms can be locked if the resident wishes. Conversations held were polite and courteous. One resident told of how she needs to have all daily requirements to hand and that this is placed in easy reach for independent use, such as the telephone and drinks. A discussion was held over the care support offered especially through the night and how this task is carried out. All rooms have their own private entrance to a toilet and hand wash basin with those residents unable to use the toilet having a commode which they use in the privacy of their own room. During the visit a staff member was carrying to each room a fresh glass and jug of water covered with cling film. This then led to appropriate conversations and interaction between resident and staff. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 and 15 People are made very aware of the Christian way of life within the Home before they arrive and all residents appear very happy and contented with this lifestyle. Residents are encouraged and offered meals and snacks to suit them. EVIDENCE: When people visit Quebec Hall or may be considering becoming a resident it is clear with the way people are greeted and the information offered in the Homes brochure (Seen) that the Home has a Christian ethos with staff employed who understand and will assist residents to follow their beliefs. There are services held throughout the week and on talking to residents this is what they wish to have. Time for prayer, time for reading and time to reflect is what one person said. On talking to two relatives who were visiting on the Sunday they made it clear this Home suited Mum because of its Christian ways and they told of the staff understanding the needs of their relative. “The Home is small with staff able to get to know the residents and offer the lifestyle that suits them.” “We are very happy with the care offered”. Throughout the morning visit residents were offered hot drinks, biscuits and fresh fruit, which was peeled and sliced for those who would like it. On talking to two residents this happens every day with a choice of what fruit they would like prepared for them. (All served neatly on a plate with a napkin). One lady has no appetite but told of how she is constantly encouraged and tempted with different foods. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 23 and 24 The overall opinion of the environment of this Home without looking at any records shows a well-kept, comfortable and clean place with the personal touches making the bedrooms suitable for the individual resident. EVIDENCE: The majority of the visit was spent walking around the Home and talking to residents and staff. The Home is very clean and tidy with nice furnishings and decoration. There are fresh flowers and house plants around the communal areas to add to the homeliness. There are two lounges, one upstairs and one downstairs that are very much in keeping with the period of the building. Both are tastefully decorated and offer a warm, cosy feel to the Home. Services and activities are held in the downstairs lounge and most people tend to occupy this room.(The Quebec Hall Christian Eventide Home DS0000027266.V257732.R01.S.doc Version 5.0 Page 16 day previous the residents had taken part in a coffee morning in aid of Macmillan Nurses and they were discussing it on the day of the inspection). The Home has improved its fire safety by providing self door closures in the event of the fire bell being triggered. However, there is still at least one door that was noted to be propped open with an extinguisher and must be replaced by a proper door closure. (Requirement). In total four residents were seen in their bedrooms. Each room is unique in its shape and size. They contained adequate furniture with them all having personal ornaments, photographs and small items of furniture they have brought from their own homes. All Bedrooms seen have a room off to a toilet and hand washbasin. Each one has large windows and most have a nice view into the grounds or looking up the long drive. Each resident spoken to said they loved their room and were very happy with their accommodation. The assisted bathroom is large and holds a hoist suitable to assist people in and out of the bath. The room was clean and tidy with a pleasant fresh smell. The room is large enough to take a wheelchair, staff members and any equipment that may be required. The main kitchen was seen and appeared clean and tidy with staff wearing the appropriate overalls and head covering to ensure good hygiene management. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 On the day of the inspection the Home had adequate and suitably skilled staff to meet the residents needs. EVIDENCE: On the morning of the inspection one supervisor, three carers, one cook and one kitchen assistant were on duty. With eighteen residents to care for, this number ensured the needs could be carried out in a timely and suitable manner. A lot of the staff, have worked in the Home for some time with one person over twenty years. One new staff member of three weeks talked to the inspector about his induction and the support he is receiving as he learns. (He has experience in another county of residential care and has already gained his NVQ). He is also very keen on developing stimulation and activities for residents and the Home is going to encourage this to supplement the activities programme. One staff member was spending time encouraging a resident to eat some fruit. She sat beside her in the lounge and spoke softly and politely assisting only when the lady was ready to accept assistance. Another staff member was taking round fresh water jugs and drinking glasses to each bedroom. Because she was using the kitchen area she had placed a white hat and overall over her work clothes and once that task was completed removed the protective clothing to carry on with care duties.
