The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 12 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

People living at this home have a good quality of life. There is a stable staff team who care about the people living in the home and have good relationships with them. There is an experienced team of staff at the home who have the relevant skills and training to support residents to become as independent as possible. The home is good at promoting the individual lifestyles of each resident which is reflected in the range and variety of activities that residents take part in, both outside of the home and inside. Bedrooms reflect the personalities and interests of the residents and there is a homely atmosphere. The home is run well with a wide range of records being kept up to date and accurate.

What has improved since the last inspection?

The bathroom on the ground floor has been refurbished and adapted so that it better meets the needs of residents. A food store has also been renovated. The home is fully staffed and is able to run without the use of agency staff. As a result the home has continued to provide high quality care to the people living there.

The home may accommodate eleven service users of either gender who have a learning disability. Five of these service users may also have a physical disability. They may only be accommodated in the rooms on the ground floor. Date of last inspection 13th October 2005 Brief Description of the Service: 35-37a Solna Road is registered to accommodate eleven adults between the ages of 18 and 65 years who have a learning disability. Five of the eleven people may have a physical disability as well as a learning disability. The ground floor of the home is wheelchair accessible. The home is operated by Robinia Care Limited. The home has been operating for eleven years and was taken over by Robinia Care Limited in 2001. The home is situated in a quiet residential area with access to local shops and buses. The home comprises two semi-detached houses joined together, with five bedrooms on the ground floor and six on the first floor. The residents attend day services and take part in activities organised by the home. All residents have a learning disability, some have additional disabilities such as epilepsy, physical disability and a mental illness. The stated aim of the home is to provide a warm homely environment for eleven adults which respects their independence, protects their status and encourages their individuality, growth and community integration. This inspection was unannounced so nobody at the home knew the inspector would be coming and lasted 6 hours. The inspector looked around the building, met with the deputy manager, spoke to the 5 of the residents who were at home and interviewed a member of care staff. A variety of records, including care plans and health & safety documents, were looked at. A pharmacy inspector also visited and looked at the home’s policies and procedures in relation to medication. The overall impression from the inspection was of a home that is providing a good standard of care within a friendly, homely and supportive environment. All medication obtained or received for service users in the home to be documented and signed for on the administration charts. The administration of all medication administered by staff must be signed for on a medication administration chart or non administration coded as to the reason for non administration at the time of administration. The home to work with the supplying pharmacist to ensure that all discontinued medication is deleted from the service user’s administration chart. The medication history for each service to be kept up to date. The home to ask the service users’ GPs to sign the list of ‘homely remedies that can be administered to the service user for minor ailments for up to 48 hours without consulting the GP. The medicine cupboard to lock securely and to be attached to the wall. The medication not supplied in the nomad boxes for each service user to be put in separate containers in the cupboard. The room temperature where the medication is stored to be monitored and recorded daily and maintained at 25oC or below. A refrigerator for the storage of medication to be available for use if required. The storage conditions for rectal diazepam to be adhered to. The identified resident’s consent to the use of their shower to be recorded. The company recruitment procedure to include verification of references and a copy of this procedure sent to CSCI. An effective and inclusive process for reviewing and improving the care at the home to be implemented. . The findings of the fire servicing check on 23/01/06 to be acted on and CSCI notified. JUDGEMENT – we looked at outcomes for the following standard(s): 2 The home is good at assessing and meeting the needs and wishes of residents. EVIDENCE: No new residents have moved into the home since the inspection in February 2005. At that inspection the inspector saw that the needs of a new resident had been carefully assessed and that his family were fully involved in identifying his needs and wishes before he moved to the home. From discussion with staff and residents it is clear the home responds to and meets the changing needs of residents. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6 Residents benefit from having staff who respond to their changing needs. EVIDENCE: The inspector discussed care plans with the deputy manager and two of the residents and looked at another resident’s plan. The member of staff interviewed had a good understanding of the needs of residents. Residents spoken to indicated that their needs and wishes were being met, with one saying: ‘They look after us. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 Residents have a good quality of life with opportunities to learn new skills, follow their own interests and develop new ones. EVIDENCE: Each resident has a weekly plan of activities. The majority of people go to a day centre during the week either on a full or part time basis. The other residents follow an individual programme of activities organised by Robinia Care locally or by the staff at this home. There is plenty of opportunity for personal development, learning new skills and following a variety of interests. The home organises for residents weekly drama therapy, reflexology, cookery, community awareness activities, an art class as well as a variety of leisure activities. Observation on the day of the inspection together with feedback from residents indicated that they take part in a wider range of activities. The home has continued to maintain a high standard in this area. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20 The home is good at responding to and meeting the health needs of residents. The residents are usually protected by the medicines’ policy and procedures but more care needs to be taken with ensuring that all the documentation is kept up to date. EVIDENCE: At the time of the inspection one resident was in hospital. Their care plan and records were looked at. At their last review it had been noted by the social worker that the home was continuing to meet the health needs of the resident. The records showed that the individual was having regular checks with a range of health professionals. Staff at the home had liaised closely with the individual’s family so that the resident continued to be properly supported whilst in hospital. On the day of the inspection a resident had complained of feeling unwell and this had been promptly followed up by staff who arranged an appointment that day with the GP. The pharmacy inspector made the following comments: The company’s medicines’ policy is complete but there is no local policy on the procurement of medication. According to the staff, none of the service users Robinia Care Homes Ltd (Solna Road) DS0000010657.V279012.R01.S.doc Version 5.1 Page 13 are capable of giving informed consent but there are signed multidisciplinary reviews for each service user which include their current medication. None of the service users are administering their own medication. No signed record for the receipt of medication was available. [Requirement made] The documentation for the administration of medication was generally satisfactory except that two gaps were found in the administration record where it was unclear as to whether the medication had been administered or had been refused by the service user. The medication profiles kept for each service user were not completely up to date. Service users sometimes had their own medication such as vitamins which the GP was aware of and the staff had been asked to administer. There was, however, no record of this administration by staff. [Requirement made]. On the administration charts some discontinued medication had not been removed by the pharmacist. [Requirement made]. There were clear instructions in the home on the use of rectal and oral diazepam. Training in the handling of medication and the use of rectal diazepam had taken place last year. More medication training was being organised for this year. The home does have a list of ‘homely remedies’ but this has not been signed by the service users’ GPs. [Requirement made] The medication cupboard is kept in the hall, the temperature of which is not monitored to ensure it remains at 25oC or below. It is also not fastened to the wall. [Requirement made]. At the time of the visit the area was below 25oC. The medication cupboard was quite untidy with the service users’ medication mixed up together and also it would not completely lock. Also no medication refrigerator was available for use if required. [Requirement made]. The home keeps rectal diazepam which needs to be stored in a cool place. No Controlled Drugs were being kept at the time of the visit. Service users were attending day centres but no medication was required during the time they were at the day centre. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 23 The home has good procedures in place to safeguard monies belonging to the residents. EVIDENCE: A sample of financial records was looked at to see if residents are adequately protected against any risk of financial abuse. These records are up to date, accurate and satisfactory. The finance officer in Robinia Care responsible for residents’ finances was spoken to in order to clarify the new arrangements in place at the home. Residents now have their own accounts into which their benefit monies are paid, including the Christmas bonus, as previously required. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,29 The home is well looked after and comfortable which helps residents to feel relaxed and at home. Residents benefit from having specialist equipment in the home which promotes their independence and safety. EVIDENCE: One of the residents showed the inspector around the home and took pride in pointing out how she had made her bedroom her own with family photos and other personal items. The home is well decorated and furnished, with a good standard of cleanliness achieved. Those residents who have a physical disability have appropriate equipment to help them such as a ceiling hoist and walking aid. The ground floor bathroom was in the process of being refurbished. As previously required this is being carried out in line with the guidance from the occupational therapist. As a result the residents will benefit from a bathroom which is better adapted to their needs and safer to use. Whilst this bathroom was being refurbished some residents were using the en suite shower in the downstairs bedroom of one of the other residents. However this had not been recorded. The manager must ensure that this is done to provide evidence that this resident had been happy for others to be using her shower. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34 Staff in the home are sufficiently experienced, competent and qualified to meet the individual needs and wishes of the current group of residents. Improvements need to be made in recruitment practice in order to protect residents. EVIDENCE: There is a stable staff team who benefit from a comprehensive training programme. Most staff have completed or are currently undertaking NVQ Level 3 training in the care of adults. Staffing levels vary according to the needs of residents. Staff receive regular supervision. Staff interviewed showed that they had a good understanding of the needs of the residents and a commitment to good practice. As a result residents get the benefit of being supported by staff who are aware of what they need and how to get these needs met. One resident said: ‘The staff are good at helping me.’ A requirement had been made at previous inspections that Robinia must ensure a thorough check of the authenticity of staff references before a new person commences employment. The absence of relevant staff files meant that it could not be verified whether this requirement had been met. Service users are confident their views underpin all self-monitoring, review and development by the home. JUDGEMENT – we looked at outcomes for the following standard(s): 37,39,42 Residents benefit from a well run home. The health and safety of residents and staff is promoted. However safety procedures need to be efficient at all times. The process in the home for reviewing and improving the quality of care needs to be strengthened. EVIDENCE: The manager has completed her Registered Managers’ Award training. Both the manager and deputy have worked at the home for many years and have built up considerable experience and expertise in running the home well. Feedback from both residents and staff was positive about what it is like both to live in and work at Solna Road. There is a happy atmosphere in the home as a result of the good relationships staff have developed with residents. Residents’ meetings are held two-monthly and give residents an opportunity to make suggestions about how the home is run, including outings to be made and activities in the home. However there was insufficient evidence that in the home there is a thorough process for reviewing and improving the service Robinia Care Homes Ltd (Solna Road) DS0000010657.V279012.R01.S.doc Version 5.1 Page 20 which focuses on the views and wishes of the residents’ themselves and other key parties, such as families and professionals. The monthly regulation 26 visits and the self-assessment done on the day of the inspection are not sufficiently critical to provide evidence that Robinia Care and the home have a robust process for evaluating the service provided at Solna Road. The registered persons must make sure there is an effective and inclusive process for reviewing and improving the care at the home. Some health and safety issues were inspected. As previously required fire doors are not blocked open and staff had been reminded about this at a staff meeting. The routine servicing of the fire equipment took place on the day of the inspection and identified some possible faults in the home’s electrical systems. The manager must ensure that these faults are put right and CSCI notified of the action taken and outcome. A cleaning cupboard which is to be locked when not in use was seen to be open. The manager must ensure this cupboard is kept locked when not in use.
