The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Management actively promote liaison between service users, relatives and staff and the sharing of information. Visitors spoken to said that they were always made welcome by staff and felt they that they were fully informed of their relative`s health, general condition and visits by other Health Professionals. Lobley Hill provides homely and welcoming accommodation and continues to be refurbished to a high standard. Families particularly remarked that they liked the home having several smaller lounges that their relative can choose to spend their time in.

What has improved since the last inspection?

There has been an improvement in the way that care records are completed and updated. Care records identify clear instructions for staff to enable them to meet service users needs. Service users, their relatives and staff identified that food is very good and that they always have a choice. Staff identified that they felt that the positive changes had been facilitated by the recent appointment of the new cook. The Home`s Manager has been now been in post since January, however weaknesses still remain with the management of the home, which will be addressed by the Manager having mentorship and supervision programme identified.

What the care home could do better:

Staffing levels remain a concern. Staffing levels were increased following the last inspection however there remain insufficient staff to meet service users needs. There is currently a significant deficit in care staffing hours. There has been a relatively high staff turnover with new care staff not yet in post, necessitating a reliance on existing staff working a considerable number of addition hours and agency staff covering many shifts. Recruitment and selection procedures are unsatisfactory, incomplete and must be improved to safeguard service users. The pre-admission assessment tool used by staff does not identify all required elements as identified within the National Minimum Standards for Older People As a consequence new residents are admitted to the home without a complete picture of their care needs and how these needs may be met by the home. Social and leisure opportunities are inadequate to meet the needs, choices and capabilities of service users who are accommodated at the home. An activities programme must be available, however, to enable care staff to follow this activity programme, sufficient staff must be available. The home must ensure that all residents who require pressure relieving equipment have the correct equipment which is fully functional. Correct pressure relieving equipment will reduce the risk of residents developing pressure sores and also assist in the healing of existing pressure sores.

Date of last inspection 30/11/04 Brief Description of the Service: Lobley Hill Care Centre is a large detached property, which has been considerably extended and improved. The home is currently registered for 45 older people with nursing needs, however recent improvements have resulted in a reduction in the number of bedrooms and the home is now only able to accommodate thirty- eight people. The Home is situated in its own grounds, set back from the main Stourbridge to Wolverhampton Road, near to Wordsley. There is ample parking at the front of the Home. To the rear there are extensive gardens, which provides an attractive outdoor environment. The home is privately owned by Highfield Holdings. The home is on three floors with the lower ground floor accommodating the laundry. Service users are accommodated on the ground and first floor with access to the the first floor by a passenger shaft lift. There are thirty-two single bedrooms of which sixteen have ensuite facilities and three double bedrooms with two having ensuite facilities. The interior of the Home is pleasantly decorated and has been recently refurbished. The home provides five lounges and two dining rooms, an assisted bathroom and toilet facilities, bedrooms and offices on the ground floor. A range of single and double bedrooms, assisted bathing and toilet facilities are also provided on the first floor. This was an unannounced inspection. The inspection proceeded a meeting at the home to discuss a change in the home registration. The time spent on the inspection included a tour of the building, a review of records, talking to four visitors, five service users, four members of staff, the Manager and both the Regional and Area Manager. Care records were reviewed as part of the “case tracking” of four service users, staff records were also reviewed. The Manager has been appointed since the previous inspection and has been in post since January 2005. There has been a recent change in the ownership, with the home coming within the umbrella organisation of Southern Cross Healthcare. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1 and 3 There has been no progress to improve assessment procedures prior to prospective service users being admitted to Lobley Hill. The lack of comprehensive assessments prior to the service user moving into the home give no assurance that their care needs will be met. EVIDENCE: The Manager or another Senior Nurse must assess all service users prior to their admission to the home. A record of pre admission assessments of three new service users did not include all required elements, or even the prompt to direct the Assessor to ensure that required elements are included, as identified within the National Minimum Standards for Older People. In addition one pre admission assessment seen did not identify what medication that service user required. There was no record of the involvement of service users in the assessment and agreement of their care needs. It is important that when ever possible that there is a record of the involvement of either the service user or their representative in their assessment of care needs to ensure that the home is able to meet the service users needs. The Manager was unaware of the need to write to prospective service users following their assessment confirming that the home would be able to meet their needs. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 8 Some progress has been made to ensure that the health care needs of residents are identified and met. There remain weaknesses in care risk assessments and the availablity of required pressure relieving equipment which has the potential to put service users health at risk. EVIDENCE: Four service user care records were reviewed, all contained a plan of care which explicitly identified care needs and instructions how those needs will be met. All care plans were updated at least monthly. Daily entries seen in care records show that staff follow up health concerns. All relatives spoken to stated that they had been very relieved when their loved ones had moved to Lobley Hill, one relative stated how her mother had improved since her move to the home, another service user’s family were very pleased with their mother’s care. All relatives spoken to stated that they were fully informed by staff about the relative’s progress. Risk assessments for the use of bed rails, risk of pressure sores, nutrition and moving and handling risk assessments were incorrectly completed and failed to identify the degree of risk and how the risk would be managed. For example, the bed rail risk assessment identified that the service user was at risk of trapping their head between the bed rail and the mattress, discussions with staff highlighted that they were Lobley Hill Nursing Home E55 S4896 Lobley Hill NH V221195 110405 Stage 4.doc Version 1.20 Page 11 unclear of the requirements of the assessment. Pressure sore risk assessments were found to be inaccurately scored which significantly increased the score which identifies the risk of the service user to develop pressure sores. It was of concern that one service user who is cared for in bed had no pressure relieving mattress, a second service user had a pressure relieving mattress which was not working appropriately. The moving and handling risk assessments were incomplete and did not include the service user weight. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 13 There are insufficient and suitable activities available for people who currently live at the home. EVIDENCE: Service users and relatives spoken to said that residents mostly watch television, although some do read. There is a weekly movement to music session which they can participate in if they wish to and occasional professional entertainment. Visitors spoken to said they would like to see more leisure activities. There were a number of service users who were being cared for in bed, and as a consequence were socially isolated. One service user did have the television switched on, however there was no stimulation for the other three service users visited who were also being cared for in bed. The home has no Activities Organiser since the retirement of the previous post holder, which has resulted in a decrease of activities available. Care staff agreed that there is a need for social activities to be recommenced but this has been difficult within recent staffing levels. There was very limited information available on service users preferred leisure interests, this would also need to be translated into interests that they have that most suits their needs and capabilities. The Manager has identified a programme of activities which needs to be displayed, circulated to all service users and also adopted by care staff. Service users and their relatives spoken to during the inspection confirmed that they are able to receive visitors in private. Visitors spoken to during the inspection stated that they were both able to visit their relative in their own room or in one of the many lounges, saying that the number of lounges available had been one of the many reasons why they had chosen Lobley Hill. Relatives spoken to said that they were always made welcome by staff and felt that they were kept fully informed and updated about their relative’s condition. All service users spoken to said that they like the food and always had a choice of meal available. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 The home is well maintained and pleasantly decorated. Improvements are required to enable service users improved opportunities to safely access and utilise the extensive gardens of the home. EVIDENCE: Areas of the home seen were found to be well maintained, pleasantly decorated, homely and clean. There are extensive gardens at the rear of the home but the grass in the lawn was very long and needed cutting. Access to and within the garden is limited with only a small paved patio area accessible from the dining area, this enables limited access for people with mobility problems or in wheelchairs. There is good access throughout the building where residents are accommodated, with a range of aids and adaptations available for dependent people. The Manager has recently undertaken an Infection Control Audit of the home and is currently identifying required actions following this audit. Highfield have purchased new laundry trolleys since the previous inspection to reduce the risk of cross infection and minimise the requirement to “sort” dirty laundry, Lobley Hill Nursing Home E55 S4896 Lobley Hill NH V221195 110405 Stage 4.doc Version 1.20 Page 16 however, these are being inappropriately stored in the downstairs bathroom. In addition full clinical waste cardboard bins were being stored in the treatment room prior to their weekly collection and alternative storage arrangements must be found. The requirement that hand washing facilities are available in the sluice room has not been met. It is advised that both the full boxes of clinical waste and the dirty laundry trolleys are stored in a dirty utility area which has also hand washing facilities where these could be more appropriately stored. Infection control systems within the home will also be reviewed at the next inspection of the home. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 29 The procedures for the recruitment of staff are not robust and do not provide the safeguards to offer protection to people living in the home. The number of staffing and ways that staff are deployed, is not sufficient to meet the needs of the residents. EVIDENCE: Staffing levels have been increased since the previous inspection, however the increase has been insufficient to ensure residents needs are met. An assessment against a staffing tool that makes recommendation to the number of care staff, identifies a considerable shortfall in recommended staffing levels. Staffing levels are currently for 26 residents: 8am-2pm 1 trained Nurse and 5 Care Staff 2pm-8pm 1 Trained Nurse and 3 Care staff 8pm-8am 1 Trained Nurse and 2 Care Staff. Several care staff members have left, although at the time of the inspection no new care staff have been employed. This reduction in the number of staff has resulted in staff being required to work additional hours and twelve hour day shifts. When staff are unable to work these shifts are filled by agency staff who may be unfamiliar with the residents and their needs. A new cook and domestic have been employed since the last inspection, however their staff files were found to be incomplete. There was no proof of their identity available and no criminal records or protection of vulnerable Lobley Hill Nursing Home E55 S4896 Lobley Hill NH V221195 110405 Stage 4.doc Version 1.20 Page 18 adults checks returned. The Department of Health has explicit guidance that all staff who have any contact with Vulnerable People must have received a satisfactory criminal records check (CRB) prior to commencing employment. In exceptional circumstances new staff may commence work without receipt of a CRB check, however the POVA check (Protection of Vulnerable Adults) must have been received, a risk assessment must be carried out and the new post holder must be appropriately supervised until the CRB disclosure is received. There is also a necessity to share this risk assessment with the Commission for Social Care Inspection, which has not been undertaken. Previous inspections have also highlighted that recruitment and selection procedures do not meet the requirements of the regulations and do not safeguard service users. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32 There is insufficient leadership, guidance and direction to staff to ensure that residents receive consistent quality care. The results of lack of knowledge in some areas and resulting practice do not adequately safeguard the health, safety and welfare of the residents at the home. EVIDENCE: The Home Manager has been in post since January 2005 with Lobley Hill her first position as a Home Manager. It was very disappointing that she highlighted that she had received only very limited support and guidance. The lack of an appropriate induction/ support programme for the new Manager has not enabled her to go forward with the home. She was unaware of the National Minimum Standards and also appeared unfamiliar with the previous inspection report. Her lack of understanding has also resulted in some conflict within the staff team, which has and is being addressed. It is to the new Manager’s credit and her enthusiasm to continue to manage and develop Lobley Hill that she herself raised this with both the Regional Manager and the Inspector. The Commission for Social Care Inspection has received no reports of unannounced visits made by the Home’s proprietors which is required by the regulations. STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard 1 Regulation 4 Requirement The registered person produces and makes available to service users an up to date statement of purpose setting out the aims, objectives, philosophy of care, services and facilities and terms and conditions of the home. This requirement although partially met is oustanding from the inspection on 26/6/02 The Registered Manager to amend the service user guide to include a description of the communal spaces provided, an actual copy of the complaints procedure and the relevant qualifications and experience of the staff, manager and provider etc. The home is referred to Standard 1.2 to ensure compliance with this Standard.
