What the care home does well

People who live in this home are well cared for by a well managed, educated, competent team of staff who meet their individual needs. All those spoken with commented on the kindness of the staff and that they responded to their needs in a courteous, kind and respectful manner. The assessment, care planning and review of care are thorough with resident`s involvement. Staff receive training in order to meet the needs of both the residents and themselves.

What has improved since the last inspection?

Four bedrooms have been redecorated following discussion with residents concerning the colour scheme. A bathroom and toilet has been repainted. Ten new lounge and 8 dining room chairs have been purchased.

What the care home could do better:

The acting manager is already identifying improvements. Very detailed internal audits to monitor the quality of service have been introduced.

The home is a three storey building with a two storey extension providing accommodation on ground and first floor levels. Rooms to the first floor are served by a shaft lift. The home is registered to provide care and accommodation for up to 23 people requiring nursing and personal care over 65 years of age. On the day of the inspection there were 22 people living in the home. The home is not on a bus route but there is limited car parking at the side and front of the home. There is a small patio area with a pond at the side of the home with access from the lounge/dining rooms. Most accommodation is single but the home provides 5 double bedrooms, which have en suite provision. The registered provider of the home visits the home every week and works closely with the acting manager. This was an unannounced inspection and started at 8.30 am. It took place over 5 hours. The inspector spoke to 7 residents, one visitor, 5 staff, one nursing student and the acting manager. The main method of the inspection was called “case tracking”. This involved selecting 2 residents and tracking the care they received through the checking of records, discussion with them, the care staff and observation of how their care was delivered. The inspector also observed how care was delivered and how staff responded to other residents living in the home. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 4, 5 and 6 Residents receive information to enable them to make a choice as to whether or not they wish to come to this home. Red House Care Home meets the needs of residents coming into the home. EVIDENCE: All residents are assessed before entering the home and written confirmation is sent to them that the home is able to meet their needs. There is a detailed statement of purpose and service user’s guide and a copy of the service user’s guide is given to each person when being admitted to the home. Staff receive a comprehensive induction programme which enables them to understand the needs of the residents in the home. One recently appointed new member of staff commented, “I received a well planned induction and had a member of staff who supported and guided me during my induction”. Comments from a resident was, “I have come here for respite care and received a very warm welcome and found staff very kind and helpful”. The home does not provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 11 There is good care planning in this home. The health and welfare needs of the people living in his home are fully met. Resident’s dignity is also met. EVIDENCE: All residents had detailed care plans, which described their health and welfare needs. The records included the resident’s agreement to their care plan with their signature, moving and handling assessment, risk assessments, dependency assessments and the care plan. There was evidence of dates of reviews. There were clear directions enabling staff to deliver the individual care to the residents. Care plans also showed evidence of promoting resident’s independence, respect, dignity and choice. All medication is administered by registered nurses all of whom have been assessed as competent. Records showed that they were well maintained. Care plans were individual, clearly written and person focused. Staff receive training on how to care and support those residents who are dying and to their relatives. The home also has a detailed policy concerning death and bereavement, which gives guidance to staff. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 14 and 15 Social activities are well managed with the involvement of residents. Residents receive good and nourishing meals. Visitors can visit whenever they wish to do so. EVIDENCE: On admission details of the resident’s interests are obtained. The home employs an activities organiser. Activities are provided Monday, Tuesday and Wednesday. During the inspection this person was carrying out gentle hand massage for a number of residents. The home has a written programme of activities, which was displayed on a notice board at the entrance to the home. Activities included table games, craftwork, reminiscence, quizzes and gentle exercise. On admission, details of resident’s dietary likes and dislikes are obtained and this information is sent to the Kitchen. Residents were offered a choice of well-balanced and wholesome meals. Staff were seen to ask the residents what they wished to have for their breakfast and approach them in a polite manner. They were also seen to assist those Red House Nursing Home DS0000002638.V261165.R01.S.doc Version 5.0 Page 11 residents who could not feed themselves in a patient, dignified and kind manner. At breakfast there is a choice including a hot meal, at lunch there is a set meal but an alternative is available if required and for tea there is a choice including a hot meal. At other times, snacks and beverages are available. Residents commented positively about the food. Comments were, “food is very enjoyable”, and “I am on a special diet but the home have been so helpful towards me”. The last Environmental Health Officer’s inspection was on the 10/3/2005. There were no concerns. A visitor commented “I can visit whenever I like, I always receive a warm welcome and have confidence in the staff in the home”. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17 and 18 The home takes the issue of addressing complaints and ensuring that residents are safe very seriously and has a comprehensive complaints and adult protection procedure. EVIDENCE: Each resident receives a detailed complaints procedure when they are admitted to the home. No complaints have been received by the CSCI and home since the last inspection. None of the residents had any complaints about the home and felt they could discuss any concerns with staff or the manager. Staff also knew what to do if they received a complaint from a resident. All staff receive adult protection training and staff knew what to do if adult abuse was suspected. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 21, 22, 24 and 25 The home is well maintained, clean and attractively decorated. Furnishings are of a high standard and any maintenance required is attended to swiftly. Residents are also safe. Measures have been provided to ensure that residents are not scalded by being in contact with hot temperatures and surfaces. EVIDENCE: Red House Nursing Home DS0000002638.V261165.R01.S.doc Version 5.0 Page 14 On the ground floor there is one bath with a toilet. There are also 2 independent toilets. On the first floor there are two baths with toilets. There are also 2 independent toilets. There are also en-suite facilities in 5 double bedrooms. There are a variety of mobile hoists, slings and special beds to enable staff to meet the needs of people living in the home. Staff have received training to correctly use the equipment provided. There are 4 single bedrooms all on the first floor, which are below the minimum 10 square metre size requirements. These range from 8.44 to 9.60 square metres. This information has been included in both the statement of purpose and service user’s guide. Residents are encouraged to bring small items of furniture, pictures and personal mementoes into the home. Service users commented positively on how much they enjoyed their bedrooms and how clean they were. Thermostatic controls have been installed throughout the home to all baths and washbasins. The hot water tank has a thermostat installed to control the temperature of hot water. In addition, radiators are thermostatically controlled and have radiator covers Records were being kept of hot water temperatures. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 and 29 There is a well trained and competent staff team. The numbers of staff are sufficient for the numbers of residents. Staff are correctly recruited and there is a very well established team. EVIDENCE: Staff files examined of a member of staff who had been recently recruited showed they had been recruited correctly with checks by the Criminal Records Bureau. The home has exceeded the level of 50 of staff required to have obtained care qualifications (NVQ) by 2005. A further 6 care staff will study for these qualifications in January 2006. All staff receive an appraisal annually. There is an extensive training programme for staff, which includes training in care, internal lectures and training from outside trainers. Staff said how the training provided had enabled them to improve the care and support for the residents. Future training will include stroke awareness, continence promotion and Diabetes. The home also provides training for nursing students. The home received an Investor’s in People award for its commitment to the education and development of staff. During the inspection staff were seen to promptly attend to residents needs. Staff also felt they had sufficient time to care and support the residents. Comments were, “I have received more training here over the past 6 months than I have at a previous home over 5 years” and “there is plenty of training”. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 34, 35, 36, 37 and 38 Although there is no registered manager, Red House is managed properly. There is leadership, guidance and direction. This allows the staff to ensure residents receive consistent quality of care. EVIDENCE: There is no registered manager at the moment. However the acting manager’s application is being processed by the CSCI. The acting manager is a registered nurse with extensive nursing, education and management experience. She and her deputy are studying for a management qualification. Residents, visitors and staff were positive about the staff team throughout the inspection. Comments were, “I find staff very respectful and kind”, “I have no worries or concerns”, “I can discuss any concerns I have with the staff or manager” and “staff are so patient and kind”. Staff comments were “we work as one team”, “there is plenty of training”, “I like working here”, “I feel supported, valued and safe here” and “”when I go home I feel I have done a good job”. Red House Nursing Home DS0000002638.V261165.R01.S.doc Version 5.0 Page 18 A nursing student on her first placement commented, “fantastic”, “I have had good mentorship” and “this has been a very valuable first placement”. Staff receive annual appraisals and care staff and nurses formal supervision. The last resident and visitor’s annual questionnaire was sent out in March 2005. There were very positive responses received. The manager carried out a very detailed internal audit of the home’s 38 quality standards. The results were also very positive. Red House has comprehensive policies and procedures including clinical procedures. A number have been reviewed following the internal audit. The home also had detailed health and safety policies including risk assessment. Records showed that essential equipment had been regularly serviced. Records were well maintained, up to date and available.
