Conditions of registration: None Date of last inspection 6 January 2005 Brief Description of the Service: This home is one of three in North London operated by Robinia Care Group Ltd, who have a number of care homes throughout the UK. This home is a service for five people who have an autistic spectrum disorder. The service user guide for the home desribes it as a home environment to support people with Autistic Spectrum Disorder and Aspergers Syndrome, moderate learning disability and behaviours that challenge. There are five men living at the home. In practice, most do not have a learning disability but do have an autistic spectrum disorder. The home encourages people to gain independence skills with a view to living more independently in the long-term. This is an overview of what the inspector found during the inspection. This was the second inspection of this home which was registered a year ago. The home did not know the inspector was coming. The inspector was able to meet with four of the five residents of the home, the deputy manager and a support worker who were on duty. The fifth resident chose not to meet the inspector. Apart from speaking with residents and staff, the inspection involved a tour of all the communal rooms, checking up that the home had completed the things they were asked to do at the last inspection in December 2004 and checking some of the home’s records and procedures. What the service does well: What has improved since the last inspection?
The manager has written a simple step by step procedure for staff so that they know what to do if they have any suspicion that a resident may have been abused in any way. The manager was asked to do this by the inspector as the company procedure was too difficult for some staff to understand. A clear complaints procedure has been written, making it easier for people to understand how to make a complaint to Robinia Care, if they need to. The home has bought a new garden shed. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2 Prospective residents have clear written information about the home which they can read before deciding if it is suitable for them. Their individual needs and aspirations are assessed and they can be assured that the home will try to meet their needs. EVIDENCE: The statement of purpose for the home meets the required standard. This document has been updated since the last inspection. The service user guide was assessed at the previous inspection and is written in plain English with pictures and photographs to make it easier to understand. Three residents’ files were checked for evidence of assessments. All three contained assessments of the person’s needs and aspirations. Their needs have been assessed regularly and records updated. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 8, 9, 10 Residents at this home have their individual needs recorded and reviewed regularly and they are fully involved in this process. They are consulted about all aspects of life in the home and confidentiality is maintained. Residents are supported to be independent and take risks and a full assessment of any risks is carried out with them. EVIDENCE: Three care plans were inspected along with risk assessments, activity plans and records of reviews. The care plan for each resident consists of a number of support plans; one for each of their identified needs. The resident signs the plan as evidence that they agree to it and any restrictions placed on them. The care plans seen were comprehensive and had been regularly reviewed and changed . Residents are encouraged to make decisions for themselves and are provided with the level of support they need. This ranges from support to eat a meal to emotional support. Risk assessments were reviewed in March 2005. These

There are monthly house meetings where residents are consulted about the running of the home and any matters relating to group living. They are also consulted about decisions affecting them on an individual basis. The inspector looked at the two reviews held for one resident and saw that goals set with him had been reviewed and changed as necessary. This is very positive. Another resident had a review after living at the home for three months. The company has recently made improvements to residents’ confidentiality by requesting that their initials are recorded on incident sheets rather than their full names. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14, 15, 17 Residents are encouraged to live a full life, using community facilities and taking part in educational and leisure activities of their choice. Staff encourage each individual to lead a good quality of life. EVIDENCE: On the morning of this inspection, four of the five residents were taking part in a weekly videomaking session within the home. They were wearing costumes and performing a play which they said they were enjoying. Each resident shops for their own food and cooks their own meals, with staff support. They all go out independently. One person goes to football matches and another is doing work experience and applying for jobs. There were also records that residents had the opportunity to take part in personal development activities relating to their social and emotional needs. Residents follow their own interests and have a written weekly activity plan. These include independent living skills such as cooking, laundry, travel training and other activities such as drama therapy, pottery, meals out, cinema and

All residents are encouraged to further their education at local colleges and/or seek employment. They use community facilities such as local pubs, church and colleges. Residents choose the level of contact they want with family members. All have contract with family or friends outside the home. Three residents are planning to go to Cyprus on holiday later this month with staff. Meals are not provided by the home but each resident is supported to plan a weekly menu and offered guidance on their diet. The inspector was told by a resident and a staff member that staff encourage healthy eating. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20 Residents’ physical and emotional health needs are met. They can self medicate if they are able, but otherwise are supported by the home’s medication policies. EVIDENCE: Three residents’ files were checked to see if they had attended medical appointments. Each file detailed appointments attended and showed evidence that the home was supporting residents with meeting their healthcare needs. One resident refused to go to the dentist and had signed that he refused to go. Staff monitor one resident’s weight as this is seen as necessary. One resident has been referred to a dietician. There are opportunities for residents to access sex education if they wish to. One resident said that his medication had been reduced since the last inspection and he was feeling much better since the change. All residents see a psychiatrist on a regular basis. The others have signed their consent to medication and are given prescribed medication by staff. The medication cabinet and sample of medication records were inspected and found to be satisfactory. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22, 23 Residents’ views are listened to and acted on. A new procedure on adult protection will help staff to respond appropriately to any possible concerns about abuse. EVIDENCE: There has been one complaint made in the last year by residents and the manager investigated and recorded the outcome appropriately. The complaints procedure has been included in the home’s statement of purpose. In the absence of a clear procedure from Robinia Care (a requirement has been made on this matter in December 2004), the manager has written an adult protection procedure for this home so that staff have clear guidance on what to do if they suspect a resident has been abused or if a disclosure is made to them. All staff have signed the procedure. The inspector has been notified of incidents of self harm in the past year. The deputy manager informed the inspector of what action was being taken in this area and the inspector was satisfied that the issue was taken seriously and acted on appropriately. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 30 The home is comfortable, homely and safe with a good level of cleanliness, making it a pleasant place to live. EVIDENCE: The inspector looked at the lounge, two kitchens, toilet, bathroom, office and garden. No bedrooms were inspected on this occasion. Residents will be asked if their bedrooms can be seen at the next inspection in December 2005. The standard of furniture and decor throughout the home was good. The level of cleanliness was also good. One resident told the inspector that he enjoyed cleaning the kitchen and helped the staff with their cleaning duties. There are two kitchens for the five residents. Three share one and two share the other. Each person has cupboard and fridge space for their own food. Both kitchens were very hygienic. There were no fire hazards in the home. The garden was well maintained and a shed has been purchased recently. The house was very homely and comfortable. Two residents said they liked the house. One said he would like it to be a bit bigger as this was his own preference.

Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35 Residents benefit from a staff team who have been appropriately trained for the work and who are familiar with their particular needs. EVIDENCE: The inspector was unable to look at staff files for evidence of recruitment practice and supervision records as the manager was not at the home and the staff files are kept locked away. No new staff have been recruited since the last inspection. One staff has transferred from another Robinia home to this one. Two staff have completed NVQ 3 training and a third is awaiting her certificate. Five staff are currently undertaking this training. This is very positive. The staff training programme is of a good standard and relevant to the needs of residents of the home. All but two staff have attended training in working with people who have Aspergers syndrome. The two who have missed this training will be attending it later this month. The deputy manager said that all staff receive individual supervision on a monthly basis and the four senior staff carry out the supervision. Supervision records were not inspected on this occasion. JUDGEMENT – we looked at outcomes for standard(s) 42 Residents are protected by good practice in health and safety. EVIDENCE: At this inspection, fire safety was addressed. Fire extinguishers were checked for safety and a sample of fire doors were checked to see if they close properly. Fire extinguishers had been serviced by the supplier and fire doors fitted and self closed. Staff have been trained in fire safety and all residents have also attended this training which is commendable. There were no health and safety hazards noted. Risks to the health and safety of residents were documented and acted on.
