What the care home does well

The staff in the home supports the residents in the home to maintain contact with their families and friends. Visitors to the home are made welcome at all times. The staff interacts sensitively with the residents and are aware of their needs. The residents are treated with respect at all times by the staff working in the home. The manager for the home ensures that the staff are aware of the need to protect the residents and themselves from abuse by providing regular training.

What has improved since the last inspection?

The building of the extension to the home is well advanced. Visitors to the home are updated weekly with the progress of the building work. The manager of the home is ensuring that the construction work does not cause any disruption to the residents.

What the care home could do better:

The manager works to ensure that staff are supported and well trained. Constant monitoring of staff and services provided in the home will ensure that a high standard of care continues to be provided.

A maximum of 7 service users may be accommodated between the ages 55 - 65 in the DE category. 23rd June 2005 Date of last inspection Brief Description of the Service: The home offers nursing care to older people with mental health difficulties. It is a large converted house in a quiet rural area near Titchfield. The home offers accommodation in both single and double rooms. The house is set in its own gardens with car parking for visitors at the front of the house. This was the second statutory unannounced inspection for the year 2005/2006. One inspector conducted the inspection over three and half hours. During the inspection the inspector spoke to residents, staff and a visitor to the home. The inspector received written feedback from the families of some of the residents in the home prior to the inspection. All the comments received were positive and spoke highly of the standards of care that are given in the home. The residents spoken to appeared to be settled and content and were able to confirm that the staff were kind and caring towards them. Their clothes were clean and tidy and their finger nails were trimmed and their hair combed. Members of staff stated that they were well supported by the manager and their colleagues and that the training they received enabled them to provide a continuing high standard of care. The inspector spoke to a visitor to the home who told the inspector she was always made to feel welcome in the home. She commented about the high standard of care that is given to her mother. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 and 14 Visitors are welcomed in the home at any time enabling the residents to maintain contact with their families and friends. The arrangements for meals ensure that the residents have a choice food and are able to eat when and where they wish to do so. EVIDENCE: The home has an “open house” approach to visiting in the home. All visitors to the home are requested to record in a visitor’s book when they enter and leave the building. Any relative wishing to make a late visit to the home is requested to phone the home prior to the visit. This ensures that the home is able to maintain security at all times. Staff in the home are sensitive to the needs of the residents who live in the home. The comprehensive training they receive for caring of the residents with dementia and other complex mental health care gives them insight and understanding about the residents day to day needs. The social care plan highlights the residents’ areas of interests that formed part of their lives before their admission to residential care. The staff ensure that the residents receive a choice of food at each meal and meal times are flexible to accommodate the changes in mood of the residents. JUDGEMENT – we looked at outcomes for the following standard(s): 18 The vulnerable adults procedure ensures that any allegation of abuse will be addressed appropriately. EVIDENCE: Protection of the residents and staff from abuse in the home has a high profile in this home due to the care needs of the client group accommodated in the home. Staff receive regular training updates. The manager reported that all staff have looked closely studied “No Secrets” as part of the training programme. All the trained staff, including the manager, are currently undertaking a training course, “Yesterday, Today and Tomorrow” which discusses and raises awareness of abuse in the elderly. The manager makes awareness of abuse part of each staff members’ formal supervision. Staff are encouraged to have insight into their own feelings and to talk about and be aware of their own limitations. Extra support is provided for staff if the manager identifies areas of concern. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for the following standard(s): 28 and 30 Appropriate training for staff at all levels, that ensures that the residents needs are met. Appropriate training for staff at all levels, which is regularly updated, ensures that service users needs are met and are provided with care that is based on current research and best practice. EVIDENCE: Care staff in the home are undertake training for NVQ at levels 2 and 3. Two staff members have achieved level 3 the remainder have level 2. Four new staff members will begin their training for level 2 within the next few weeks. Staff in the home receive regular training updates. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 and 35 The manager of the home is able to demonstrate that she is suitably qualified to ensure that the home is well run and that the needs of the residents are met. The manager provides strong leadership in the home providing guidance and direction to ensure that standards within the home are maintained at all times. Procedures are in place to ensure that residents’ financial interests are safeguarded. She has undertaken training to update her skills and has completed her Registered Managers award. The manager for the home informed the inspector that she has introduced a bulletin board to inform the families of the residents in the home of any changes that are to take place and to update them on the progress of the building work for the new extension to the home. The information is updated weekly and the manager is very pleased with the response from visitors to the home. The “open door” policy operated by the manager allows visitors to the home to be able to discuss any concerns they may have with her so that action can be taken promptly. Surveys about services in the home, in the form of questionnaires, have been sent out to families of the residents but the manager reported that she had a very poor response to these. The home only looks after small sums of monies for service users. They have a number of residents’ personal allowances in safekeeping. All monies for residents are stored in individual envelopes. Receipts are kept for all purchases on behalf of the residents. Records were checked and found to be in order.
