The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

People who live in this home are well cared for by a well managed, educated, competent team of staff who meet their individual needs. All those spoken with commented on the kindness of the staff and that they responded to their needs in a courteous and respectful manner. Specific comments were "we are well looked after", "my carer is so good" and "everyone is so kind". The assessment, care planning and review of care are thorough with resident`s involvement. Staff receive training in order to meet the needs of both the residents and themselves.

What has improved since the last inspection?

Locks have been installed to all bedrooms and toilets and bathrooms in order to enhance the resident`s privacy but to allow staff to enter in cases of emergency. A programme of installing radiator covers throughout the home continues. This to prevent people being injured by being in contact with hot surfaces.

There is very little that the home can improve upon.

Date of last inspection 18th November 2004 Brief Description of the Service: Red House Nursing Home is situated close the centre of the town of Stamford. The home is three storey building with a two storey extension providing accommodation on ground and first floor levels. Rooms to the first floor are served by a shaft lift. It is registered to provide care and accommodation for service users requiring nursing and personal care over the age of 65 years of age. The home is not on a bus route but there is limited car parking at the side and front of the home. There is a small patio area with a pond at the side of the home with access from the lounge/dining rooms. Most accommodation is single but the home provides 5 double bedrooms, which have en suite provision. The registered provider of the home visits the home every week and works closely with the acting manager. This unannounced inspection started at 8.10 a.m. and was carried out over 5 hours. The inspector spoke to 7 residents, 5 staff, 2 visitors, and a student nurse who was on placement as part of her training as well as the nurse in charge. The main method of inspection was called “case tracking”. This involved selecting three residents and tracking the care they received through the checking of records, discussion with them, the care staff and observation of care records. Comment cards had been received prior to the inspection from 4 of the residents and 2 visitors/relatives. There were no adverse comments. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2 and 3 People living in this home receive detailed information to enable them to make a choice about whether or not they may choose to live in this home. EVIDENCE: Residents receive an assessment before admission and written confirmation is sent to the person to confirm that the home is able to meet their needs. There is a detailed admission procedure, which identifies the needs of residents coming into the home. Records were examined of 2 recent admissions and these confirmed this observation. Each service user also receives a copy of the service user’s guide which, outlines the services provided by the home and is in large print and therefore clearer to read. This document also had information concerning the terms and condition of the admission. Staff clearly knew the needs of the residents by the way they went about their work. Individual records are kept for each resident and inspection of 3 resident’s care records showed evidence of a full detailed assessment. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, and 10 There is good clear care planning in this home. This ensures that the health and welfare needs of the people living in this home are fully met. EVIDENCE: All residents had detailed care plans, which described their health and welfare needs. The records outlined the resident’s agreement to their care plan with their signature, moving and handling assessment, risk assessments, dependency assessments, the care plan. There was evidence of dates of reviews. There were clear directions enabling staff to deliver the individual care to the residents. Care plans also showed evidence of promoting resident’s independence, respect, dignity and choice. All medication is administered by registered nurses all of whom have been assessed as competent. Records showed that they were well maintained. Care plans were individual, clearly written and person focused. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 15 Social activities are available and provide stimulation and interest for residents in the home. Meals are nutritious and varied. Visitors are made welcome whenever they visit the home. EVIDENCE: There is an activities organiser who visits the home Monday, Tuesday and Wednesdays of each week. A programme of activities was displayed on the notice board and on the day of the inspection a visiting minister was administering communion to 3 residents. All residents said they liked the food provided. One commented, “the food is very good”. Breakfast was seen to be served to residents throughout the morning as residents came down for their breakfast. It was served in an unhurried manner. Visitors were observed to be warmly greeted by staff and said they could visit whenever they wished to do so. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Any complaints received are taken seriously and there is a comprehensive complaints procedure. There are systems in place to ensure that residents are safe as a result of abuse training for staff. EVIDENCE: Each resident receives a detailed complaints procedure, which is in the service user’s guide when they are admitted to the home. No complaints have been received by the home or the CSCI since the last inspection. None of the residents or visitors had any complaints about the home. They all felt they could approach the staff if they had any concerns. As part of the homes’ induction programme all staff receive training concerning abuse prevention and all staff receive checks by the Criminal Records Bureau. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 23 and 26 Since the last inspection locks have been installed to all bedrooms, bathrooms and toilets. In addition radiator covers continue to be installed. The building is well maintained and attractively decorated. Any maintenance is attended to promptly. EVIDENCE: Residents said they liked their bedrooms and spoke of how satisfied they were with the cleanliness of the home. Maintenance records showed that equipment was regularly serviced and hot water temperatures tested. The temperatures were within safe limits. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28 and 30 There is a well trained and competent staff team. The number of staff are sufficient for the numbers of residents. Staff are correctly recruited and there is a well established and committed team. EVIDENCE: Residents and visitors did not express any worries about the level or availability of staff. During the inspection, staff were seen to promptly attend to resident’s needs. Staff said they had sufficient time to care and support the residents. Specific comments were “we have a very low turnover of staff”. There was evidence of well trained and motivated team of staff. Training includes National Vocational Qualifications, internal training and attendance at outside study days. There were details of forthcoming training opportunities and the home is a placement for student nurses. One of the student nurses commented, “this is my first placement and I received a very warm welcome and my mentors are very supportive”. Throughout the inspection it was apparent that staff knew what to do and worked as one team. Staff were seen to knock on doors before entering rooms and politely address residents in a friendly manner. They were also seen to laugh and joke with the residents. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32 and 33 Even though there is no registered manager Red House is managed properly. There is leadership, guidance and direction and staff ensure residents receive consistent quality of care. EVIDENCE: There is no registered manager. However a person has been recruited and their application is being processed by the CSCI. Residents, visitors and staff were positive about the staff team throughout the inspection. Comments were “I am comfortable here and because of my blindness staff indicate to me where food is placed”, “I find staff very respectful and kind” and “I am very impressed with the home”. Staff comments were “we work as one team”, “there is plenty of training”, “I like working here” and “I feel supported and safe here”.
