What the care home does well

This home offers an exceptionally well-maintained environment with particularly attractive gardens. There is both adequate communal and private space so that visitors can be entertained in private or in one of the lounges. Comments from residents "that you couldn`t go wrong here" " how happy they were " and "how much the lovely gardens were enjoyed" reflected their satisfaction with the home and relatives also expressed their satisfaction with staff members who were "patient and caring". Comprehensive assessments, prior to admission, ensure that placements meet the needs of the residents and these form the basis for care plans, which are subject to regular review so that they reflect current needs. Other members of the multidisciplinary healthcare team contribute to residents care as necessary and there is sufficient equipment in use to ensure their safety. Accidents to residents are monitored and appropriate interventions are used where needed. A pharmacist visit also occurred on the day of the inspection and no problems were highlighted. Activities are varied and tailored to suit the needs of the residents and include trips out and visiting entertainers. Regular meetings offer them the opportunity to influence the services that are being provided. Robust recruitment practices and an emphasis on staff training ensure that residents feel confident that they are being cared for by people who have the necessary qualifications and skills to do so.

What has improved since the last inspection?

Since the last inspection a new activities organiser has been employed, the range of activities offered to residents has increased and others are planned. These include trips out and visits from entertainers. New gardeners have been employed and in response to a request from residents have built a "sensory garden" with plants that are particularly attractive to feel or smell and the home has also entered the "Croydon in Bloom " competition. There were only four requirements resulting from the last inspection and all of them have been met in full, no further ones were issued at this visit

What the care home could do better:

No requirements or recommendations were made at this visit. The home will need to continue to build on the good standards that are in place and will be monitored regularly in line with The Care Standards Act 2000.

The home is situated near to the centre of Croydon and is an extended house which provides accomodation over three floors.Two lifts ensure acessibilty throughout the home. All the bedrooms are single occupancy and have been fitted with hand basins and almost half of them have en-suite facilities. There are two dining rooms, a main lounge, two smaller seating areas and an activities room. The home is surrounded by exceptionally well maintained gardens in the front and an orchard to the rear of the property. The home is attractive and well presented and maintained and there are off street parking facilities to the front of the building. This was the homes first inspection for the year 2005/2006 and was unannounced. Previous inspections have always found a well maintained home offering a good quality service and this was confirmed at this visit. Any requirements are always complied with promptly and the comment cards, which had been received by The Commission, reflected the satisfaction of residents and their relatives. The visit took place over three and a half hours during the day and included a partial tour of the premises, the assessment of, a sample of care plans, staff files and training schedules. The records detailing complaints and compliments and accidents were also viewed and the manager conducted a fire drill, during the visit.Ten residents and three relatives who were visiting were spoken to and the lunchtime meal was served during the visit. The Registered Manager has been in post for some time and staff turnover within the home is low. The Commission has not received any complaints about this service since the last inspection and no extra visits have been necessary. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,6 The pre - admission assessment undertaken with potential residents is comprehensive so that they can be sure that the home will suit their needs and any risk of the placement being unsuitable is minimised. This home does not offer intermediate care. EVIDENCE: In the sample of care plans that were assessed there was clear evidence that a comprehensive pre admission assessment is undertaken with potential residents, prior to their moving into the home. This assessment covers both physical and psychosocial needs and then forms the basis for subsequent care planning. It also includes details of their previous life to enable staff to gain an insight into the resident’s current needs and behaviour patterns. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 All residents have an individual care plan, which ensures that the care that they receive meets their assessed needs and that staff are aware of the appropriate action to be taken. Residents are confident that they will be treated with respect, which encourages them to retain their individuality and dignity. EVIDENCE: Each resident has an individual plan of care and five of these were examined during the inspection. Each of them contained a considerable amount of information including risk assessments, manual-handling protocols, and monitoring of risks, which could lead to the incidence of pressure sores. Staff complete daily entries of the care that has been given and there was evidence that the plans are regularly reviewed. There were entries in the plans, which illustrated that the GP, dentists, chiropodists, opticians and tissue viability nurse make visits, and equipment was seen in use throughout the home that is designed to minimise the risk of pressure sores. The accident book was seen, one resident at risk of falling had now got a sensor pad in use on his floor so that staff were alerted when he got up. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 The activities within the home are tailored to ensure that they meet residents needs and visitors are always welcome so that links with friends and families can be maintained and encouraged. A nutritious appealing and wholesome diet helps to add variety to residents lives and they are able to comment on aspects of the home so that they can influence its running. EVIDENCE: The range of activities on offer in the home has increased since the last inspection and the appointment of a new activities organiser. Outside entertainers provide amusement for both residents and their relatives and photographs of these events are on the walls. There have been more outings using the Dial-a –Ride service and it is intended to display a stock of birthday cards and toiletries so that residents can independently purchase items themselves rather than having to rely on their keyworker. There has also been an increase in art sessions the results of which are displayed. Visitors are always welcome and several were in the home on the day of the inspection both sitting with their relatives in the lounge and enjoying the attractive gardens. The lunchtime meal was observed and was well presented and looked appetising. All those spoken to agreed that the food was varied and enjoyable and appropriate records of the food provided were seen. Regular meetings are held, every three months, at which residents can comment on the running of the home. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. JUDGEMENT – we looked at outcomes for standard(s) 16, An appropriate complaints procedure, a copy of which is given to residents as a part of The Service User Guide, and regular meetings ensures that residents can be confident that their complaints will be taken seriously and acted upon. EVIDENCE: There is a complaints procedure in place and residents are given a copy in their Service User Guide. The complaints book was seen and there had not been any entries since the last inspection. The Commission had received none during this period either. Compliments are also recorded and there had been seven of these made since the last visit. JUDGEMENT – we looked at outcomes for standard(s) 19, This home offers an exceptionally well-presented and maintained environment, which meets the needs of its residents and allows them to live in a comfortable homely atmosphere. EVIDENCE: The home is very well maintained both internally and externally and attractively furnished and decorated. There is an ongoing programme of redecoration and residents have been encouraged to personalise their rooms. All areas of the home were scrupulously clean at this inspection and there was no evidence of any malodour. New gardeners have recently been appointed and the gardens are especially attractive .The residents have asked for a “sensory garden “ and an area has been planted with shrubs and plants which smell and feel particularly pleasant and the home is currently competing in the “Croydon in Bloom” scheme. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29,30 Staffing numbers comply with previously set levels and training is given a high priority, ensuring that residents can be confident that they are being cared for by staff with appropriate skills and qualifications and the homes robust recruitment procedures help to safeguard them. EVIDENCE: The off duty rotas were seen and indicated that, in line with previously set levels, there are always sufficient numbers of staff on duty to meet the needs of the residents. Eighty –five to ninety percent of care staff are now qualified to at least NVQ level 2 and four have level 3. A TOPSS training manual was seen which forms part of the induction programme for new staff and contributes towards their qualifications. Training for all staff is a high priority in this home and the schedules were seen. In addition a “ customer focus” scheme is in place called Personal Best. This is aimed at individualising the care that is given in the home. Completion results in the award of a badge. Staff files of five members of staff were seen and were all in order. Evidence of POVA clearance was in the files even where Criminal Records Bureau clearance was still awaited and work permits had been obtained where necessary.
